About me: Dr Sizwe Mpofu-Walsh is an author, scholar and founder of the Sizwe Mpofu-Walsh Xperience (SMWX), a digital, youth-centred current affairs platform. He holds a DPhil in International Relations from the University of Oxford and is a Lecturer in International Relations at Wits.
The fund system is completely unnecessary. The current health system provides free care to the poor and its only flaws are ANC corruption and mismanagement. They are funded from the provincial health budgets and revenue from patients that do not qualify for full subsidy. A central fund system requires even more management. Some private doctors milk the private funds and they'll do he same with NHI at the taxpayer's expense. The rural problem is inherent to living in the middle of nowhere - you simply cannot operate a public or private hospital in a sparsely populated area. The problem of remoteness can be solved by introducing mobile facilities or patient transport, which can be funded through the health budget. Simply get rid of the problem, which is the corrupt and incompetent ANC - we have that rare chance in less than two weeks.

What recourse do citizens have to oppose this suspicious NHI?
Given the horrible track record of @MYANC with money, this may be a brilliant idea on paper, but thinking that this is steered by ANC is a failure before it even takes off.
Striving for equality is such nonsense, pie in the sky. People will never be equal, there will always be someone who's better than you when it comes to resources, looks, health, location, strength, IQ, etc. So pulling everyone down to one horrible level, is just a bad idea and it's immoral.
Thanks for explaining this thing for laypeople like many of us. I think that the idea is noble but the track record of our ANC government is so bad that nothing good will come off this - it will be another reservoir for money to be burned (stolen).

It is very sad that in South Africa when we talk about health we only talk about allopathic medical health services or only focus on the diagnostic or clinical aspects of healthcare. South Africa does not take a proactive approach when it comes to health which causes a ridiculous health burden on our healthcare system. Multiple negative health outcomes can be prevented if professions like Environmental Health which helps prevents the diseases, healthcare services are not only treating an illness but rather preventing it, which prevents outbreaks and reduces the demand on clinical care.
Like the sugar tax thing. Increasing the price of an unhealthy product doesn't actually dissuade consumers. It just makes them sacrifice more for it. It helps only to dissuade the casual consumer, like those who buy cool drink once a week.

Absolutely. Functional medical doctors are rarely covered as it is. It's a corrupt scheme ruled by Big Pharma.
Love the idea/practice of moving away from allopathic medicine and developing educational strategies early on with our youth as to the importance of health as a form of building legacy and wealth. Capitalism is indeed a big barrier to the collapse of allopathic medicine since latter is based on a profit motive not to mention big pharma.
'Premium' health care is a luxury. A private hospital is probably the most expensive hotel you'll ever stay. I was once in an accident near Paarl, and I was taken to a provincial hospital - did not have a TV - did not need a TV - boredom was not exactly my biggest problem at that time. I also know of these two tier practices run by private doctors where they have a proper reception and waiting room with flowers and paintings, a clean bathroom and they bill the crap out of your medical aid, but just behind the same building there's a queue of plastic chairs where you can wait with R50 or a good excuse why you don't have it and see the same doctor. I don't mind paying taxes for health care and other essentials for the poor and needy - it's the only reason I feel good paying tax, but I don't want to pay for their luxuries or some crooked liar to steal it.

I trust and hope this is the end of this evil man.
My big concern around NHI is the funding thereof. I've not yet heard a plan regarding where they get the money for it.
I know that the UK spends about £180Billion a year on their NHS (that's more than R4Trillion)
Last financial year, SARS collected R2.15Trillion across all tax revenues (Income, VAT, Import, etc). I'm aware we can't compare UK NHS to our NHI, I'm sure there are big differences in the cost planning, and the estimates I've seen are all over the place, but fundamentally both are an attempt at providing quality healthcare to about 60Million people. There is obviously an enormous difference between what we have and what it will cost.
I don't see how raising taxes alone will cover this. Our upper tax brackets are amongst the highest rates in the world, and increasing those along with VAT, Corporate taxes and others will surely have other negative consequences, like people leaving and thus we lose out on all their tax contributions. Of course, I've left out that this would be the largest single fund in SA's history and I doubt the corrupt gov officials would resist delving in.
I have it that NHI only becomes financially feasible if and when we have more people paying tax. Right now, I just don't see the numbers adding up, and I don't think they do either because they've been pretty vague about it. Are there other numbers/factors I'm missing?
There will have to be a tax and the socioeconomic impact assessment said that there would be a special tax for NHI for employers, employees and companies.
Excellent explanation. This can only work with NO Corruption. Stealing etc. Only VAT can finance this as ALL South Africans should contribute NOT just a few Tax payers
1. We already have Universal Health Care. Government just needed a way to double the budget. But they should compete instead of wanting to monopolize health. It's better for consumers of services.
2. We need the same thing for education. Education gives people a better chance of employment, and therefore, buying services that they need.
3. It's likely that by the time NHI is a reality, ANC won't be governing. Hopefully younger leaders will do a better job.
The destroyer can't build anything. Ramaphosa was not created to do anything good for his fellow people but only for his captors.
Basic concept is good and fair. Have serious concerns about the financing, implementation, quality , maintenance etc given government track record and growing disregard for ethical practice within wider society. As things are at present, I can't see it achieving it's worthy objectives
Indeed Healthcare is already underfunded, qualified Dr's without jobs and the funding to NHI bill, who , when and how ,,?would it materilise and manifest, to a successful Healthcare provider. For now only a paper gets signed!.
I just see our tax and vat going up🤦 instead of creating jobs so that people can afford good health insurance for them selves and their family . This can only work if rigid spending and logistical controls are put in place. All management and financial operations must be taken away from government.
Medical aid companies should take over these functions .. the government should pay the funds for all citizens that do not have private medical aids … they could fund this by doing away with provincial government, stop funding parties for election campaigns aand by reducing the number of ministers and deputy ministers in parliament… alternatively their salaries and perks must be reduced .. it’s unfair that normal citizens have to pay more taxes etc to fund governments shortfalls and government officials are not put out of pocket.
Basically the multi party system system is a nightmare as far as inefficiency, corruption and mismanagement goes .. we need system change urgently.
I was fortunate enough to be part of the pilot program in the northern Cape, the most important thing about the NHI is that facilities that can get access to the funding scheme need to be categorised as ideal healthcare facilities and these facilities need to comply with all the relevant legislation. That means, even though there might be looting with regard to procurement the facilities in theory would be compliant, thus rendering a standardised system and greater access to all.
Not sure that I understand - how can facilities be smoothly available if procurement is corrupt & money is not flowing where it should?
@@sharonadams9721 For example let’s say you’re a service provider for the disposal of Health Care Risk Waste, you can only loot by charging a fixed rate rather than per kg collected at that facility. Either way, the job is done.

