    Most informative so thank you for taking the time of explaining this in great detail. My Hematologist has written a report to my doctor noting that I have developed mild Pancytopenia and have already started on B12 injections last month. My B12 is within normal range however the HoloTC (Holotranscobalamin)level was abnormal so is still being looked into as to why. I have been having severe Hemiplegic Migraine with other Neurological conditions over the past 3 years.

    Just saying, I am a committed vegan and obtain B12 from cereals. Keeping well after circa 10 years. Animal products also have their drawbacks, especially red, processed meats and dairy, best avoided. Good video, thanks and best wishes. 🕊️🧡✨

    I came to this video in the hopes that it would help me understand my B12 deficiency after seeing a neurologist for my symptoms. It was very helpful in understanding the processes; however, I just wish the doctors could tell me the REASON for my deficiency. I’ve been receiving injections but I’d still like to know why.

      Sometimes, it’s difficult to identify the cause.
      If your symptoms are getting better with the treatment plan, then you’re on the right track. If not, see another doctor.

      Do you have stomach or digestion issues? Mine is from celiac disease which ruined my stomach lining so I couldn’t take in vitamins and got really malnourished

      @@oxyroid I do have GI issues. I go to the bathroom numerous times a day. I normally can’t plan long outings from home because I’m afraid I’ll need to use the bathroom. It’s pretty terrible. I’ve mentioned this to my doctor many times but she has not had an interest in testing me for anything.

    2 years ago i used metformin for weight loss, i lose wieght but got ice pick headach which i never experienced in the past, after that i got grey hair in th area of ice pick headache. Than i stop using metformin and from past 1 and half years im using vitamin B12 sublingual, Now i get less pain and some of grey hair turn blach. But now due to my stress and depression nothing works, my hair turns grey faster. Im 32. Now what can i do. Also premature grey hair is not in my family genetics .

      Try to take B12 injections. Very big difference. Your r protein could be not created by body and b12 not absorbed. Move to another city out of depressive environment.

    American Academy of Neurology:
    People with inflammatory bowel disease may also be at risk for
    developing nerve damage and other neurological problems, according to
    research that will be presented at the American Academy of Neurology’s
    59th Annual Meeting in Boston, April 28 - May 5, 2007.
    Inflammatory bowel disease includes digestive disorders such as
    ulcerative colitis and Crohn’s disease that cause swelling and/or
    bleeding in the intestines. For the study, researchers compared 103
    people with Crohn’s disease and ulcerative colitis to 51 people with
    other digestive disorders, such as irritable bowel syndrome, gastritis
    and chronic heartburn.
    The researchers found that those with inflammatory bowel disease were
    about four times more likely to develop neuromuscular conditions,
    including carpal tunnel syndrome and small fiber neuropathy, which
    causes pain and lack of feeling in the feet. Those with bowel disease
    were also more than six times more likely to also have a disorder called
    sensorimotor polyneuropathy, a nerve disease that can cause weakness,
    pain, and numbness. These diseases were more common in women with bowel
    disease than men.
    “Inflammatory bowel disease patients commonly suffer from several other
    medical conditions, like B12 deficiency and glucose intolerance. These
    nerve conditions we have reported are often not diagnosed by their
    primary care physicians,” said lead study author Francisco De Assis
    Gondim, MD, MSc, PhD, Professor of the Federal University of Ceará in
    Ceará, Brazil, and a member of the American Academy of Neurology.
    “People with inflammatory bowel disease who develop new symptoms, such
    as pain or numbness in the feet, should see a doctor.”

      @@starrysky1588 I really hope so. But the problem is that I cannot study medicine in my country as they mainly offer it for Highschool graduates. I got admitted in NewZealand as an international student but I don’t have money to study. My university will start in three months and I don’t know how to pay my fees yet lol 😂
      If no miracles happen, I may kiss my offer goodbye 🙈

    This B12 is a promiscuous vitamin. First it binds to R protein, then to intrisic factor and in the end to the transcobalamin

    Thank you! SO complex. PS: some chronic parasitic infections eat our RBCs and B-12. Over several years time they cause Malabsorption, chronic fatigue, B-12 anemia. Hookworm, Babesia, and Fish Tapeworm. Case numbers exploding. Doctors do not know about this!!! And the lab tests for them are often false-negative. 😖🤬 Thanks for helping us.

