Actually in novel Head Games, Ace told Mel that she had killed 423 people. Indeed Ace never hesitated about killing enemies after she rejoined 7th Dcotor in VNAs. In Blood Harvest she shot some gang member in the head and simply ignored Doctor's complaining.

Depending on how many Daleks there were, Rose could have the highest number. But I'm probably wrong.
There were something like half a million Daleks in The Parting of the Ways.

Since he was an assassin before meeting the McGann Doctor & getting sent back to the Dark Times by the TARDIS and meeting the TennantB Doctor, followed by briefly becoming a companion to the TennantB Doctor as they fled The Battle Of Mordeela, I imagine Brian the Ood probably had one of the higher death counts to his (& Mr. Ball's) name(s).
Brian has killed millions and i won’t hear otherwise.
As far as I can remember the Doctor being a pacifist who is anti-gun only came to prominence with the Tenth Doctor's era. Every other incarnation of the Doctor to my knowledge had little to no problem with killing the villain(s) to save the day. 4th Doctor casually snapped the necks of henchmen in Seeds of Doom afterall.
A post-Time War vow, I suppose.
@@jaschul A post time-war vow that only the Tenth Doctor has followed. Every New Who Doctor before and after Ten has casually killed or left the villain to die with no qualms or morality preaching about it. Maybe the Tenth Doctor is the man who never would, but for every other face the Doctor has quite a murder streak.

Katarina had a kill-count of two her first time out and then left the show. So strictly in terms of kills per screen time that has to count for something.
Tegan comitted genocide in Time in Office by destroying the faith that kept the Armecei, a god like species alive.
I hate that I forgot this. It does mean it can get its own fact though!

Sgt Andy has a couple more kills as far as I can recall. In ‘Aliens Among us,’ he’s mind controlled into shooting an immigrant, and in ‘Goodbye Piccadilly,’ he telepathically transmits Martian access codes from his head into the mind of London gang boss which causes her head to explode.
He also brutally murders Owen’s good mood at the end of ‘The Hope,’ where Owen experiences a momentary resurrection and wants to eat something before it fades away. Unfortunately, Andy only had ready salted crisps on hand, which Owen loathed.
The dig at Teghan and Ace returning to the series with massive guns got me. As did Adelaide's own goal. Well played.

Such as the Silence all being naked, the Nimons having cybernetic heads that intentionally look like crap masks, and it implies the Quarks are actually the Toclafane.

No mention of cinder she was basically the war doctor's companion and engines of war and while I think that's her one major appearance I still counter just due to the fact that war doctor needs more friends

Marvel UK-era Who comics had a character named Abslom Daak. If you're including Daleks as kills, I think he probably wins, as he was eventually dropped into the Time War and actually _enjoyed_ it because of all the Daleks there were to massacre (and that was at the _end_ of a rampage through time destroying pepper-pots left, right and centre).
The Doctor has caused genocides. Abslom Daak _IS_ a genocide.
Plus, he's technically mainstream canon, as he appeared in the TV show.
(Okay, it's for like half-a-second on a computer screen in "Time Heist", but it's still a TV appearance.)
Sam talked about Abslom pretty extensively literally last episode. Safe to say he knows about that.

This was in a since-retconned timeline where she became deeply unhappy with her life after marrying Cliff and having a son.
