    Dr. Tim, you are amazing! I have terrible migration and cannot find a good practitioner to dissolve, they have all said to add more to disguise. I know from experience this isn’t the way to go. I hope your good word spread to everybody!

      If you have migration then the right course of action is to absolutely go for reversal. Adding more will only make it worse.

    Dr. Tim can you do a video on lip filler nodules. Why they occur and what to do to get rid of them? Do they go away over time?

    After the bird came I couldnt listen mo more but stare at the birdie. Great video by the way. What is your opinion on Restylane kisse? For liptreatment?

      Many people like it a lot. We don’t use it I’m afraid so can’t really comment.

    Until there are real studies about filler migration I would advise to wait. Without an expensive CT scan it seems to be hard to tell where the filler went to.

    I find more migration initially in patients who swell significantly. I believe the swelling can act to displace the product in some cases. Because of this, I have taken to premedicating patients with an antihistamine like Zyrtec. It seems to be useful. What are your thoughts on this? Thanks for helping us all keep our patients safe and happy.

      I think it’s a valid point but it’s a very small number who swell to this extreme. Absolutely we have seen migration secondary to extreme swelling. But we do not advocate prophylactic antihistamine as a rule. Maybe if someone knows they’re a big sweller in the past. But could we be cloaking a warning sign? Especially in the rare even that a patient may need emergency reversal and the patch test results are potentially masked.

      This is interesting.
      I had fillers done and didn't swell or bruise much at all, but mine migrated horribly after the first couple of days. Never looked right :(

    Doctor, I had a cheek filler a few years ago, and only after one week, the filler was gone. I consulted with many cosmetic doctors and experienced nurses first, and they said, my injector probably gave me fake filler or hydrolyzer instead of real filler which is why the filler was gone after a week but you are telling us fillers can break down easily because some patients may not absorb it. The mediSpa never refunded me because I did not claim a refund. Is it appropriate to ask for a refund if the filler goes away fast?

    I was told that, if you wait In- between treatments a decent length of time (not sure the protocol per person) there is less likely a chance of migration as the tissue has had time to stretch and form a stronger bond. A little like having larger breast implants put in in stages?
    Is this another good way to prevent migration ?

      It is better advice yes. Lips for instance. It is a small space and if over treated the filler will follow the path of the least resistance which may end up in the white lip or behind the lip. Little treatment at a time allows things to settle before stretching. But ultimately it can still happen even with cautious filling.

    Dr Tim do you thread on the lips. I feel that with doing treads you won’t have filler migrations.

    Does massaging help dissolve old filler?

      Massage can help it disperse a bit.
      But in cases of migration it will only make it worse. Migration has caused a larger surface area pulling in more fluid from surrounding cells creating a more puffy appearance in that wider surface area.
      Dissolving is the best choice or leaving it to let nature take its course.

      Migration means that water is being pulled into the tissues in that area. Technically you could be dispersing the filler more and can create a larger surface area for more water to be pulled to.
      Ultimately the best option is to reverse if it’s causing a distraction.

    So if a person only does filler a few times end stops and it was placed well, does it still eventually migrate?

      They key is how long it lasts- in most people it breaks down before it migrates, but if it lasts for 10 years then it could.

    That is so cute the bird is such a good sign your an amazing man!! So why not just do a lil Botox to prevent the muscle from pulling & therefore no migration?

      Migration can be caused from an insufficient vermillion boarder potentially secondary from cold sores. Or just literally too much product in such a small space. Even half a ml can cause migration in some individuals. Toxin won’t be the answer for many patients with migration.

    Got mine to make them plump not bigger. Looked great and just recently they are blowing up and I feel it in my cheek next to my nose. I’m terrified
