For what it's worth, the different difficulty levels not only change the enemy health and damage, but also the layout of many rooms, including the checks some of these rooms need. extremely minor spoiler: Except for the "hell" and "one hit" difficulties, that only alter the health and damage. edit: Also that was the only item that is not "visible" on the map. Congratulation.
