Yes, you're correct! In the following year you can either apply for Advanced or Physician R programs (which start in the year of the Match), or you can apply for a categorical specialty and repeat your PGY1.

You can apply to categorical neurology programs as well as preliminary IM programs combined with applications to Advanced neurology programs. We suggest contacting the ACGME regarding whether or not Transitional Year programs offer a path to Advanced Neurology tracks: accreditation@acgme.org
Since EM is not an Advanced Training program that requires a PGY1 before beginning, it is very likely that you would need to repeat the PGY1 unless somehow you are able to Match directly into a PGY2 position. You may wish to call EM programs to find out how they would deal with situations like this.
What about clinical genetics, they ask 1 or 2 years before entering the 2 years advanced program. Or clinical genetics is considered a fellowship? I think not.

I am currently a 3rd year senior resident at department of orthopedics. I have one research publication. Step1- pass, step 2- 243. Which programs should i go for? Would applying to orthopedic residency be foolish?
I want to match into general surgery and then afterwards i want to go to cardiothoracic surgery. So in my case, should i plan to join a preliminary general surgery residency or a categorical general surgery residency?

It is possible to get from GS to NS. However, this is best accomplished if you Match into a program which offers both training. That way, you may be able to easily switch to the other residency track based on your elite performance in GS. However, switching to an entirely different hospital from GS to NS as a non-US IMG may be very difficult. For trauma surgery, "Trauma surgeons generally complete residency training in General Surgery and often fellowship training in trauma or surgical critical care." Yes, you do get paid for these residency positions. You can look up individual programs for that information! Yes, you do have to do residency training in the US in order to practice medicine, regardless of your previous overseas experience.

If you are applying to advanced programs, you can apply to both your prelim/TY program and advanced program during the same Match.

Yes, it is common for applicants to apply for both in the same year. We hope you Match into your Categorical of choice!

It is okay to apply to both. However, applying to only categorical - if that is your goal - may be preferable for those with both tracks.

is doing a transitional year good to get a PGY-2 in psychiatry?
I believe psychiatry is mostly categorical programs, so you'll have to see if getting a PGY-2 position is possible.
