Well the loss of high notes was unfortunately starting as soon as 1987. You can hear it here during the start of Fugazi. He used to mask it either by whispering part of the lyrics or going full octave down (in case of Incubus). It became more apparent by 1993 - the version of He Knows You Know on Sushi shows it quite clearly. By 1997, the voice has totally changed. I like to think about it that Fish gave all his voice to us, fans, which is the biggest sacrifice a singer can do. He sang his heart out so often and so much that the voice simply could not hold for a long time.
