Adult ADHD Diagnosis and Treatment Review Psychiatry UK 1 Year On

  • This is my review of the whole process from referral to diagnosis, treatment, and nearly a year of taking stimulants for ADHD.
If you think you have ADHD your GP can refer you for an ADHD assessment with your local provider or you can choose to go down the private route. NHS waiting times across the UK, especially in England, can take anything up to FIVE YEARS in some areas, with the majority of cases taking at least two years just to get an initial assessment.
    **April 2022 Update**
Psychiatry-UK is fully booked until at least October 2022 and is currently not taking bookings. The clinic will get in touch with referred patients as soon as possible, and they aim to schedule new clients in chronological order of the date their referrals were received. Their website is updated on a regular basis with new information.
Since posting this video a number of other clinics have been used through Right To Choose in England. The ADHD UK website has some useful information, including a letter you can send to your GP if your Right To Choose is declined.
    Find out more at adhduk.co.uk/r...
After waiting for two years for ADHD treatment through the NHS in England I eventually received treatment from Psychiatry-UK, a psychiatry service providing Adult ADHD Diagnosis and ADHD treatments virtually.
My own ADHD diagnosis in early childhood is in my medical records but the local ADHD provision rejected my referral to their service after several attempts by my doctor, because they would not re-diagnose ADHD and only treat patients that have been diagnosed by their service, leaving me in limbo, and potentially thousands of others.
Private ADHD treatment in the UK is relatively expensive, and not always accessible. I asked my GP to refer me to Psychiatry-UK (P-UK) for assessment and treatment under The NHS Choice Framework which gives you the Right To Choose your treatment provider. My GP agreed and the local commissioners agreed to fund my treatment.
Within two weeks of the GP referral, I had a virtual appointment with a psychiatrist specialising in ADHD and received a diagnosis (again!) of ADHD. Just having this in writing was a relief and a huge weight off my shoulders. The titration process began a week or two later and once I found a medication that seemed to help, my GP agreed to a shared care agreement so that I can now get prescriptions easily and via the NHS instead of paying hundreds of pounds each month.
I hope this has been useful. See some links below to find out what ADDtrepreneur is about and to learn more about adult ADHD in the UK.

True, but consider how much more lousy you'd feel if hadn't got diagnosed for couple of years even longer.
You've got a diagnosis which means you can start getting treatment.
I've yet to get even that far. You're doing well.
Never forget that some 90% of ADHD sufferers don't even know they've got it.
      I'm very jealous of you.
      Good on you.

      Hi Neill,
It's a common theme amongst recovering alcoholics, *everyone* wishes that they'd sobered up sooner. Everyone.
... but you slowly come around to the notion that your sobriety date is your sobriety date and you learn to do what you can with what you've got.
Stephen King says it, "at some point you've got to look in the mirror and decide what to make of yourself".
You've got a diagnosis and, reading between the lines, getting treatment.
I wish I could make such a boast.
      Y'know.
Dr. Barkley says it beautifully... "The side effects of medication *are* real, but so too are the effects of untreated ADHD: a life lived below your potential, substance abuse and addiction, driving whilst intoxicated... the list goes on."
I cannot tell how much I envy you.

So grateful for this video, I'm a student and don't make a lot of money but I was willing to save up money for as long as it takes to get a diagnosis from psychiatry UK, but finding out you can get it funded by a CCG gives me hope that I might get the life changing diagnosis I've always wanted! I don't care if it takes 6 months or a few years, I'm just dying to be diagnosed so I can put this to rest

I'm 28 and after struggling with education and jobs for my entire life, despite feeling intelligent, maybe even above average intelligence, I decied to applyfor an ADHD assessment about 2 months ago through my GP, selecting psychiatry UK through the 'right to choose' scheme. I was told waiting lists are upto 2 years, however I received a phone call 2 days ago and scheduled an appointment for the 4th November, only 2 weeks or so from now. So despite them telling you about long waiting times, for me it only took 2 or so months. I've read similar accounts from people on Reddit and youtube comments. Good luck and hopefully you get seen sooner rather than later!

