Badly programmed: it should've taken longer to detect the load rather than jump to the main wash. Also, I noticed that many machines nowadays relies on the weight of the load rather than its degree of absorbency which should be the main priority; this is in fact what happens with a load like this, being bulky and highly absorbent: the machine is yet washing it as if it had a small load in. Having said that... I'm sure it would've eventually wet it given it more time... but I doubt it would've been enough to shift a stain if it happened to be right in the middle of that load :-/
Whenever I wash duvets, I use a bucket to add a truckload of extra water (usually 10-12 litres) to make sure everything gets wet....
