    I just went to a new injector. I requested an eyebrow lift, and she injected right below my eyebrow, along with other areas, which i have never experienced when getting a brow lift. I've been so worried about why she did that until i watched this video. Thank you for this explanation, and im hoping it is successful!

    In the beginning, I followed my Instructor and injected away from the eyes. But after time I felt much safer to do the opposite because I hit the right depth better or safer. Sometimes I wonder why it seems that some injectors don´t know where the m. orbicularis oculi exactly is and what he does, because you explain this over and over.

If I may ask, a lot of people tend to use Nasalis more often after injecting the toxin in the frown area. do u think it is better to inject the nasalis from the beginning to prevent its excessive future use?

    For some reason i watched this like 5 minths ago and had a really hard time understanding this. I couldnt actually picture the anatomy looking like that sandwich. I just couldnt. Now after actually injection more clients and seeing their results, i am really realizing how the botox really moves through the tissue. This is much easier to understand now. And it's very helpful! I feel much more confident about injectjng safely! 🎉

    So what if a person has wrinkles in the Cat ear area ? If you treat other areas of the Frontalis and omit this area, won't it look weird?
    And if you do go ahead at a later date to treat this Cat ear area, would you use less neuromodulator? Say .5 or 1 unit?

    Very nice explanation overall. I fully understand why you spare the cat eye area, it kept giving me the spock eyes though on about 1/3 of the patients. I went back to injecting at least an unit about an inch above the lateral 3/4 of the eyebrow. You had no spock eyes results leaving out this area?

    I am a male and i had an unnecessary browlift performed (i just needed a lower blapheroplasty), my eyebrows now look super high and i want to reverse it. Is it possible to reverse it with botox? or do i need to do another surgery to get back to my original brow position?

    I'm a little confused about the muscle anatomy of the orbicularis oculi. When I look at a diagram of where the muscle is under the skin; I don't see that it reaches all the way up to the brow, especially at the lateral end of the brow. ?? So when injecting the area I feel its not going to be inserted into the correct muscle

    What a great video with great information. Please, for we patients who are watching to make sure we are getting the right treatments in the right places, could you do a video to help us if we are looking to get Botox and perhaps other treatments for the first time. Questions like, who would you recommend, a cosmetic Dr, a dermatologist or a plastic surgeon?
    What do we look for, what questions should we ask or qualifications to look for, to make sure we are in the hands of someone who knows what they are doing?
    How do we check that they know what they are doing? What might be the best way to bring up the knowledge you have discussed in this video for example? Any guidance would be much appreciated 🙏🙏🙏
    There are a lot of ‘Drs’ out there who are making the big $$$ without the qualifications or experience.

    Very well explained. Thank you. I had an operation, and the doctor took too much skin. Now I cannot completely close my eyes. If another doctor injects Botox in order to lower my eye lid, will it work? Should he inject right over my eyebrows (in the middle), on my eyebrows (in the middle) or just under my eyebrows (in the middle)? Thank you!

    Where would you inject when you get a lifted outer brow and dropped inner brow from odd muscle pulls on same client? Very strong outer pull naturally ..
    What layer would you want to be in for strong pulls?
    Use less at first ? Like you say..

    Dr. Tim. I’ve been seeing so many injectors injecting just above the brow in the oculus muscle, and that is giving a Lyft as well. I was always under the impression that injecting above the brow like that, like when people get those creases above their tail that it could potentially lower the brow I mean, I have always done it very superficially, but how come I’m seeing people be able to do that and get a lift they’re just going do you know I’ve seen people do all the way across on top and on the bottom from the mid pupil or just to the outside of the middle people line all underneath the tail does that just send keep their brow perfectly shaped and then the perfect positioning when you then go in and do the forehead you don’t get any weird brow shapes?
    Another question I have is when you see people doing frontalis injections what is the difference like in these people that they’re injecting don’t have much movement but some people do the stagger step really high above the mid pupil high up on the floor head and then in the middle, so what does that do versus just doing straight across injections because it seems to me if you just do straight across injections right in the middle of the floor head and you don’t go deep and you do it consistently then you would not Cause any sort of miss shaping to the brow, but maybe that’s wrong I know it’s a lot more useful to have a round brow versus a strong lateral arch so if someone wanted to keep the arch, more medial and the brown, more rounded with the best frontalis injection technique, be the staggered step injections and then also injecting,

    It's crazy to think that I did my advanced training a few months ago and was told that to lift the brow you always inject above, and that if you wanted to lower it back down you inject below!! I've been following your videos for over a year now since I did my foundation training and always look to your videos and advice now, as some of it is alot different from what I've been taught!?       The real goal is to find out the first principles, when you really understand the direction and pull of different muscles you can be free of bad advice and training where ever you may find it.

      btw, above AND medial frontalis does lift- but lateral and inferior will drop. Sometimes lateral and high up can still lift, but that's because you have so much untreated muscle above the eyebrow still able to lift.

