    In this video, I explain my interpretations from articles on rumination by Dr. Michael Greenberg, a clinician who suffered from Pure O, OCD. He intends that all ruminations are analytical thinking, that is under our control and we can make a decision to stop. I believe that some of our very best teachers are those who suffer from the conditions we are trying to heal and that is one reason I believe his work is important to the Pure O, OCD community.
    Here is the link to the video I discussed where I explain how to do the I. A.M. Method to Stop Intrusive Thoughts. • Stop INTRUSIVE Thought... .

    I’ve never felt like I had “permission” to stop. The rumination just feels automatic, and before I know it I’m in a hole. Thank you 😭

    • @PaigePradkoTherapy
      @PaigePradkoTherapy  2 роки тому +30

      Yes, it can feel very automatic. Try to catch yourself doing it, and practice stopping. It takes practice to notice when you start ruminating and to notice when you have an urge to do it, and to learn that you can stop. It is a process of learning how to do this.

    That is the problem with rumination, is that you get caught up in it. Realizing that is what you are doing and making a decision to stop is very powerful. It really helps to take a step back from yourself.

    Wow this is the first video where I feel like I'm actually getting some sound advice on how to deal with my anxiety and rumination. My problem is me trying to fight my thoughts and now I'm realizing that's what's actually making things worse. This will be a process but I know that I will eventually be able to fully handle this. I just have to remember that I control my thoughts they don't control me.

    Hi Paige, I’ve only just come across your videos and I have to say I’ve learnt more in the last couple of hours than I have in over 30 years suffering with severe OCD. My case has been a little unique over the years to the point where the medical profession weren’t sure how to help. It came out when I was 17/18, I’m now 51. In the early days it was the typical compulsions of rituals and hand washing. In my twenties it morphed itself into harming thoughts of other people. Exposure therapy back then did help to a point but it was never explained exactly how the brain works. When I was 30 my eldest daughter was born and when leaving the hospital I had an overwhelming fear that I’d somehow swapped my baby daughter with another baby! You can imagine how distressing this was and the next 2 years were absolutely awful. Every day I was having thoughts or convinced myself I’d swapped her for another baby if we were out. If things in the house weren’t the same the next morning after sleeping, for example, if my then wife (she left us in 2013 and I brought my two daughters up as they were with me full time) had moved something in the house I’d convince myself that it was proof that I’d been out in the night and swapped her. At first I was under a postnatal depression psychiatrist who basically told my family I would never get better and put me on a cocktail of medication. I was then moved to another psychiatrist when the previous one left and she took one look at the medication and said I shouldn’t be on half of these. With her help and a psychiatric nurse we found that sertraline along with CBT helped. My youngest daughter came along and with the CBT technique of letting my anxiety ride things weren’t as bad with her. Moving on from those years I kind of managed the OCD with a couple of little rituals here and there but generally had it under control. Recently I’ve had lots of trauma in my life. I lost my mum 18 months ago, moved house, relationship break up and lost my father last month. I’ve been going downhill for the last year as after watching my mum being bed bound for a few years deteriorating and once she died then watching my father who was then bed bound too with Alzheimer’s go down hill I didn’t have any time to process all of the stresses in my life. I’m currently waiting for cognitive therapy to start but seeing your videos I can honestly say that how you explain the thought process and exactly what happens to the brain through exposure therapy that everything makes a lot more sense. I thank you for making these videos and I’ve already shared them with other people. Best wishes from the UK and thank you once again

    Thank you Dr.Pradko as always ! It is truly a blessing to have you in my life. I have been watching your videos for the past weeks and they helped me so much. I have experienced rumination and pure O for almost 10 years, and sometimes it is just so hard to stop ruminating about my compulsive thought because I keep getting the feeling “what if I get it wrong and bad things happen because of my negligence ? “ Lately I have come up with a coping mechanism that I would refer to as “when someone points a gun to your head “ method. If I ruminate about a certain problem, I would ask myself - if someone points a gun at my head and asks me to answer the question that I am ruminating about in under two second (i.e. did I actually lock the door? Did I hurt someone ? etc ), what would be my answer. Surprisingly this method kind of solved my rumination problem ! If you don’t mind me asking Dr Pradko, would you recommend that I keep using this as my way of coping with rumination? Or does this count as reassurance-seeking and should be avoided (I am worried that my coping mechanisms sometimes are actually reassurance-seeking behaviors. ) Thank you so much !!!

