Could've been that they had dental insurance, and then whatever welfare they were receiving took care of enough of the essentials that the family could scrape by to cover the braces.
Dental/orthodontia is a strong investment for your kids if you're trying to help them jump to a higher economic level than you could give them. It's shitty but when lots of people see messed up teeth they think "burnout who doesn't care and neither has nor deserves a good job."
In the US, people who are poor can get free health insurance and this includes dental. Granted, it doesn't cover everything and can be very limited in some aspects but kids usually get quite a bit covered.

Bulimia is serious, poverty is serious, rumors about either should be taken seriously and this plot twist or whatever they were trying to do trivialized both.
This show is set in such a weird universe, where pastor means social worker, no one has ever heard of birth control, and the Mean Girls quote that “you will get pregnant and die” is literal instead of the bullshit fearmongering that even Mean Girls’ own characters know it is.
It’s honestly mind boggling that the writers of 7th Heaven couldn’t see they were just highlighting the already damaged reputations that a lot of the most hypocritical christians already have, with these characters. And dialogue. And stories. The father is the most frustrating, pompous, arrogant, meddling, holier-than-thou @s$ in TV history. And they think there’s nothing wrong with the way they portray him. Furious and disgusted with your wife of a few decades you have 6 kids with because she smoked pot in college… I mean, that face he makes… and “How could you drop a bomb on me like that…”. My lord… How did people enjoy this show? Also, Lucy notices Shelby BARELY EATS then always runs to the bathroom… But then it turns out she goes to the bathroom to brush her teeth, and the truth is that she’s actually STARVING due to poverty… Wait.
Did the preacher really ask if the guy had both parents? As if people who have both parents don’t smoke marijuana? Or if everyone from a single parent home smoke weed? That has to be one of the most absurd things I’ve ever heard on television.
