Especially OSHA. That agency completely failed it's mission. From testimony it sounds like OSHA had impossible backlog and insufficient staffing to handle case load. So maybe OSHA is prey to lack of adequate funding from congress, like so many other regulatory agencies under Republican congress.

@TGP109 My understanding is that the passengers signed a waiver/were warned about the risk of going down in a submersible, but no one sat them down and explained how shoddy the design and execution was, and they should have been able to trust that it was sound. Even the question of “has this been classified?” may not have even occurred to them, they were completely out of their depth (so to speak) to make an informed decision. Don’t get me wrong, I think the reason this story continues to fascinate is because humans love a story about hubris punished. But OceanGate relied on the public perception that subs are safe, which was based on decades of development in design, engineering, fabrication, regulation, oversight, the list goes on. Lockridge talks about this, and how high the standards are in every other part of the industry, and OceanGate was a singular anomaly. They floated (so to speak) on that established trust and betrayed it at every step of the way, to horrific results.

No one actually reads papers they sign, and I was SHOCKED when I started looking into this story that no one has died in a submersible for 50 years BECAUSE of the standards set by certifications, and because Rush bypassed those certs, 5 people have died. Before then, the number of deaths in 60 years for submersibles was 1. That requires a level of research most wouldn’t do. I was SHOCKED to learn no one’s died in a submersible accident in 50 years. That’s insane. Also, this is the 2nd hearing and there’s already a 2nd person saying Cyclops 1 was the first and only unregistered submersible they’ve ever worked with. So 100% Rush was preying on a reputation he refused to participate in, like you said.

And it's all the more tragic that it all was for nothing. If he had not been there, the exact same probably would have happened with Rush managing to kill not only himself but also paying passengers. This failure seems to be largely thanks to a failure, or lack of legal power, of regulatory authority about commercial submarine travel.
@@Knuspabrot YES. It’s important to note that Rush was specifically playing the rules and lying to exploit loopholes. They can now tighten regulations to make sure manned submersibles have to be certified, make sure all members of the submersible have the proper paperwork and have to prove it, etc. And make these regulations international, that way they have to be adhered to regardless of whose waters the dives are being performed in. Rush got around US regulations because the Titanic is in international waters.

I was reading every in depth analysis I could about this in the aftermath, so David Lochridge was obviously a name familiar to me, and I knew about the inspection. I’m glad to put a face to the name and hear how much he tried to keep people safe before being fired. Reading about it was already so troubling. Hearing the specific details makes it horrifying, and so egregious.
So frustrating to see that honesty and integrity aren’t allowed to come forward until the evil is done. Glad the truth is finally getting to come out though. I’m sure he, along with others, wish it didn’t take this tragedy for his voice to be heard.

There’s so much wrong, that’s impossible to list them. This was not an accident waiting to happen, this was premeditated homicide and suicide.
More specifically, whoever was in charge of OSHA during that 2017 time period that Lochridge was thrown under the bus is the one who has 'splainin to do. The every day employees of each department do not set the agenda or priorities for the departments they work for, they do what they can under ever changing circumstances, i.e. the top person in each executive branch department changes whenever we have a new POTUS. That's simply a fact. A fact that needs to change. Instead of political appointees being in charge of each department, it should be someone from within the department with a long history of understanding WHY that department exists, and WHAT that department should be doing. OSHA was set up to protect employees not employers. However, depending on who is the new person in charge of OSHA, we may see what happened here. OceanGate, as an employer was prioritized over Lochridge, the employee.

@@Liminal-Galaxy-System6819 Only Paul-Henri Nargeolet's family has sued, citing the lack of safety certification and the use of unstable carbon fiber, but given he was fully aware of the sub's design flaws and the industry's many concerns and still chose to work for OG as an on-board guide, the suit is unlikely to succeed. The two other families have not sued but it's possible they ruled it out after reviewing the waiver everyone signed and knowing a bankrupt business can't pay compensation. Also, this happened in international waters and OG was registered in the Bahamas, a very business friendly country, so once this investigation wraps up, any further lawsuits would be surprising.

Mr. Lockridge, I have learned from similar things happening to me but in a different set of circumstances that the people who are called heroes are those of us who just feel like we were simply doing our jobs properly. I never saw my job as being a hero and I'm sure you didn't either but when you help people out of a tight spot they just feel that way about you sometimes. What we also have in common other than just doing the damn job we were supposed to be doing properly is that we were willing to stand up firmly against people who had decided to put themselves in charge when the task seemed like fun to them without ever considering the importance of due diligence and actually knowing what the fuck you're doing. the disagreements between Nissen and Lochridge are fascinating. You get the sense that neither of them are giving the full story, not because they're trying to hide something, but simply because they worked in different teams, had different social groups and fields of expertise and have different personalities. Both of their retellings are true, yet neither of them are THE truth.
Nissen was much more cautious and calculated in his testimony, it is self-evident that he is only disclosing what is convenient for himself. Lochridge's answers are far less cautious, much more immediate and to the point. He's hiding nothing.
No, they were well aware of the dangers and signed disclaimers very much detailing that death is a real risk. All passengers were rich people and wanted bragging rights to visit the Titanic, so you could argue ego killed them all. Furthermore, PH wanted to see the end with the Titanic so was not concerned with a critical failure.