    • @PaigePradkoTherapy
      @PaigePradkoTherapy  2 роки тому +3

      Hi Yuying. Believe it or not, your method is actually a method discussed in a book by Jonathan Grayson called Freedom from OCD. But, he uses the method with the question about answering correctly your own “what if” question. Surprisingly similar to your method. Although, I can see how it might become compulsion. I advise people to hold the thought they are ruminating about in the background (like background noise) and then shifting their attention to something they were doing before the rumination began. It is basically my I. A.M. technique applied to rumination.

    Thank you as always. The instruction of Dr. Greenberg was very helpful, but I've also been using your I.A.M. technique. That has been helping me, too, especially the mindfulness step. I go through those 3 steps when I'm dealing with intrusive thoughts and the rumination on them, and the "connect with the moment" step actually tends to comfort me the most, kind of squashing the thoughts. It depends on the severity, of course. Thanks, again :)

I pray that God blesses you for helping people and everyone you work with or no that does the same and anybody going through this or any mental illness🤟🤟 without God‘s peace, I would not be able to apply any of this tomy life. truly, there is hope, I ask this in Jesus Christ Holy name Philippians chapter 4 NKJV “6Be anxious for nothing, but in everything by prayer and supplication, with thanksgiving, let your requests be made known to God; 7and the peace of God, which surpasses all understanding, will guard your hearts and minds through Christ Jesus.”

    I love those Videos, thanks so much for them Dear Paige. By the way, before I heard this concept of a math problem, I already thought about it myself, since my Mother, when I was young, was compelled to resolve any math problem that I brought from the school, it bothered her so much (more than myself) and at the end of the day, when she succeded, she felt so proud and at peace. She loves counting and resolving mathematical issues, and OCD/ruminations feel to me like the sufferer tries to achieve this inner peace at the end from resolving a rumination, which is a neverending story. So- yes some of us can stop trying to resolve a mathematical problems without much effort, but some are such perfectionists it's hard to just forget and give up. It would be harder for a Mathematician perfectionist to give up such OCD themed issues (for example if he didn't feel relaxed all day long because he couldn't resolve something ) than for a usual person I think :)

      Pashteem, Only if thoughts become uncontrollable counsulting a physician will be needed. Your breath is directly related to your mind [brain] causing rumination and anxiety. For a relaxed life sit on a chair, back erect, hands on lap with palms upwards, eyes closed, be still and observe your natural incoming--outgoing breath and body sensations for around 10-15 minutes. Be as still as possible. Avoid moving too much. Many thoughts will come which is ok--slowly negative thoughts will reduce and your mind will relax. During your daily activity at home or office keep observing your breath with eyes open or closed. Best part is mood swings also reduce after daily meditation. Do the above meditation daily to feel relaxed. Best wishes. Shyaaam Sir. -Counsellor.

    Perfect explanation of an intrusive thought and logical rumination that follows in my case to prove the thought wrong as i know its not valid in my situation. However as you suggest to let the thought be and not engage with it... Unfortunately such thoughts get stuck in my mind and the associated anxiety doesnt subside even after weeks no matter how much i distract myself by doing other work .. What to do in that case doc?

      There is a form of ERP called imaginal exposures. One form is bringing the obsessions to mind on purpose by listening to recordings of prepared scripts or sometimes trigger words with pauses in between. I encourage clients to immerse themselves with these scripts, listening to them for hours and hours, everyday. It is a highly successful method if you are willing to fully immerse yourself. You may also gradually build up to listening to the scripts as well. I am working now on a video about this method and hope to have it on UA-cam in the next few weeks.

      Thank you. I am thinking about doing a video on that theme, because it is often asked about. In my opinion, the thoughts keep coming up for you because you are still analyzing them in your past and trying to figure things out about the relationship or maybe even reassurance yourself about some aspect of the relationship. Instead, when you have the initial thoughts, let them just float there without responding or analyzing them. You will have to do this repetitively. Eventually they will fade if you do not feed the thoughts with more thoughts. Here is my latest video on Mental Compulsions ua-cam.com/video/mAWc8fmrv6U/v-deo.html

    I've only recently figured out I had Pure O, I didn't even realize this sub-type of OCD existed for most of my life, so I've been trying to learn even more. And watching this video made me realize something. My biggest 'obsession' in my Pure O, is the obsession with needing to come to a conclusion. Thinking about how I have to figure things out now. Figure out right now why I would have such and such intrusive thought, what that means for me as a person. Concrete answers. But there are none. I have to try to learn to accept that sometimes, thoughts are just thoughts and don't mean anything about me. That sounds like sound logic, but I know it'll be a fight to fully accept that, mental illness is harder to sway with logic than people who don't struggle with it tend to realize. I'm already working with my therapist on other mental issues I have, but I think I should definitely bring all this up with her. Thank you for making this video.

