The masked lady already knew that Deemo is her brother and she doesnt want Alice to leave. She wants them to stay but Deemo does not agree with her (there was a cutscene that shows this). Deemo showed his true form at the end because if he did from the start, Alice wouldnt want to leave and she would die just like him.
For those of you who dont know, this is based off of a true story... and their are numbers in the game which give you coordinates to a hospital in which this story actually took place... and if you havent cried Im crying while i type this due to this emotional music, i've replayed this song over and over again at least 10 times and this entire game and all this music is so emotional and over time deemo has become one of the things that has made me cry the most... Im still crying.... i swear... i love deemo... and will still love deemo... i've played the epilogue at least 15 times... so sad

The mask lady did not want Alice to leave is because she knows that Deemo (Han's inner spirit) will have fulfil his job to wish Alice his goodbye and would disappear, whereas the Mask lady would be alone until Alice has finished her life. A prove that Mask Lady is still in that world is by getting a full combo in "Suspenseful Third Day", where the artwork will show you the Mask Lady sitting alone on the thorn piano. As Deemo as disappeared and Alice being on Earth, she is now left alone. HNGHY

I think it's more along the lines of it being her own knowledge of the truth, or a part of her own consciousness that knows Han is dead. So, while Han and the Unknowing Alice play the piano in hopes that she will be able to escape, the piano is a symbol of pain for the Masked Alice since she knows she'll eventually have to part with the illusion of Han still being there.
I dunno why exactly, but the first time I saw this scene, I didn't expect such a cliche plot where the MC dies from being hit by a truck could still make me shed tears :"(( I mean like, in real life, there are even hardly any trucks travelling at high speed in urban areas... It's kinda unrealistic and overused in mangas and animes. That's said, I regret nothing. The team did a great job in making the script and audio-visual, no matter how cliche the plot they use is. What a masterpiece, I should say. And now, I'm still waiting for Deemo The Movie to be released officially... It's been a year since pony canyon posted its trailer on youtube in 2019...

i came back to this video after like half a year after finishing deemo and i forgot how emotional the music is and now im crying
