Your accuracy math is missing the enemy's dodge chance, which is
(AGI/5 + LC/10 + Sanctuary) * [0.75-1.25]
That number gets subtracted from your accuracy, so if the difference is too huge your 45% could be reduced almost to zero.. against an enemy with high stats such as vivec, that's very likely.

Have you played Morroblivion? I had a lot of fun with it! It's a little uncanny, but it has the fun extra challenge of enemies being able to follow you through doors.

57:00 I'd honestly argue that Souls games aren't even *that* hard, but they're good at making you feel like you've overcome an insane challenge. They've gotten extremely good at three things:
- Harnessing the catharsis of beating a tough encounter without actively inflicting CBT on the player
- Making deaths funny enough & not punishing enough that you don't hate the game when it happens
- Convincing the player that they've succeeded on their own terms (hard to describe this one; think about how a mobile game might have you roll their gacha as part of the tutorial, and you get some SSS thing, and you feel elated, and then the tutorial text actively mentions what you got, and you realize "Oh, that's predetermined, well okay." FromSoft's really good at avoiding this feeling.)
Sekiro is probably the most genuinely hard FromSoft game, because you largely can't grind or cheese through it; it's all tightly-designed combat encounters based around a limited set of tools.

The Soul Trap cheat was basically console commands for those playing on console, which was a much more likely case when this game first came out than it is these days, when most laptops should have little issue with it. :)

Man, after finishing the last dozen or so Achievements in Skyrim SE and my first Morrowind playthrough, I have to admit Oblivion was the worst of the 3 Bethesda TES games. Or at least the most boring. I mean going into Morrowind somewhat blind was fun, and Skyrim SE with some QoL and character mods was just what I needed after how crap this year's been so far.