    • @PaigePradkoTherapy

      Hi Riley. You now have a good understanding of what you have been experiencing. This need to come to a conclusion is a type of OCD that is sometimes mixed with completeness and perfectionism. It can be a mixture of OCD and OCPD (Obsessive Compulsive Personality Disorder). It sounds like your main compulsion is rumination. You will have to leave things incomplete, inconclusive and imperfect. Because no matter how hard you try, they cannot be figured out with complete certainty. I am glad you have a better understanding. I have more resources for OCD treatment here:

    Loved the video so helpful , I just wanted to ask do you know how can I deal with intrusive thoughts ( like I'll just randomly think of a thought like getting hit by a bus while I'm walking on the road and then I'm scared that I'll actually get hit)

    • @PaigePradkoTherapy
      @PaigePradkoTherapy  3 роки тому +1

      It is interesting to learn that all of us have these kinds of random thoughts. Neuroscientists have estimated that we have between 50-80,000 thoughts a day. So many of them are these cautious, dangerous thoughts. It is our reaction that matters. People that develop repetitive, unwanted thoughts react in a way that alerts the brain that the thought is so significant, it is crucial for your survival. This is why the thought makes you anxious, and also why it becomes obsessive. Not everyone reacts to these random thoughts. In fact most people do not even catch the thought because they know it is insignificant. Act as if this thought is meaningless. Just background noise that has no significance to you at all. Every time something like this comes up....no reaction...have a “whatever” attitude....it’s just background noise.

    What is more irritating is , when you have job in hand and when you get caught up in rumination . Stress of not doing job is building up aswell plus rumination is becoming stronger by spending too much time in it. Stress and pending work makes you more worse even that stress simply adds up to the stack of the rumination .
    But good thing is that actually work in hamd also helps you to slowly come out of rumination if you try refocusing into it.
    Its not easy to refocus but ya slowly detaching and reattaching with your daily work helps.

    • @zzzz546
      @zzzz546 Рік тому

      Hi,
      I know its long post but really need your expertise because i am at huge conflict sometimes the approach needs to be followed.
      What i am following is as below:
      FYI that i am using CBT approach(4R steps ,where i disregard content of the thought with Reattribure and Relabel ) and then Refocus ( which is now easy as Relabeling helps to look ocd thoughts as ocd thoughts ) medication and healthy life style and keeping busy in work which helped me to feel much better over the years , but may be 70 to 80 percent better . But when question comes on 100 percent and what happens if i leave medicine ? Am i ok to this extent with the help of medication ? then thinks about complete cure that what experts say like ERP is the gold standard . But i had bad experience back few years when i had try that. But now my mind forces me to consider that ?
      I have some question on ERP:
      It suggests to face fears , but eventhough ocd thoughts are stemmed from fear but later its just became the egodystonic thoughts you feel against your values or just mind tricking you to make you discomfort or irritates you.
      Also considering on the fears , regarding facing them and what happens with that? because fear is endless if you dig into it.
      How normal ppl handle fear ,they get some amount of fear and they just neglect it to certain extent or switch gears . Why cant we do that ?. Either our fear is irrationaly amplified version OR we lack neglecting part OR filtering to switch the gears OR self confidence OR some amount of belief that helps to over come the doubt and the negative thoughts. Example , before going to exam even though i might be prepared for 80percent of the syllabus and when negative thoughts say also huge fear that you get all remaining 20 percent in the exam , but having even a pseudo confidence that chances of remaining 20 percent to come in the exam is very less and better to stay optimistic helps .While what i understand from ERP approach is stating "ya i may get 20 percent of what i left in the syallabus , what ls the deal ? I may fail the exam ? ". What you are having already high anxiety and that leads to more negativity as we are not talking that in neutral perspective , we have been already stressed and negatively biased and it fuels into more of anxiety. It is just an example there are more scenarios with my themes like any bad happens to any family members , and bad disgusting thoughts on god and performance anxiety.
      Because fear is always going to feel you fearful.
      Then your mind keeps on dwelling over fear and you might get sad and feel depressed also. Like for normal ppl ,they say if you think negative you feel negative . Its like feeling fearful is always expecting negative outcome . So your nature becomes like that .
      I do not understand logic of ERP here . Its asking you to be in a negative pit or cognitive misunderstanding or stay with irrational belief ?. Every time response is fear and ERP says to do Response prevention but here Response is fear , then where is response prevention?. So that means we need to accept cognitve distortions as true ? I actually got more bad following this stupid classic ERP.
      Its like ERP is asking to enter Rabbit hole , stay there , come out, but get the point we have ocd anxiety , its keeps digging the hole with fear and rumination . Then you still go into groove, rather than staying on the ground . I really faced this , what kind of approach i need fo follow, where i am going wrong .
      May be ERP is saying just go into the hole and come back without digging into it just stay there, its ok if you feel discomfort. But what about content of the thought like there is mud in the whole it gets stuck to you . Its feels more worst as you cannot really seperate the content of the thiught and fear because they are so glued. May be we need to handle first with proper cognition to seperate them ? Then stop rumination which keeps digging the whole ? .
      I mean if ERP is done without handling these can cause more harm than better. Because i know ERP is difficult but heard it leads to you better but i only felt more misery . Then everybody talks its a golden standard and i feel i dont have enough guts to handle it and force myself to try . Its a conflict , its like you dont go near fire because you might have already burnt and learnt , but there is a unwilling force to fall into it in the name of theraphy. May be its even like this , ERP is actually is like peeled Apple and i am confusing it with peeled Potato which looks same and my ocd tricking me to think potato as apple ?"but when i read about ERP , it says we initially feel bad or worst but how i know is this really ERP or my borderline ( not typical but sounds real problem than old irrational patterns ) ocd tricking me in the name of ERP ?