    When there is no little bump of botox visible like on the model after injecting, does that mean that they've injected too deep, or does it depend on someones skin and how thick it is? Since I've never had such a bump but heard multiple times that it should be like that and now I also see it in this video. But I think the models skin is thinner than mine is

    So, I’ve been doing ‘brow lifts’ in the orbicularis occuli with ‘tox for nearly two decades. I just tried DAXXIFY on one model patient and at three weeks she is reporting ptosis. Because of her natural,pre-existing laxity and ptosis her frontalis was not treated at all. Only her glabellar and orbicularis occuli were treated. I will see her tomorrow to assess. What is your experience or information regarding DAXXIFY and ptosis? I know that it uses ‘peptide technology’ to achieve longer lasting results but I haven’t heard anything about migration.

    hello dr .. i would like to ask you my case .i took a botox injection from month at beside of my eyes as well as at my wrinkle cheek lightly at skin not muscle and after 3 days my smile.become frozen and couldn't smile any more and my smile become fake ...i wear mask all time at work to avoid negative comments from my colleagues ....Really i did more than 10 radio frequencies sessions but no effect unfortunatly .. i did the same injection before twice at same places it was fantastic but this time it was so bad ...kindly help me what can i do to become normal speedly as before ...thanks a lot

    Hi Tim, I have a client who always gets a couple of ladder lines above the lateral brows, only when she raises her brow, not at rest. She loves her brow lift, I have tried putting a unit above each side, but doesn’t make a difference, wondering if it might be the trade off ? Any suggestions please?

    Is there ANYTHING that can be done to treat the drooping brow?? I had it done 3 days ago, and while I still can't see results yet from other areas that were treated (other than the "11's"), I have already noticed drooping of one of my brows to the lateral half. I had wrinkles (she called "quotation marks") in the cat ear areas, so she said she'd just inject a tiny bit of botox there, but inject by the tail end tip of my brows to help counteract the potential of drooping. She also did around the eyes for crows feet. I am SO scared that this is only the beginning and it will keep dropping further. Can anything be done at all for this or is waiting until it eventually wears off the only option??? 🙏🙏🙏

    Thank you for this tutorial! Wondering how you treat/counsel patients who already have low forehead directly over the brow. I always worry that leaving that area untreated will worsen them by increasing resting tone, but I also don’t want to drop the brow by injecting there initially. My approach is typically to treat there if needed on follow-up. What’s your approach there?

      Absolutely to what you have said.
      Not everyone can have a smooth forehead. The older you get the harder it is to have the best of both worlds.
      If they have a low / naturally heavy brow then the patient needs to understand that to have movement and lift their brow they will always have lines and may have more exaggerated lines because as you said the new resting tone.
      But we agree bring them back at review 2 weeks later. Working on skin health will support there skin too.

    Minute 8:57 to about 9:08 - “Spock brow” - “if you ‘overtreat’ the middle but ‘undertreat’ the side”. Am I misunderstanding because I would think based on earlier principles of video that if you overtreat the lateral it would lift the lateral brows, and if you under-treat the middle (glabellar area), that this would cause the Spock brow? Because if you treat the lateral area under the brow this would relax the muscle causing the lift, and if you didn’t treat or you undertreat the middle it would contract/lower the medial brow which would be disproportionate to the lift of the lateral brow.. right?

      When we are treating around and under the brow we are treating orbicularis oculi. It’s not frontalis this is how we get lift from this method.
      Under treating frontalis laterally will leave more movement for the brow to lift causing “Spock” or precise lift if you are after the lift from the cats ear approach on frontalis.
      If you don’t treat glabella complex you will have lift here.
      In the mirror hold the area of muscle you add botox (glabella) and then raise your brows. The untreated area you will see raise. This is a good visual to how it will work.

      The botox that is injected comes in vials of 50, 100, 200 allergan units. It is a term we use for dose. Different clinicians may make up different dilutions using saline so I cannot tell you what that means on a drawn up syringe.

    I love it. Been doing it for years. Loved that brow lift video BTW. Great upgrade. Can u show a great forehead pattern for those clients' frontalis seems to go all the way over and past the temporal crest

      Those that go past the temporal crest are more likely to Spock so just make sure you treat frontalis where you see movement, it will hinder lift if that is what your are trying to achieve

      @@DrTimPearce. When someone has one hyperactive cat ear. And there are ladder lines. One patient specifically when the toxin wears off one brow tail falls and one goes up. Cannot find the right spot to inject that cat ear to get it to relax. It seems tight even over into the hairline. I know there is a place somewhere in that area that if u inject u cause a lift rather than relax. Is it best to treat in a straight line down in the cat ear or just to do sprinkles with in it? It’s very strong. I have gotten result when treating in more than one place but then I wonder if all of those pts were necessary. And the biggest question is how close do I go down to the brow when trying to get a relax Spock. I do not want to give more lift by injecting the ocular muscle ??? Thank u. Sorry so long

    Does the SupraOrbital Foramen play a role in Lid Ptosis ?

