    The septum is the wall that separates the 2 sides of your nose
    Pink mucosa (skin) covering cartilate and bone
    When its not straight = deviated
    Can de deviated from birth, any trauma
    Can narrow or block one or both sides of the nose
    Can lead to stuffy nose, trouble breathing
    Can contribute to apnea
    Can also indirectly worsen sinus issues
    Sometimes can cause crusting, scabbing, bleeding
    Septoplasty = relatively simple surgery to straighten the septum
    Requires anesthesia
    Can affect the tip of the nose and nostril shape if deviated all the way to the front
    Minimal or no swelling, even that night (unless other procedures done at same time)
    Can greatly improve quality of life
    Not every little bend needs to be straightened

    Dr. Liu I didn't know what a deviated septum was. I was browsing UA-cam and this video about it appeared in my recommendations. I believe I have this condition: restricted airflow in one nostril (left), nose bleeds when I was a child, a consistent buildup of phlegm in the affected nostril and during strenuous exercise the need to open my mouth to breathe properly. I have lived with this for decades. Thanks for your video.

      @@ilqar887 i have deviated septum i cant breathe through my nose sometimes (also cat allergies witch worsen it) and occasional but really bad headaches (usually in the afternoon.) I rly want the surgery but i have to figure out if itll help.

    After being tired of not being able to breathe at all from my left nostril it has impacted my sleep and is extremely annoying 24/7. I looked just inside my nostrils with my flashlight and a magnifying mirror and the nostril that I can breathe from is open and I can see the hole but the other side is completely closed up where there’s barely a tiny sliver that opens when I try the push air from it super hard. Definitely looks like the sever photo you showed. Thank you.

    Just a question, I saw a procedure video. If the septal cartilage is removed and not replaced, would that not make the nose weaker or collapse in the future since there is no support?

    When a piece of the septum is removed during surgery and then they place some of it back inside will it regenerate completely and become one whole piece again? Or if it’s removed, will your body regenerate it?

    Hi. Great Video. If you have symptoms like breathing through your mouth 80% of the time while sleeping, and even during the day; regular dry and sore throat; post nasal dripping while lying down, and blocked, dry sinuses, could even a slightly-deviated septum be contributing to these symptoms?

    Dr Liu - do you perform septoplasty under general or IV sedation? Also do you add splints or gauzes? I have read several surgeons don't add splints / gauzes anymore and only have patients with dissolvable sutures.

    I need to do this... I tried radiofreqency ablation.. Has been a week and i cant breathe any better. They said it shrinks a little. My deviated septum is too large.
    Need this after i have healed and still cant breathe on the left side.

    I think I have a deviated Septum, but I also have a hole in the centre of my septum due to years of using nasal sprays and constant blowing and removal of dry scabs and buggers. I am worried about going to the doctors as I don’t want a big operation on my nose/face but it pretty much wakes me up every night when I am unable to breathe.
    The hole in my septum doesn’t bother me but would that make the process of correcting my deviated septum more difficult?

    Hi, i have question about nose breathing, cause i am breathing from the mouth mostly and have headaches almost everydays so i have this more than 5 years, can this be fixed through nose surgery and what can happen if not done surgery?

    Hello Gene, I was seen by a Otologist and they recommend surgery, septoplasty and turbinoplasty so those two procedures will not remove all of the swollen tissue just a portion? In one of your videos you said it be better only to remove a little bone or tissue and each for each person it would be different. Could you briefly explain the two procedures septoplasy and turbinoplasty? Also I had a second opinion from another doctor she looked at my exams and recommended the surgery and said there are no other options because it's severe, chronic sinusitis. Also are any of these procedures risky during this covid-19 pandemia. I'm really leaning towards having these both procedures done because its been I think years since I've had a good nights rest. Thanks

    Great video, thank you for posting. I have a deviated septum and also get sinus infections every now and then. I work out a lot, but notice my cardio conditioning is not as good as it should be. Can the deviated septum result into poor conditioning because I’m not getting all the air I should be getting?

      A deviated septum can limit your exercise tolerance...but most are not that severe/significant. Your septum has likely been the same for years (unless you've had recent trauma). So if the cardio conditioning issue is worsening, and the septum has been the same, it would hard to say that one caused the other.

    I have a deviated septum and I will often get stuffy when lying down and can't breathe through one of my nasal cavities. I also always have a slightly runny nose especially when its cold. Are these symptoms that would warrant surgery? Also, the side of my nose where the septum veers to (left nostril) is slightly convex. Whereas the right side of my nose is slightly concave. Would surgery correct this asymmetry, making the convex side more concave like the right side?