      Exactly, to bad they don’t test here in the states until they’ve caused lots of damage and making tons of $ off of people and insurances. It’s a cheap test and cheap treatment. Unfortunately they recommend we treat our animals for them at least twice a year but not humans even when we are around them on a daily basis. Not including all the other places we can be exposed to them such as restaurants, unwashed fruit ect.. it just doesn’t make sense to me. Thankful we are able to get the treatment for them from a local place that was willing to get us some. One time treatment. One pill then one more 6 weeks later to treat eggs that may have not been rid of. They recommend doing this at least two times a year to be safe.

    If alcohol prevents folate absorption and folate deficiency is a cause of macrocytic anemia then alcoholism can cause both megaloblastic (folate def) and non-megaloblastic (chronic alcoholism)?

    how about other b vitamin (B1,B6,B9,B5) gets absorbed in body similar to B12 ? If someone has B12 deficiency will there be problems with other B vitamins as well ?

      All the b vitamins work together, if you are b12 deficient then you need to take a b complex too.

    What if someone has a MTHFR mutation and can't utilize the standard form of B12? Is it true that only methyl b12 is effective?

      It’s my understanding that people with the MTHFR mutation shouldn’t take folic acid supplements

      Great question! Yes. I have been recently injecting methycobalamin and feel so much better. My GP kept saying that my b12 levels were too high in my blood work and to stop taking it but I was barely taking any supplemental form at all so it clearly didn’t make any sense. Then I learned that high b12 levels in the blood can actually indicate a deficiency. I was having severe heart palpitations and horrific anxiety amongst other odd symptoms. I am homozygous for two MTHFR mutations and decided to do my own research. Since doing my intramuscular injections I feel like a different person so it’s clear that I was in fact deficient and my GP was very wrong. Also Viatmin K2 and a very low dose of Lithium Orotate will help the absorption of B12 a lot. Hope this helps a little 😊

      Peaka01 Absolutely correct! Methylfolate is the correct form for us mutants 😉
      It can cause anxiety however.. so in my experience it is best to start very slowly and work up to a comfortable level. 😊

    I have some questions - To which protein is cobalamin bound in the liver? Is it still transcobalamin? Is it correct that amylase cleaves cobalamin and „animal protein“ as it actually serves as an enyzme for carbonhydrate metabolism? Thanks a lot!

    Fun fact: it's actually quite rare for a vegan to be B12 deficient these days. Plant-based milks and vegan products are now fortified with both B12 and folate

      ​@@MedicosisPerfectionalis You are incorrect. The best source is not from animal products. b12 is made from bacteria....however the world is more sterile now. We used to get our b12 from pond water (water is now treated) and we triple wash our produce (soil )...so it's affect the general population (meat eaters and vegans). Animals are supplemented with b12 so why not get it directly and cut the middleman? go plant based

    Does taking cobalamin under the tongue by pass the whole process with intrinsic factor? Can it be absorbed by those who have trouble absorbing due to their intrinsic factor?

      Yes because it is being absorbed directly into the blood stream which bypasses your stomach

    I am a vegan who is making sure I get enough B12; but my meat-eating grandfather supposedly died of pernicious anemia. Is it possible to die *from* pernicious anemia? Or is it really death from *complications* of pernicious anemia?

      Pernicious anaemia itself means that a person can't absorb b12 from food and supplement, they survive only on b12 injections.

    I have kidney disease 3 stage but the doctor telling me I can’t eat things that have potassium I been eat spinach because I heard is good for the iron but not for my kidneys check that out now I feel weak I had to go to a health store and bring up the iron so tell me what is good that I can eat that won’t affect my kidneys 😩

    Just found your video, great! 3 years ago I had bleeding due to ulcers, due to H pylori, from there they told me I had pernicious anemia, I suppose to get B12 shots every month for life, however my anemia improved, the Dr. said I don’t need shots, now my B12 is borderline and I do not have anemia, why does this happen?

    this only discusses very drastic symptoms. what about subtle symptoms like high triglycerides, higher homocysteine and slow memory loss, in cases where b12 remains low in spite of oral supplements.....