Thanks for your comment. One thing I can't get my head around is why when we've had a diagnosis as a child do we then need to go onto a waiting list to be diagnosed again as an adult? Surely the diagnosis should just carry over? I hope you manage to get help and support soon.
I know Psychiatry-UK now has massive waiting lists but I would definitely recommend looking for other private providers and asking if they would support your treatment being funded by the NHS. Then write to your CCG and ask them to fund it. Good luck!

For me, Concerta has been massive and the first time I've seen what neurological people must experience. The analogy of it being like the equivalent of prescription glasses, but for your brain has rung very true for me.
I have continued to have some caffeine and stick to my strategies that I know work, as well as making sure self-care, sleep is on point and I'm having Omega 3 too. I was doing a ton of stuff before i was diagnosed and medicated, so I had great foundations in place to give it the best chance. I titrated up from 18mg right through up to 54mg in the end. Have been on it 9 months and so pleased with it.

Only now at 44 do I reach out for help again. But I’ve made a mess of that as well after telling a doctor recently of my symptoms now he thinks I have anxiety.

Thank you for your helpful video! I’m awaiting assessment for an official adhd diagnosis and really want to move things on. I have no doubt that I have adhd, but need the diagnosis to get help from the system.
I want to mention that while I was in the US last year, I tried psilocybin as a treatment experiment ~ it was legal or decriminalised where I was (Denver, Colorado) ~ and I have never felt so functional, relaxed, “in the flow”, focused, and even happy as I did after doing that dose. The effect lasted for more than a couple of weeks.
I’m aware that there are many recent studies that are demonstrating the great effectiveness of psychedelics in treating a wide range of psychologic disorders, and feel that with this mounting body of evidence it’s only a matter of time before it becomes legalised. Do you know of anyone who’s talking about microdosing psychedelics to treat adhd and if so, when it might become available as a treatment option? I’d love to hear what you might know about this. Many thanks again for your help.

    • @ADDtrepreneur

Thanks for your comment @MrBongoEdminds. I haven't tried psilocybin but it is something that I am interested in learning more about too.
I don't know of anyone but will ask a friend and I'll ask the question about micro-dosing psychedelics in the ADHD Entrepreneur FB group and see if anyone has any experience to share.
I know that there are a few studies being done on the use of psychedelics for ADHD but nothing definitive yet. It would be interesting to find out more about your experience and how it helped. Do you have any links to studies that you know of

@@ADDtrepreneur thanks for your response. I confess that I tend to listen to youtube videos or vlogs while I work and rarely have the time to read papers (as I’m not that organised 😄), and it’s being talked about a lot online. I did a quick search prompted by your question and saw that there are published papers on psychedelics for adhd research. I’ll return to those and see what I can find.
My recent experience was with a macrodose of psilocybin, but not what psychonauts would call a “heroic” dose. I took it with the intention of turning inward for exploration rather than for recreational use, so had on earphones with meditative music and eyeshades. It was a cathartic experience. I did it 2 days before travelling back to the UK. I normally get anxious before travelling, but found myself very relaxed about it and though I had some stressful moments during my travel (nearly missed a connecting flight), handled it all well.
On my return, I had an impossibly short amount to time to prepare for 2 back-to-back exhibitions ~ I make sculpture and jewellery ~ which would normally put me into a panic, but I found myself clear-headed despite jetlag, able to take action without any procrastination or anxiety, and enjoying my time in the studio immensely. Creativity flowed from me and I can’t remember being so productive without being in panic mode. On both a personal and professional level, I would love to have that as normal in my day-to-day functioning. I’m mostly interested in microdosing for that reason, though would also like to eventually explore a heroic dose in the right setting at the right time. Friends report profoundly deep and personally important experiences doing heroic doses, and I also hear of people who report gentle but positive life-changing effects from microdosing. The most well known of those to me is Ayelet Waldman, who wrote “A Really Good Day,” in which she describes her experiences microdosing LSD for treatment-resistant depression.
Right, ending this novel and getting to work 😊, hope you find it helpful and would love to hear if you find any new info on psychedelics in adhd.

