In order to charge for the plugin or addon, they will have to include assets for it. This assets are what you in reality are paying for. This goes for all addons developed for Blender. According to the GNU license rules, the developers of these addons need to provide the free version on github ( a stripped version) or similar. Some of these fail to comply by this rule. I am not sure about the developers of SCATTER, but I can't find anything on their site.

No, the thumbnail is not rendered in blender, but as you asked, it is not a very difficult task to scatter that massive amount of grass, you can do that easily using scatter add-on and proxy system (that comes with scatter add-on) will help you optimize your viewport. and there are many other techniques to do this, if you want, I can make a complete video discussing this

I totally built a Christmas scene with animated train, Santa Claus, snow and wind blowing trees and then rendered the entire 2 min animation and this video still was going..

No, the thumbnail is not rendered in blender, but as you asked, it is not a very difficult task to scatter that massive amount of grass, you can do that easily using scatter add-on and proxy system (that comes with scatter add-on) will help you optimize your viewport. and there are many other techniques to do this, if you want, I can make a complete video discussing this