    But i have two questions one is amylase for carbs digestion ? But u said it help to dissociate B12 from protein ? Is this mean amylase has two functions ???
    The other question is normal flora in our intestine also synthesized B12 true ! ? If the person is strict vegan B12 not become zero ?? And don’t reach to severe state ??
    Can you clarify this to me ??

      Answer to:
      Q1: yes, amylase has more than one function.
      Q2: just because it’s in your gut, doesn’t necessarily mean that you will absorb it in sufficient amounts (i.e. the concept of bioavailability).
      Instead of looking into theories, look into the statistical scientific studies of patients with B12 deficiency, a huge percentage of them are strict vegans.

    Hi thank you for this video. I am not in the medical profession and I am getting in touch because I keep getting elevated vitamin b in my blood results even when I am not taking vitamin tablets. It keeps coming up as abnormal on my blood test results but my healthcare professional does not explain why. I heard you can still have a vitamin b definitely with elevated vit b in blood. Could you help at all? Could the elevated vitamin b in blood mean I am not processing it correctly? Thank you so much.

      Bcoz it interferes with neurological symptoms improvement, which is due to b12 deficiency. After loading doses of b12, then folic acid should be given, meaning first b12 levels should be raised . After loading we have to take folate as well, because both work together to make red blood cells along with iron. Also folate helps in processing b12.

    Sir what are the normal levels for Vitamin B12 ? below than 200 ng/ml B12 level, is it considered normal or low ?

      For optimal health, it should be atleast 500,some people feel symptoms even in 300 to 400 range. Below 200 is very low. In Japan minimum is 500.

      - iron deficiency anemia.
      - anemia of chronic disease.
      - sideroblastic anemia.
      - Thalassemia.

    I got some lad work it’s showing my b12 is 377 and in the ranges says ppl with 400 less experienced symptoms. I been loosing hair my hand and feet get numb and cold , I have Poor circulation what should I do

      You should start supplementing according to your symptoms. If you are vegan, then you have to either change diet or take supplement, if you are taking non veg frequently, still symptoms, then you have difficulties in absorbing b12 from food , in that case you need injections. Serum b12 tests are not reliable. We should always look for symptoms and try to treat them. It's true some people feel symptoms even in 400 level and some don't even in 200 range. The only way is supplementing and seeing whether you feel better, this is what I did. I am on b12 injections. Along with this you also need to take folate in order to process b12, they both work together.

    Sometimes when I eat meat it feels like I am not digesting it properly. I am suffering slot from either B12 or B6. I feeling very bad. Tired, bloated, dry skin, esp scalp, esp. I feel like there is something above my navel. I feel thirsty, I lost glow on my face and much more..

    Hi, would you help me clarify if this anemia is or not hyperchromatic? In italian wikipedia page it says: "it used to be called hyperchromatic anemia" and one of my professors wants to hear "hyperchromatic" during the exam but i'm not sure :/

      There is no such thing as hyperchromatic or hyperchromic anemia...It’s self-contradictory!

      Why would it be self-contradictory? You could have less RBCs (anemia) but the ones present could be very pigmented (I don't know any condition but logically it's possible).

      Check Harrison’s internal Medicine...It states that clearly; when RBCs acquire more hemoglobin, they become larger, not darker! I am also aware that Robbin’s pathology still uses the term hyperchromic!

    what is this lemon yellow skin? Is it jaundice. Both folic deficiency and B12 deficiency lead to big blood cells, which will be break down by the macrophage. Does this leads to jaundice?

      never mind. I believe lemon skin is caused by decreased Hgb and jaundice. pallor+jaundice probably will be yellow. Don't know what skin color change will happen in Asian people. We are born yellow.