    I don't know if anyone will be able to answer this but will medical aid cover this? Parts of my turbinatus were removed at 7 but my septum still causes me a lot of trouble breathing. Xrays from when I had braces also clearly shows that my one nostril is basically closed off.

    Is it normal that if you try to breathe through your nose(like try to blow air through)it automatically comes through your mouth? I have that and I cant breathe through the right side at all, a little bit through the left. I'm terrified of doctors and I've had this issue all my life, it makes going to a dentist tough because I need to breathe through my mouth.

    I have this my septum is pretty bad I went to the hospital when I was 15 years old and the doctor told me to wait till you’re 17 years old to full grow out because it may get better and you won’t need to have surgery. Now I am 16 years old nothing changed it’s still bad 5 months till I enter 17 so I’m thinking about going to the hospital tomorrow or soon because this is real pain I can’t breathe well at all.

    Does a septoplasty cause noticeable scar tissue? I had a rhinoplasty three years ago but I still have a deviated septum and lots of issues due to it. I had significant scar tissue from my rhinoplasty and I'm worried about more forming after a septoplasty.

    Has either the surgery procedure itself or the recovery improved lately, as of 2019? I watch older videos where the patient says they have to return to the office a week later to have their nose packs removed. In other videos I hear 2 days. Are they both different visits for different purposes?

      In years past, almost all doctors placed splints or packs in almost every patient. Nowadays, many docs don't place splints, and very few use packing. Even nowadays, though, depending on the anatomy and the exact nature of the individual surgery, and surgeon preference, there may still be packing or splints utilized.

    I woke up not being able to breathe from one nostril. Landed on this video with my symptoms (unable to breathe, extra mucus, post nasal dripping) , but I don’t think it’s severe enough to go through surgery.....is there anything else that can help me?     I had septoplasty and sinus surgery in 2010 and stupid me I bumped my nose a couple months ago and since then I have not been able to breathe properly and Lots of stuffy nose since then. It was not like this before bumping my nose and after my surgery. Now I Can’t breathe :( and seems like allergy is worst now because whatever allergen gets caught and doesn’t drain out properly now therefore a lot of sneezing, something that I didn’t have after my sinus and septo in 2010. Even when I do the nasal rinse it doesn’t flow out like it used to and when I blow from one nostril to clear the sinus rinse out, it actually blows back into my right ear. I feel miserable. I don’t know if anyone will touch me bc I already had the surgery. Please advise. I’m out of options. Saw my ENT and he says things look clear he can see right into my sinus, but I know it’s deviated, I just know it. I know my body when there is a change. Is there a way that I massage it back to straight?

    I am sure this is a basic question, but is it possible to self diagnose a deviated septum or is that really only if you can visually see your nose is crooked? What is involved in diagnosing it?

      The septum goes from near the front of the nose all the way through the middle of your head. Looking at the front of the nose and just inside your nostril can reveal if that part of the septum is deviated or not, but tells you nothing about the septum further inside.

    My mum had this surgery and reckons I will need it as I have a very hard time breathing but I don’t want my appearance of my nose to change cuz I’ve just kind of gotten comfortable and accepting of my nose I like my nose and I don’t want this surgery to change it but I realy need to breathe I also want a septum piercing I’d that somthing I can get after the surgery

    I snore and my husband thinks I may have a deviated septum. My nose doesn’t loom crooked to me but I did have an injury 20+ years ago....my young son hit my nose with a golf club. How do you determine if you have a deviated septu?

      Just because the nose is not obviously crooked on the outside tells you nothing about the septum as it goes through the middle of your head...it may well be deviated on the inside. The only way is to look through that entire passage. An ENT doc can easily tell you during a consultation.

    Sir I hv a left DNS slightly ...2 months ago I did hot steam through my nose and my both inferior turbinate were swelling. so my doctor prescribes montelukast 10mg table n nasal spray for 1 month.but it didn't work well...now i m starting medicine (montelukast 10mg) n metered dose nasal spray(antazol plus spray )for about 1 month again. Same things happen.I can't breathe properly through my nose. always my one or two nose block,dryness n can't sleep properly. plz suggest

    Sir, I hope u will read this and reply me I really need some help and answers on this Im suffering from this I had an accident years ago, and it cut my nose made it swollen like so big the swollen healed a lil bit after somedays but the swollen gets back whenever i touched my nose, and until rn the swelling keeps getting bigger whenver i touch it, I can feel the tingling and pain in my nose, my nasal congestion wont stop almost everytime I have it and I have a trouble breathing i cant sleep well altho the cut is healed the injury is still there, can u answer me doc what might it be 😞 i really need help plss..I also have headaches daily

    The doc also said that after she remove the deviated spur...she will just left it and close the wound with the tissue that cover the septum.is this procedure can affect my external nose appearance?as there is no implant???

