Heme synthesis is essential to form hemoglobin, which is responsible for the RBCs ability to carry oxygen.
Iron (ferrous, Fe2+) combines with protoporphyrin to form heme, then, heme combines with globin to form hemoglobin.
If you understand heme synthesis, you are more likely to understand topics like acute intermittent porphyria (AIP) as well as porphyria cutanea tarda (PCT).
This is an introduction to sideroblastic anemia.
Can anyone tell me why do we give dextrose to treat porphyrias?

Because glucose and other compounds like dextrose are inhibitors of delta ala production hence porphobilinogen won't be produced.

I believe it slows down the reaction softens the blow. The reaction is still enevitable at some level.
Glucose inhibits the heme synthesis pathway in 1st itself.
glucose inhibits the first step, so there will be no accumulation of porphyrins

I have this doubt in head for year, have my own hypothesis but not sure, why in lead poisoning we have increase levels of ALA and protoporphyrin 9 if we inhibited 2 enzymes, I mean if we inhibited ALA dehydratase the pathway should stop here, how come it continues until protoporphyrin 9 ?

Lionel’s books are great...Videos with the book are better...The trick in biochemistry is to take out a piece of paper and try to write down all the pathway from scratch...Then try to focus on the rate limiting steps, and the enzymes that will cause diseases when deficient! Good luck!

No, but the more you know the better...I will first start by memorizing the rate limiting step and the steps that have a disease associated with their deficiency, then I will try to add more to my memory!

Carbidopa which is oftenly combined with levodopa, inhibits the conversion of B6 to PLP which is a cofactor of delta-ALA synthase.

At which stage of a Rbc age is heme synthesized ?.As mature Rbc's lack mitochondria how is then some part of heme synthesized in the mitochondria?.
All these steps occur in erythroblast.

Thanks for your comment! Carbidopa is correct...and Levodopa is even more correct. Because when we give Levodopa and Carbidopa together, we minimize the bad effect on vitamin B6