      Hi James.
      It is common for people to think about their interests and pursuits. If your thinking does not cause a disruption in your life or prevent you from working or interacting with people or cause you suffering, then I would not be too concerned. However, the part about having to write it all down does sound concerning. Could that be a compulsion? Could you stop writing it down if you wanted? Other than that, it sounds like you are a motivated, deep thinker and good things may result from that thinking.

    • @PaigePradkoTherapy
      @PaigePradkoTherapy  2 роки тому +2

      Hi Joe. Yes, stopping the compulsion is practicing Response Prevention and it contributes to stopping the ocd cycle. We cannot stop the initial thought, but, we can stop reacting to the thought. Eventually the obsessive thought stops coming as frequently because it is not being reinforced and new neuropathways are created that are not associated with a fearful response.

I have pure o. Mainly suicidal thinking and also getting remarried. Relationship stuff. What approach would you suggest. The three days I had last week that were better I just sat with the thoughts and tried to not ruminate. I'm struggling knowing how to do both or if I should try to do both. Just an FYI. It is confusing for everyone, you are not alone. And every therapist treats ocd slightly different. There are lots of ways to do exposures. I try to design exposures according to research and the latest research on exposures is called Inhibitory Learning Theory. I use several approaches to ERP that keeps the client learning and keeps their brains surprised and keeps them emerged in treatment. First, I recommend identifying all of your compulsive behaviors and obsessive thoughts. From there, you pinpoint your core fears. Most people have several....some of yours include a fear of losing control and a fear of having these thoughts forever along with fear of harming self or someone else, and never knowing about certainty of my relationship. Then, you develop several ways to hit these core fears. Writing & recording scripts, recording trigger words, watching certain movies, etc. Every day, practice ERP for at least 2 hours a day. Try to change up little things about how you hit your core fear. When you are not practicing a planned exposure, then, use the I. A.M. technique, or you can even use I. A.M. when listening to scripts. When listening to scripts or doing other exposures...no compulsive behaviors. I hope my answer helps you and many others as well. I really need to review this question on a Q & A video.

I recently got remarried to a lady I've known for a long time, but I'm always wondering if I should have married her, married someone else, out even got remarried.... Would you suggest this "I AM" therapy for this? It just feels so real that maybe I made a bad decision. I've been trying script reading for ERP as well. Any suggestions please.

      I am sorry that you are experiencing so much doubt. I work with people with ROCD often, and I understand the suffering. Yes, I. A.M. works well for ROCD, and I use scripts as well in treatment. Your main treatment must be fully accepting that the questions you are asking yourself are unanswerable questions. Every person on the planet must tolerate their doubt in relationships. We have no guarantees. OCD creates doubt, you must say, “oh, there’s that ocd doubt again. My job is to not try to answer the questions, my job is to tolerate and live with the uncertainty and not knowing.” I also have my clients “double down” on expressing and showing love to their partners regardless of their uncertainty.

      What you are experiencing is very common. Doing ERP on images of children and your little brother, and then talking to your brother and spending time with your brother will be part of your exposure treatment. You can also write scripts that trigger your fears. I would recommend that you do these exposures several times a day, 15-20 minutes at a time, with no rumination. If you do these exposures several times a day, you will see improvement and these intrusive thoughts will die down. POCD is horrible, but it does not mean you are a dangerous person or a bad person. It seems to happen to the most wonderful people that would never, ever abuse or harm a child.