SA Government should come to Brasil and consult here to see how private healtcare and fully funded government healthcare can co-exist and everyone gets medical cover, those that can afford the private care goes there and those that cannot gets treated by the government system which is also of excellent quality and the Gov hospitals here are growing more each year.

We wait one whole day for government ambiance. We need to do foundation work before promised which will take l0 years if they get it right today is 5 months i lost my dear husband. Before the bill got signed they should have visited all goverment hospital and clinic.

Our government has never been in favour of us, everything has always been to sponsor their investments and families.
Just a few years ago there were scandals regarding the billions which were allocated for COVID, the health department leading all departments with corruption.
So that could explain the negative reactions behind this bill.
Following my comments, what is the major cause of alcohol abuse, millions of people have no work, so people have no direction. And who caused the joblessness, only one answer = anc. The anc have grossly failed its people.
Can you present a statistics bases on total number of people per race vs percentage on medical aid in the same race. E.g. Total coloured population 1 million, total coloured people on medical aid 10.
If everyone should be treated the same .. everyone should have jobs so that everyone can contribute to NHI.
There aren’t enough tax payers to support governments lavish spending. There’s 4 times as many grant recipients as personal income taxpayers.
SA has 7.1 million taxpayers .. 2 million of those pay 90% of income tax. The number of people receiving grants is 27.8 million.

He passed on 18 December 2023. One just gets the run around. Because you are penioner you to old .To sum in simple language we can help you. So take this meds. Election Gimmick: Yes. No money to pool for now. If managed well, the NHI would be a great system, and I would support it. But with the ANC track record, good management is unlikely for now.

We will have a mass exodus of health care professionals from the country. This will impact everyone and middle class/in demand skills will also be lost, as this puts their families' lives at risk!! 1:17 I think u cannot say that access to premium health care is separated by race, that is a logical miss step. Discovery and other medical aids only care about the color green. Medicals aren't turning blacks away. The race issue is secondary to the economic issue. Yes you are correct that white south Africans benefit from the private institutions disproportionately but to fix that ensure that more black people can afford it. 15 years of a stagnant econmony and poorly done transformation is the cause of the demographic make up of private health care usage.
Truth is we already have universal health care. Anyone can go to a clinic, see a doctor and then continue treatment in theory and we already pay tax for the current health care. Private health care exists because the public version is bad people will self impose a tax upon themselves to not go through the torture that is the public system. But the government sees an opportunity to loot more and also appeal to its base of poor black voters, two birds one stone.

NFSAS doesn't produce more employable graduates, or is there some statistic I'm not aware of. NFSAS gets glory, but considering those qualifying for tertiary education are the 'cream of the crop' among their peers, we're basically distributing wealth from the have nots to the haves. We spend more on students than on the elderly and disabled. I reckon if we found spare money for education, we'll have more to show for it if we spent on pre schoolers and first graders from piss poor families, not on a laptop for someone who fails his first year in humanities. RSA has had spending deficits since Zuma/Gordhan and we now owe over R4 fuckin' trillion, with no economic growth to show for any of it.

The vocational training of doctors, nurses and other medical practitioners is in essence subsisidised by the taxpayer. On completion of their studies they should be compelled, as a moral duty, to be deployed to the rural areas to pay off their debt to the taxpayers!
I think we already make them serve two years community service. But maybe that should be bumped up to 3-4