I gotta say I've had ADHD from around 3-4 years old I'm now 33yrs old. And having ADHD as a kid and as an adult I find are two completely different things..
As a kid I couldn't stop moving always fighting and getting into trouble went to numerous special needs schools got locked out of a few. Bit I think the main thing is the alienation you feel like you don't feel the same as the other kids so it builds up alot of frustration and negativity. That's my experience.
As an adult I have 2 beautiful boys 5&3 and I manage a barbershop. I find ADHD as an adult my thoughts are always racing I talk super fast it's like my brain feels like it's working 110% I question everything about my day I find it really hard being in big crowds like a sensory thing feels like there's too much energy around me and I find it really easy to pick up on peoples good energy and bad energy. My short term memory is terrible bit seem
To remmember names really easily I'm tottally fixated on movies and would know all the directors and cast and could tell you the whole story from start to finish without hesitation.
Relationships are super hard as your always up and down emotionaly and question everything about start to present.
Freinds a few friends not a very sociable person as I find peoples energy can either be fulfilling or draining.
I haven't taken any medication since 17yrs old and have managed my ADHD through gym running and healthy eating
I Find with my ADHD my personality is super addictive sooo drugs can't do as would definitely be a problem I smoke 40 a day and drink like a trooper as I find it stops my thoughts racing and helps with my sleep management. I've tried weed but seems to make my focus even more intense I dunno why..
Also I find having ADHD I have a trigger temper soo working out and mediation really helps me and just try to focus on being present.
    Soo yeah that's my story

    • @ADDtrepreneur
      @ADDtrepreneur  2 роки тому

Thanks for sharing your story, Luke. It's interesting to hear how ADHD affects people in different ways and how it can be managed in different ways too.
Parenting is hard too! Do you think your boys could have ADHD?
From your comment, it sounds like you have a good understanding of your ADHD and how to manage it.
One thing I would say is that it's important to try and find healthy coping mechanisms for ADHD instead of relying on drugs and alcohol. These can make ADHD worse in the long run and cause other problems. I've always been exactly the same and do things in excess; whether it was going out drinking, smoking weed, going to the gym, buying things or just drinking Pepsi/Coke.
Relationships were also the same since they were always new and thrilling, and I'd drop everything to spend all of my time with a new girl until we broke up, never learning to slow down but it's like we only go full speed or not at all. ADHD definitely makes everything more extreme!

Shared care is a great option as it can be expensive to see a private psychiatrist on an ongoing basis.
Have you started meds since your last comment? How are you getting on?

@@ADDtrepreneur hey there I can't believe it is a year since my comment. It's nearly a year since I started on meds (Elvanse and eventually added dexamphetamine booster) and it's definitely been an interesting journey. The titration process was tricky for me as I have other mental health conditions, it took a while to get used to how the meds worked and the best way to take them. Personally I have found Elvanse to be a good match for me, I just had to figure out my metabolic rate (fast) to work around the sharp dip in my mood when they started to leave my system, and split my dose to take 50mg in the morning and 10mg in afternoon with 5mg of dexamphetamine as a booster. I take a low dose of Cipralex to manage irritation (which is linked to serotonin) and anxiety which I found one of the side effects. Other side effect has been very low appetite so I made sure to make myself eat regularly. Overall they have helped me in ways I wasn't actually expecting, where I have greater awareness and understanding of patterns of behaviour and relationship dynamics. This was more important to me to help me with my overall mental health and put the pieces of the puzzle together. I'm really glad I stuck with it as it's definitely helped me get to a better place in my head 😌 hope you are doing well!

