As someone who knows the fallout lore very well, my theory is that the serum he got was the Forced Evolutionary Virus or “FEV”. This means he will probably slowly evolve into a super mutant. He definitely didn’t become a ghoul, as ghouls don’t regenerate, but people exposed to FEV might until they finish evolving into a super mutant

But then again, there were sentient and fully functional glowing one ghouls who were exposed to so much radiation that they they begin glowing and emitting their own radiation… even being able to control their radiation output to move amongst normal people or blast it out in lethal doses to kill enemies and heal other ghouls.
IF you think this way, if the scientist guy had taken the drug, he would have survived and he wouldn't have told Lucy to cut his head. One small decision would basically change the whole story.

So they really like to add “perks” from the game into the show. The ghoul has a perk called the messy kill. So whenever one of his shots kill, it’s usually in a gory mess. Like when ol lady June episode two got shot it was a smaller hole. But anyone killed nearly exploded. The amount of blood splatter from little tommy shows it was a kill shot. The vault 4 dwellers were super nice. But that’s after they discovered they were lab rats and took over the vault but kept the earnest parts of the vaults continuing. Like no longer experimenting on vault dwellers or new comers. So they in turn wanted to be what they believed the pro gal vault dwellers were supposed to be. Awesome reaction guys keep it up!

Most likely Thaddeus didnt turn into a Ghoul, he is most likely a SUPER MUTANT who are constantly healing and mutating to their environment, its why his foot and neck healed. if the Ghoul healed like he did he would still have a nose and proper skin.
I think most ghouls are natural and this alchemist just got tricks.

I read somewhere that Vault 32 is the rejects of Vault 33. So if they were sent tk Vault 32, it's because, for some reason, they weren't good enough for 33
