We used to joke to our IBM contacts, "Eh, where is the "I" in IBM?" Eventually, things resolved themselves. But still, look at your keyboard and see what's missing!

Not remembering name. The way my ADHD memory fails mei's my inability to remember someone's name, until I have used it with the person several times, and sometimes seen it in writing over a short period of time. I have grown up in the US & lived here my entire life. I visited a local church who's members have names that are actually, Mary, Larry, Ann, and so on. I convinced them that everyone needed to go back to wearing name tags. The typical tricks to remembering a name do not work for me. How should I let someone know this without offending them.
I also struggle to remember names, especially if I'm meeting multiple people in a row at a large gathering (loud, socially stressful, and l can only keep, like, 3 names in my head at once). I've had good results from (like Anpu suggests), repeating the person's name at least once after they say it to make sure I got it right, but also just saying upfront, "I promise to do my best, but it's really hard for me to try and learn a lot of names all at once, so I will probably need to ask you to remind me later." I also find that for me it's a lot easier to remember someone's name after I've gotten to know them a little bit, so if I'm introduced and then we chat a bit and are about to part ways, that's the point at which I will try hard to remember to say, "it was so nice meeting you-- can you remind me of your name again? I don't want to forget it." Or even at the end of the gathering, I will sometimes circle back and seek people out and say, "Hey, I really enjoyed talking to you. Can you remind me of your name one last time?" I feel like even though this is a little awkward, it shows that I'm making an effort and that it's not a lack of interest/care that is making the memorization hard for me. In general, I always find that a name with a goodbye is easier to remember than a name with a hello. Like, when I'm first meeting someone I'm just kinda too overwhelmed to retain it, and then I'm concentrating on what they're saying next. But if I get the name again as we part ways, it sinks in better along with the memory of how the conversation went. If this is also true for you, you can even say that upfront, like, "I would love it if you could tell me your name again when we say goodbye, it will just help me remember it better." Basically as much as you can do to convey that yes, you want to succeed at remembering the name, and here are the tools and accommodations you need to help you succeed.