  • @JamesC183

This was a helpful video, I am early in my medication titration and after the first month I’m like “meh” - benefits (so unnoticeable) certainly don’t warrant the side effects.
I paid privately as the wait is so crazy long in my NHS route, but now I’m not sure I can afford the titration trial and error. I may end up using psychiatry uk
I was going to ask, CBT - I have had years ago now, for managing mood stress and anxiety, is there a special type of CBT or collection of skills for ADHD?
Interestingly enough with talking therapy I never took away any lasting relief other than the benefit to offload, I can’t write and talk at the same time so I never remember anything from the sessions.
I diligently turned up to all my sessions for CBT for low mood, anxiety and stress, and when it was explained to me I was like “wow! this is great!” but for whatever reason I just couldn’t put it into practice, story of my life really! Start stuff but rarely finish it nor stick at it.
So now I’m like well medication isn’t going to “fix” me but I am also terrible at sticking to, implementing or completing things like a worksheet from CBT (which I lose) and I found I spent so much time trying to make changes it became unrealistic.
Damn I sound like a waste of space - but honestly these practical struggles are ridiculous.
I picked up on that “ideas” point - I feel I have some great ideas, lots of potential but just can’t harness it.

This was very helpful! Tahnk you so much!! Are there any UK based private clinics that are recommended/have good reviews? I think I have ADHD and I've been struggling to get an appointment with my GP (have been trying for few months, but they keep camceling/postponig) so I'm considering going private but don't know where to go..

    • @ADDtrepreneur
      @ADDtrepreneur  2 роки тому

Sorry to hear that but there are definitely plenty of reputable private clinics you can use if you have the budget. Prices seem to vary massively too. Dr Peter Mason in Liverpool (adultadhdliverpool.co.uk) gets lots of recommendations in Facebook groups and charges £350 for an initial assessment yet The ADHD Clinic in Oxford charges £1200!
If you're on Facebook, I'd suggest joining some groups; alternatively, look for a local ADHD Meetup group, forums, and ask for suggestions from people who've been diagnosed privately.
If you do go down the private route remember that you'll need ongoing care and support and that can be expensive. Shared care with your GP is a good option but your GP will have to agree to prescribe your medication so it's worth trying to speak to them first.
If you do manage to get a GP appointment you can also ask for a referral to your local ADHD service but don't be shocked if you're told there's a long waiting list. ADHD services are massively underfunded and overstretched.

Good vid. I went through the same process. I wasn't diagnosed as an adult though. I didn't get on with concerta and now I'm on vyvanse. Both give me anxiety and make depression worse. Are you on concerta? I'm going to ask if she can prescribe zyban and a non ssri to see if that helps. CBT would probably help. Do you have ongoing appointments? I have found working very difficult and I've tried to apply for PIP. I had no success with that, seems assessors look at invisible illnesses just as that and don't consider ADHD a serious condition.

    • @ADDtrepreneur
      @ADDtrepreneur  3 роки тому +2

Thanks Marco, I'm on Medikinet XL, same drug as Concerta but it's an eight-hour release instead of twelve. Have you asked about changing your meds yet? Good luck! I've not got any appointments yet but have asked to be referred to P-UK again for CBT and I'll need a medication review soon.
I know others with PIP, it is a hard process and most need to go to tribunal to be awarded. I'm sure it's something like 50% of PIP applicants are turned down at the first assessment so don't give up! The assessors do have a bad reputation for not listing everything or asking everything but PIP awards are given for how your condition affects you, not what your condition is. The Benefits & Work website has loads of good information around PIP awards if you've not looked there.

If not then you should ask your GP to make a referral for an ADHD assessment to make sure he goes onto the right waiting list asap.
Unfortunately, the service I talked about in the video doesn't see children through NHS funding, but you can get children assessed and treated privately if you have the funds.
Also, if he is in school you can speak to the SENCO about your concerns and they can make adjustments in school to help him. You don't need to wait for a diagnosis for that either, but some schools are less helpful than others.

How's it going with the medication Lizzie? I know P-UK offers CBT but I'm not sure how the process would work when you're already under a clinic. If it is an NHS provider and your dr agrees you would benefit from CBT then I'd guess they could put that in writing to your GP and the GP could then go down the Right To Choose route but that is just a guess and you'll probably be offered IAPT first.