Why are taxpayers paying for wealthy peoples' ill-conceived adventures investigations? We know what happened and why. It's not like there are dozens of businesses like that one out here operating and we need to add some "best practices" to the mix.
To ensure that laws and regulations are created that protect employees and passengers of all income levels from dangerous conditions in under-regulated fields. Also, for the practical exercise of conducting a large investigation with transparency and public scrutiny, as most citizens do not follow Coast Guard investigations. This is a unique opportunity for Coast Guard recruitment.
This is so insensitive. People lost their lives to the same capitalist machine that us Poors also suffer under. I dispute this characterization of experimental aviation, at least in the US. While builders don't have to use certified avionics and materials, each aircraft still has to pass initial certification after the build. You can get the okay with an exotic engine or novel materials, but with duct tape or clearly unsafe workmanship, forget about it.

This guy came into this hearing with his suit and his extensive knowledge and his insistence on excellence and unloaded both barrels. You can hear his frustration. Testimony continues to be eye-opening and frankly shocking
OSHA should also be held accountable here. They didn’t take Lochridge’s complaints seriously.
Applause for this seafarer. How frustrating it must have been to predict the future and have no one take him seriously. OSHA and/or the Coast Guard need to testify why they did nothing.
Especially OSHA. That agency completely failed it's mission. From testimony it sounds like OSHA had impossible backlog and insufficient staffing to handle case load. So maybe OSHA is prey to lack of adequate funding from congress, like so many other regulatory agencies under Republican congress.

I would think that the average case crossing their desk falls more under wrongful termination that does not affect the public. Now maybe it was a lot more when he first filed but I have to question the guy's claim that he hadn't started investigating - but then oceangate knew that he went to osha? Further, osha lawyers should have cease and desist letters ready to roll when oceangate started to issue threatening letters. Why weren't these issued?

After listening to Nissens obfuscating and sense of totally diminished responsibility its clear Lockridge was the only grown up in the room.
So OSHA reported Mr Lockridge's complaint to Ocean Gate, and then hung him out to dry at the hands of Ocean Gate attorneys!? Sounds like OSHA could have prevented the whole tragedy and spared Mr Lockridge and his family much pain and expense in the process.

I just assumed they keep them anonymous, or don’t even tell the company they are investigating until it gets to a certain point. If it’s public health concerns I’m just shocked that they tell the company what’s going on and there actually is no protection from getting sued by the company in response. Disgusting.

@TGP109 My understanding is that the passengers signed a waiver/were warned about the risk of going down in a submersible, but no one sat them down and explained how shoddy the design and execution was, and they should have been able to trust that it was sound. Even the question of “has this been classified?” may not have even occurred to them, they were completely out of their depth (so to speak) to make an informed decision.
Don’t get me wrong, I think the reason this story continues to fascinate is because humans love a story about hubris punished. But OceanGate relied on the public perception that subs are safe, which was based on decades of development in design, engineering, fabrication, regulation, oversight, the list goes on. Lockridge talks about this, and how high the standards are in every other part of the industry, and OceanGate was a singular anomaly. They floated (so to speak) on that established trust and betrayed it at every step of the way, to horrific results.

No one actually reads papers they sign, and I was SHOCKED when I started looking into this story that no one has died in a submersible for 50 years BECAUSE of the standards set by certifications, and because Rush bypassed those certs, 5 people have died. Before then, the number of deaths in 60 years for submersibles was 1.
That requires a level of research most wouldn’t do. I was SHOCKED to learn no one’s died in a submersible accident in 50 years. That’s insane.
Also, this is the 2nd hearing and there’s already a 2nd person saying Cyclops 1 was the first and only unregistered submersible they’ve ever worked with. So 100% Rush was preying on a reputation he refused to participate in, like you said.
OG's successful silencing of all critics and smoke and mirrors social media campaign, as Lockridge accurately described, prevented potential clients from doing any meaningful due diligence.

Mr. Lockridge has the loveliest of accents! I just started watching but I hope he talks a lot because, not only is he clearly very competent, but it is delightful to listen to him!
The smartest thing this guy did was to get out of that company as soon as he did.
And it's all the more tragic that it all was for nothing. If he had not been there, the exact same probably would have happened with Rush managing to kill not only himself but also paying passengers. This failure seems to be largely thanks to a failure, or lack of legal power, of regulatory authority about commercial submarine travel.
@@Knuspabrot YES. It’s important to note that Rush was specifically playing the rules and lying to exploit loopholes. They can now tighten regulations to make sure manned submersibles have to be certified, make sure all members of the submersible have the proper paperwork and have to prove it, etc. And make these regulations international, that way they have to be adhered to regardless of whose waters the dives are being performed in. Rush got around US regulations because the Titanic is in international waters.