      Usually not, but it depends on which part of the septum is deviated. There are always things that can be done to keep the nose appearance unchanged.

      There are non-surgical options to work around a deviated septum to improve breathing, but the only way to actually straighten the deviated cartilage and bone is with some sort of procedure/surgery.

    Doc , i live a world away. Could you have a look at my CT scan and let me know if surgery is right thing for me. The doctor in my country is suggesting surgery for a deviated septum. My sinuses only get completely blocked while sleeping , I'm afraid of complications later after the surgery.

      A full consult reviewing not just the ENT history, but also your general health history, as well as the CT scan, and also a scope evaluation of the nose is usually required to render a medical opinion. There is no great way at this time to do this "a world away." I would suggest another local opinion to confirm or refute the first opinion if you are unsure.

    Besides,i would like to do the septoplasty but the doc told me that i'll get bigger nose or even sadden nose!!!what is ur suggestion and opinion to my ent doc in order to prevent it from occur.cuz I love my current nose size as it's perfectly match my face...i really need ur opinion doc cuz this deviated septum makes my breathing not optimum.and one more thing my ENT is in the government hospital in which they don't want to burden them too much...hopefully that u could suggest a simple procedure for me to suggest to my ent doc that might be helping to keep my nose in shape...

      Most septal deviations are all internal and surgery should almost never change anything about the external appearance of the nose. In some situations, the deviation does extend all the way to the front where there could be a change in the appearance, but this usually makes things straighter.
      The nose should never get "bigger" unless you are intentionally trying to make it that way. A possible complication of any septum surgery is a saddle nose, but it should be exceedingly rare...and is usually in someone who has had significant prior trauma, multiple surgeries, or some underlying medical condition that affects wound healing.

    hi doc i have rosai -dorfman disease , a nasal mass cavity last year june 2018 a have a operation nasal mass totaly block my nose after 6 months the mass return

      That's definitely a rare problem to have, and recurrence of the lesion is definitely a possibility. Whether you get a removal again or pursue other treatment options, I wish you the best of luck in managing this diagnosis.

    Hye my lovely Doc,i had see my doctor couple of weeks ago after my inferior turbinate is cut in (turbinoplasty) because of turbinatehypertrophy.During the checkup with the camera inside my nose,the doc said that my deviated septum in my right nose is only a bit.therefore, she said i dont need to do the septoplasty.But i don't understand why my right nose is usually congested throughout the day???how i should tell my doctor???do u know how to talk with the doc???it becomes very worse at night before sleep that makes me not comfortable to sleep well.

    Sir I had a septoplasty and turbinate reduction surgery 4 days ago .. i had lost my sense of olfaction for about 95% , after removal of packing surgeon suggested me liquid paraffin to keep my nose slimy after few days of liq paraffin .. i cant smell that good..
    Taste is good, absolutely okay no dysgeusia but Hyposmia is present.

    Just got a kick to the face playing rugby my right nostril is hard to breath through and I feel a notch in the cartilage area I'm a tiny bit afraid of surgery but might as well do it

      Going to get my surgery tomorrow. Bit scared but honestly the benefit as far as sleeping etc just general quality of life is well worth the very minimal risk. Do research on your surgeon and you'll be just fine.

    Hello Doctor just a question, my.husband has suffered for 2 years with.nosebleeds which have been corterized every time.at the hospital...we have been told now that he needs a Septoplasty to stop this frequent bleeding...is this correct? Thanks in.advance for your opinion.

      Sometimes, when a septum is deviated, the mucosa covering the area that sticks out gets easily dry and irritated. (think chapped lips). If it keeps happening in that same spot, sometimes it makes sense to straighten the septum so that the drying effects from the airflow is more spread along the entire septum...not just the area that sticks out.

    Got a septoplasty 10 years ago and it didn't help at all. It was painful and I was quite uncomfortable for a long while definitely not worth it. Doctor cut a piece of cartilage out dropped it in my mouth by mistake and that was it. Did nothing to help. Got it done in Taiwan. Couldn't breath through my nose for weeks and after not any better..
    Quite skeptical to try again. Saw something about laser surgery for this. What do you think the future is for correcting a deviated septum?

      Well if it didn't help at all + doctor dropped cartilage in your mouth then you should of made a complaint!
      As that doesn't sound right at all-where are you from??

