Spoilers : Peppino seems to be a failed experiment to a peppino reproduction project (for what ? No idea). In the hardest level of the game called "WAR", there is a moment where we are in a lab like environnement where we see a peppino clone in a liquide tank in the background. Plus, there are multiple peppino moving around just like our character, trying to hit us. The only difference is that they have some gelatinous animation when they are killed or when they are taunting when the player in not nearby. Fake Peppino is probably one of these clones, but a defected one. My theory is that it's the first clone prototype who have escape the facility to the sewers to make his own pizzeria (Bruno Pizzeria) or it's a regular clone who have been thrown away because it was defective and so, not useful at all. There is something I found interesting about these creatures is they have something to do about frogs. I've said before that the effective clones were taunting. Well, when they are doing it, that are also making a noise. A frog noise. For fake Peppino, it's about one of his attack animation. When he jump to dive on us, he have a posture that look similar to a frog. That is telling us that these impostors was made with samples of Peppino's DNA and batrachian specie. For my opinion, this is a sneaky reference to the movies "Jurassic Park" (for those who didn't know, the scientists in the movie mix the DNA of the dinosaurs with DNA of frogs to being them back to life).

You know what makes Fake Peppino creepy? The fact that we know nothing about him. Who is he? *What* is he? Where did he come from? Why does he live in an abandoned pizzeria? Why does he look like Peppino? Is he evil or just some creature acting on instinct? What is his connection to the tower? Is he supposed to be a being or a representation of Peppino's fears? So many questions yet zero answers. There's nothing we know about Fake Peppino. No true name, no backstory, no identity, nothing. He's just... there.