I was reading every in depth analysis I could about this in the aftermath, so David Lochridge was obviously a name familiar to me, and I knew about the inspection. I’m glad to put a face to the name and hear how much he tried to keep people safe before being fired. Reading about it was already so troubling. Hearing the specific details makes it horrifying, and so egregious.
This is a case the seems to get worse the more I learn about, even knowing it was bad going into it. ☹️

having worked for a classification society myself, the safety-, building- and company standards that were described in this meeting left me both speechless as well as angry beyond measure. There’s so much wrong, that’s impossible to list them. This was not an accident waiting to happen, this was premeditated homicide and suicide.
I remember seeing a video of Rush bragging how the viewport would bulge inwards more than 20 mm at full depth. Insanity.

More specifically, whoever was in charge of OSHA during that 2017 time period that Lochridge was thrown under the bus is the one who has 'splainin to do. The every day employees of each department do not set the agenda or priorities for the departments they work for, they do what they can under ever changing circumstances, i.e. the top person in each executive branch department changes whenever we have a new POTUS. That's simply a fact. A fact that needs to change. Instead of political appointees being in charge of each department, it should be someone from within the department with a long history of understanding WHY that department exists, and WHAT that department should be doing.
OSHA was set up to protect employees not employers. However, depending on who is the new person in charge of OSHA, we may see what happened here. OceanGate, as an employer was prioritized over Lochridge, the employee.
FINDINGS: "Our findings are that the current safety regulations and standards are adequate. Had OceanGate followed them, this disaster could have been avoided entirely."

David Lochridge is "THE TRUE HERO" of this debacle that was Titan and Oceangate. I thought his efforts were fantastic and worthy of an award. So few people EVER put themselves and their family through what he did to try and stop this fiasco before the tragic end. Respect!!!

@@Liminal-Galaxy-System6819 Only Paul-Henri Nargeolet's family has sued, citing the lack of safety certification and the use of unstable carbon fiber, but given he was fully aware of the sub's design flaws and the industry's many concerns and still chose to work for OG as an on-board guide, the suit is unlikely to succeed. The two other families have not sued but it's possible they ruled it out after reviewing the waiver everyone signed and knowing a bankrupt business can't pay compensation. Also, this happened in international waters and OG was registered in the Bahamas, a very business friendly country, so once this investigation wraps up, any further lawsuits would be surprising.

Mr. Lockridge, I have learned from similar things happening to me but in a different set of circumstances that the people who are called heroes are those of us who just feel like we were simply doing our jobs properly. I never saw my job as being a hero and I'm sure you didn't either but when you help people out of a tight spot they just feel that way about you sometimes. What we also have in common other than just doing the damn job we were supposed to be doing properly is that we were willing to stand up firmly against people who had decided to put themselves in charge when the task seemed like fun to them without ever considering the importance of due diligence and actually knowing what the fuck you're doing. So why didn't the Coast Guard ever go out there to inspect/check things out knowing there was some type of vessel doing dives. Why should Lockridge have had to chase them down?? Ridiculous. the disagreements between Nissen and Lochridge are fascinating. You get the sense that neither of them are giving the full story, not because they're trying to hide something, but simply because they worked in different teams, had different social groups and fields of expertise and have different personalities. Both of their retellings are true, yet neither of them are THE truth.
Nissen was much more cautious and calculated in his testimony, it is self-evident that he is only disclosing what is convenient for himself. Lochridge's answers are far less cautious, much more immediate and to the point. He's hiding nothing.

No, they were well aware of the dangers and signed disclaimers very much detailing that death is a real risk. All passengers were rich people and wanted bragging rights to visit the Titanic, so you could argue ego killed them all. Furthermore, PH wanted to see the end with the Titanic so was not concerned with a critical failure.

I'd imagine they probably were not legally allowed to take paying passengers in US jurisdictions, but were allowed to take passengers for scientific research.

Why are taxpayers paying for wealthy peoples' ill-conceived adventures investigations? We know what happened and why. It's not like there are dozens of businesses like that one out here operating and we need to add some "best practices" to the mix.
To ensure that laws and regulations are created that protect employees and passengers of all income levels from dangerous conditions in under-regulated fields. Also, for the practical exercise of conducting a large investigation with transparency and public scrutiny, as most citizens do not follow Coast Guard investigations. This is a unique opportunity for Coast Guard recruitment.
This is so insensitive. People lost their lives to the same capitalist machine that us Poors also suffer under. They’re wanting to find out what happened so they can pass laws/regulations and make sure something like this never happens again.
People lost their lives for no reason other than Rush’s ego. That is unacceptable, no matter who lost their lives or what kind of money they had.
It’s not like these people got into a sub that they knew was faulty and lacking regular maintenance. Rush lied, and they paid for what they believed to be a SAFE trip to see the Titanic. That’s not what they got.

I dispute this characterization of experimental aviation, at least in the US. While builders don't have to use certified avionics and materials, each aircraft still has to pass initial certification after the build. You can get the okay with an exotic engine or novel materials, but with duct tape or clearly unsafe workmanship, forget about it.