    Sir, I have a deviated septum, turbinate hypertroph and sinusitis, I can't sleep at nights and conditions are worsening. I have consulted many doctors most of them suggested surgery, but one doctor told me that surgery not required. I want ur recommendation sir. Thank u

      It's impossible for me to say without a formal evaluation and actually looking into your nose/sinuses with scope, scan, and full history. You have to go with the recommendations of the people you've seen.

    Doctor if my nose is crooked caused by a deviated septum will septoplasty help and also i cannot breathe from one nostril ( im getting my surgery by dr mc alpin tomorrow)

      In some people, septoplasty is the only thing that needs to be done. In others, there may be other procedures (ex: rhinoplasty) that may also need to be done.

    Does it change the length, shape and size of the nose? I am going to do it for the medical reasons, but i like the shape and size of my current nose. Is it possible to have Septoplasty that will keep my nose's length, shape and size intact(just remove the problem of course)?

      The vast majority of septum surgeries are focused on the internal portions which would result in zero change in the size and shape. Some people require the very front portion of the septum straightened, and this could influence the part of the tip of the nose that separates the nostrils. Definitely something to make sure you discuss with your surgeon so that he or she is aware of your concerns.

      @@georgivelev5783 "jaw exercises" can make the masseter muscle bigger and potentially make the jawline more defined...but it's not actually changing the jaw. It's just bulking up the muscle. Braces can straighten the teeth, but also don't change the jaw shape.

      Hi Dr Liu, I am considering septoplasty due to DS and constant stuffiness, and also Clarifix for extremely runny nose in cool-cold weather. My septum is too large for the Clarifix procedure so my ENT might do the Clarifix during the septiplasty. What are your thoughts on Clarifix? I haven’t found any 3rd party reviews. I’d love to reduce the runny nose, particularly for skiing, walking, biking in cold weather. But I am a bit concerned about potential side-effects of cryo-freezing the nerve, e.g. can it make the nose too dry? Thanks in advance!

      @@jaxbchjim I don't have personal experience with the Clarifix. The studies are all small in number and results relatively short term. The medical literature, however, does seem quite promising and I do know some surgeons who have had early experiences that are positive. The person best suited to discuss the risks/benefits/alternatives if your surgeon.
      Specifically, dryness and empty nose is a risk with any nasal surgery. The more aggressive the surgery, the higher the risk.

      Impossible to say without more information. Many things can cause sinus issues - go to my sinus videos and see if there is anything helpful there. This video is really only about the septum.

      That is certainly not the goal for any surgery. Although there is always a chance this could happen, usually the likelihood is low.

      If both sides of the nose are balanced, then the nasal cycle should result in breathing that switches back and forth without any symptoms. If both nostrils are narrow or small, then the 2 sides will take turns feeling stuffy. If one side is inherently narrow (like from a deviated septum), then that one side may always feel blocked compared to the other side.

    "Improve quality of life" that's the biggest caveat and it seems to me and I'm giving my informed opinion, is that they seem to use the same sentence to bait and switch just like a telemarketer scammer. DO NOT TRUST ENT DOCTORS WITH YOUR NOSE.

    I had the surgery about two years ago and am no better than before I started. In fact I feel like I'm worse. How could this be?

      There are many possibilities. Sometimes the septum was not the main issue in the first place. Other times, it didn't heal well or became deviated again. It's also possible that the septum surgery went well, but you have since developed allergies, nasal polyps, or other unrelated reasons to have trouble breathing. The only way to tell what is going on with you is to see an ENT.

      @@anitas.8808 Sorry to hear. Did you get hit in the nose again? Sometimes even trivial trauma in the recovery can cause this. Other times it may just not heal well. Even when we put a cast on a broken leg (for example), we can line everything up perfectly, and the body still needs to heal the bone.

      Not everyone that has a deviated septum needs surgery. And if the problems that it causes are serious enough, it may make sense to have a procedure for some people in order to avoid more serious issues later (ex: someone with sleep apnea who cannot tolerate CPAP).

      Gene Liu, MD, MMM I broke my nose when I was around 6 now I’m 11 my left nostril is completely blocked I can let air out of it but not in it. It is also bigger than my right nostril what would they have to do for me

      @@Holly-px5ki Had the surgery but it was quite hard. Septoplasty was not enough. They did 3x RFA and they removed some tissue too. Had to spend 1 night at hospital because they gave me so strong pain killers that it made me vomit. When I got my splints removed it wasn't nice at all. He took them fast to make it quick and it hurt.... oh it hurts so much. I almost lost my
      Consciousness. But now it heals quite well. They had to put some anesthetic to my nostrils to minimize the pain.
      Also the tampons removal was not so good. It did not hurt that much but felt so bad when they pulled them. But I think its worth it.