      Yes absolutely! If the lateral injection in the corrigator supercilli is deeper than ideal then toxin can diffuse into the foreman and ptosis can occur.

    If you do get a bit of a browtosis from Frontalis injection. On review will a superficial injection of 1 unit under the brow as you have shown help lift the brow countering the fallen brow?

    Interesting about lower face doses. I went on a skin viva course and while there under supervision of the tutor my D.A.O was injected. Unfortunately I had a numb lip for 24 hours which is a very odd side effect ( possibly nerve trauma?) and from then on no movement in my left side lower lip. This has not improved at all in almost 4 months so I’m now thinking this is probably permanent 😢. Wonky smile and when talking is annoying as a dentist having to talk all day! but not life changing so Ho hum. No discussion on the course about different doses though so you might want to feed that back !

      If she pranged a nerve it could be. Why else would I have a numb lip on the left side for over 24 hours?

      I'm so sorry to hear this! My bet would be a very rare nerve injury, if it was immediate. They do get better but takes months. It's better to use the muscle as much as possible rather than suppress the smile

      Injecting too close to the mental foramen may have led to some inadvertent injury to the mental nerve. Usually we dentists take the intra-oral approach so we can numb those lower pre-molars. I've seen a couple paresthesia cases take 6 months to resolve so hopefully it will bounce back in that time. Did you feel any zings or electric shock feelings when the injection was given?

      hello dr .. i would like to ask you my case .i took a botox injection from month at beside of my eyes as well as at my wrinkle cheek lightly at skin not muscle and after 3 days my smile.become frozen and couldn't smile any more and my smile become fake ...i wear mask all time at work to avoid negative comments from my colleagues ....Really i did more than 10 radio frequencies sessions but no effect unfortunatly .. i did the same injection before twice at same places it was fantastic but this time it was so bad ...kindly help me what can i do to become normal speedly as before ...thanks a lot

    Dr Tim, thank you for the video. How do you approach brow lift for those that do not have a nicely sitting brow (brow skin sagging). Those are the patient's that need the brow lift, but is there a safe approach, other than not treating the frontalis? Seems like all brow lift tutorials are on younger patients whose brows are perfectly sitting where its a no brainer determining the location of the orbicularis. I would love your feedback please.

      Because a brow lift on a younger patient is where you will get the best result. Some patients do need to have a surgical intervention.
      The most safest way to achieve brow lift in a more aged skin or heavier brow is to to treat the frown and frontalis but to leave out the vectors of lift. The cats ears approach. The new resting tone will provide the lift. But there will be lines. You can have both - lift and no lines.

    I stopped injecting Botox because I couldn’t find a doctor who don’t let my eyebrows like spooky or with too heavy eyelids- literally I must change the way I make up my eyes 🙄My face is very symmetric and I lost symmetry making my middle third in my face smaller! Yep a wrong way to place Botox can affect many things around your face and self esteem! I wish this great Dr. Will be near me to go and see different results😢

      I've gone to numerous plastic surgeons & dermatologists asking for the spock arch because I have naturally low straight brows. None of them have been able to lift my outer eye. Theives.

    Currently having the worst botox result in the 12 years getting it. After years in fixing my forehead drop and crooked brows from a bad botox years ago, 1 botox ruined it. Physical pain in all injection sites, bruising in all sites, pain causing migraine over my eye…the eye that dropped years ago. Ripple of botox visible across brow line. Top plastic surgeon in nyc whose nurse did the injecting. I’m so nervous about what this is going turn out now

      Sounds very odd, like something wrong with the needle used? Hope you get better soon, I can say with some confidence despite not knowing your details that a full recovery is by far the most likely outcome.

      @@DrTimPearce thank you so much for replying! I’ve Been freaking out. Each injection site hurt so bad that it felt like the biggest needle I’ve ever had for it. scull and eye bone pain. Thank you again🙏🏼 praying it dissolves, it took years to get my eye drip and forehead drop to get better.

      @@runpainfree some practitioners use 29g which you really feel. I prefer 32g (a lot finer). Some practitioners also will aim to gently touch the bone and pull back which is not nice. It’s quite easy if taking your time to feel for the muscle and not need to touch bone. Some are also heavy handed and rush. Sounds like they are not reading the muscles, just going off a pattern. Your issues are easily avoided just by reading the muscles xx
