There is finesse or there is force. Force is blend it to hell until it mixes. Finesse is futzing about with mixing in confectioners sugar in dry cocoa and slowly stiring in small amounts of cream etc to hydrate it into a thick slurry that will kinda mix with milk.
If you have a blender use a blender.

I did the same recipe just with an extra scoop of sugar and no vanilla extract. It was very good.
