Condescending to a paranoid schizophrenic patient with psychosis is stupid, and dangerous. Patronizing behaviors, superficial communications saying things like “you’re doing so well” when the facts show they are at their highest risk of a serious psychotic break is incredibly damaging, potentially life threatening to staff and patients. Never condescend or patronize a psychotic. Two or three staff should always be together when dealing with Violent Schizophrenic patients with hyper violence in their Psychotic breaks.
The ward manager did a very good job at the end. System for mental health not fit for purpose the staff are stressed by the system and patiants get stressed as they needs are not met or even looked out for. I worked with guys like him and find when you dig they been told they bad or evil so much they start believing they should be locked up even thou they hate it and want to change. By giving the time to actively listen to them and working with such a person you can make a huge difference. Sadly the system dosn't give staff time to do that or reward it.
Maybe he was psychotic, but was this an attempt to be transferred back to Broadmoor, as he seemed to prefer it there, also, the trigger might have been when they suggested his involvement in the group directed at eventual discharge, because often people can become so dependant on the system, and he might have felt terrified at the thought of coping on his own outside the hospital.

I think the mental carer was not very good, complacent, not present, not interested to really know and solve, most a re like that, just do their hours without dedication , and as the story unfolds, I can understand Donald. Although he should have been more creative and asked for the good nurse.
