You are correct, the US IRS Form 990-N (e-Postcard) requires nothing more than the qualifying checkbox - and that's just "are your gross receipts normally $50k or less..." it's definitely not a clear threshold... The 990 EZ requires formally reported revenues and expenses if gross receipts are more than $50k and less than $200k or (rev >$50k and) assets at end of the year are less than $500k.

I’m hoping you still check this. We have donors who give over $5k in a year but do not want to provide their address how do we complete the Schedule B form for these donors?

What form do you use for a "NOT TAX EXEMPT" Unincorporated Non Profit association with less than $18,000 in revenue

Good question - If you haven't found an answer, definitely worth talking with your CPA or attorney since there are possible legal implications
