    Here’s my personal method for fissure recovery:
    I made sure to pass stool once every two days (because if you go everyday you’ll not give it time to heal.)
    On my “non bowel day”: no fiber, did water fast, eat only fruits and liquid foods to break the fast, and take one pill of stool softener (example is Colace)
    On my “bowel day”:
    Eat fiber food and take 1/2 pills of stool softener. Eat Apple. Do cardio to aid digestion of food you eat.
    Every day, I also applied a fissure healing cream (research and buy one.) Don’t directly touch the sore while applying else it will get irritated.) I hope this comment is able to help at least one person going through this terrible ordeal.

    I wish there was a doctor like you close to where I live. I have 'pelvic floor dyssynergia' now, likely caused by surgery I had to correct total 'pelvic organ prolapse'(all of them😮) and my uterus also had to be removed. I didn't have these odd, 'uncoordinated' pelvic floor issues before that surgery. I just had the prolapses. My life has been a nightmare ever since. Listening to your videos bring me ALOT of comfort. These things are quite scary. I hope you realize how much you're touching people with your informative and understanding videos. Thank you for that.

    Thank you so much doctor. In crying as I watched this. My doctor in NHS is not even meeting me personally. He did not prescribe any pain or poop softening medication and I regret coming here leaving my home country. Really bad medical care here.

      Same here mate, I came to the UK but health system is a joke. I suffer from this and my GP just said eat fibre which I already do. I bought an oitment over the counter that called anusol, it helped a bit.

    I ‘ve had these issue all my life. Kegels IMO (if u have constipation) are the worst thing u can do. I have done them under the direction of a physical therapist many times & they always make the constipation worse. Deep controlled breathing 4 an hr a day is the best thing i have found

    8. medications-muscle relaxants

      The only thing that helped me healing was by lubricating it with Vaseline. That has been a life saver for me.

      I've used Clofen/Baclofen, works great! I've had anal surgery for a fissure, worse pain I've ever felt, these drugs helped.@@mahalaxmi77

    Thank you for all that you do. These videos have helped me in ways that words cannot describe. I tell everyone you are my favorite doctor and I truly mean it. You inspired me to seek out a pelvic floor therapist to help myself heal. After 8 years of chronic repeating fissures and anal spasms, botox, surgeries, medications... I'm hitting the reset button and going to approach it from the therapeutic and emotional health point of view. I just started a week ago and I'm ready for this journey.

    Hey its work for me if you have conception very hard just drink i spoon olive oile with hot water over night. And then when you wakeup drink hot water with lemon then wait for few minutes...👏👏

    👏👏👏Great advice, Dr. Chung! I would only add work on your posture when sitting on the toilet. I was getting better with the kegels, breathing, and sitz baths, but when I corrected my posture things really improved.

    Hi your a great doc wish I could see u I suffered my whole life even as a child with constipation my mum used to give me enamas in the 60s I had a hemmroidectomy at 18 it was horrid that operation off work 6 months since then 30 I got ms with makes u have hard poops no poops to many poops I’m on restoralax and stool softeners mine is all psychological now if I feel one little thing of pain, I gotta run in the bathroom and I don’t wanna go out because of bowel problems serious bowel problems so I just stay home. I would love to go to somebody to talk about this problem, but I live in a little town and there are no therapist around here. I would have to go to the city and I don’t drive anymore.     Dr. Chung I had a hemroidectimy 5 days ago. My question is is it normal to poop while I'm doing a sitz bath? I just feel more relaxed when my but is under warm water. I know it's not the most sanitary. Also should my stool be thin and longish. And should the bowl movement be painful?

    Along with having hemorrhoids thrombosis and anal fissures I am constipated too. I ate some prunes and pooped enough to fill three toilet bowls and that made my hemorrhoids etc worse. I now can’t sleep or sit down without being in excruciating pain. I’m going to try the fasting diet and do only liquid and eat fruit. I am at my wits end and almost ready to give up

    one big minus is your forced to use earbuds to listen to your music on your watch even though the ultra has a good speaker built in , find it a pain to be forced to earbuds when i walk my dog for 10 minutes , galaxy and others give you the choice something apple should offer seeing as apple used to be music nuts

    Hi Doc thx so much for being so generous and positive ... I am diagnosed with multiple superficial cuts in the anal region. They only seem to be in the outside area of anus.
    I have several questions
    1. I did kegels but my pain actually increased due to the action of pulling as I feel the skin get stretched. Is that possible?
    2. Can you use Diltiziam as and when the pain comes back? I have used it for straight 2-3 months. Thereafter not so much..now the pain is back but no bleeding..never had bleeding more than 2-3 spots in the past. The anus is sore and tender
    3. Do you do video consult for folks in other countries?

    These recommendations and exercises are appreciated - and I’ll definitely pursue them - but a timetable lasting months doesn’t help me right now. I need a more immediate fix. I haven’t pooped in 17 days and for the last 10 days, I’m spending the majority of every day sitting on the toilet hoping for a miracle. My body is almost constantly trying to expel the stool - but the passage just won’t open - even when I push with eye-popping strain. What can I try to help get past the immediate problem? I’ve been taking a stool softener for the past week and have increased my water intake. I’m scared to try a laxative because there is already plenty of pressure behind the poop - I need something to help open the door/relax the sphincter? Please help!

      I think if you stick your finger up there and swirl it around it will create that sensation, lube it up with virgin coconut oil, I know it does for me. I am having the same problem, this very issue seems to cause ejaculation issue as well, like the shooting out isnt very strong apparently the spihncter is also responsible to have a powerful propelling ejaculation , (not sure if you are a male or female) for men this affects ejaculation and the force of your pee stream. So as the doc said, and I tried, use the finger method and also I read about a few people who got linzess prescribed to them, they swear its the actual magic wonder medication, that finally gave their life back after years of living a poor quality life due to this issue. I am not at a stage that I need linzess, but I am able to manage with what I mentioned. Oh I just noticed you posted this 4 months ago, what did you end up doing, how is your condition now?

    Can you make a video about sitting and proper posture or chair to sit in with anal pain or other rectal issues? Right now my physical therapist recommends sitting on a donut cushion and getting up once an hour.

    yes my colorectal doc just prescribed a muscle relaxer for an anal fissure.(Methocarbamol) I had it cauterized a month ago but it still hurts
    and I felt it being in spasm after a BM. It has helped me.I do sitz baths and I use diltiazem too. I'll try anything!

    Just diagnosed with anal stenosis after a colonoscopy. They could only do it with a pediatric scope. Ive been dealing with this for decades and the relief i get is with miralax.. its literally the only way i function without major pain. I was actually happy in one sense when the dr told me this and my husband was there to hear it because he never took me seriously on it. Long road now with some appts with a specialist.. hope it can truly get better.

    Thank you for the video Dr chung. I just had surgery 5days ago to strengthen my anal sphincter muscles. But I feel the muscles are too tight and I'm usually in so much pain when I want to open bowels. What can I do to lessen the pain?
    My other question is, will my muscles dilate after some time?

      This was the 3rd time now undergoing surgery for the repair. I had a rectal vaginal fistula in 2018 after the delivery of twins through SVD.

    The first three you mentioned I do all three. The very last one diaphragm breathing. I do it all I have severe rectal dysfunction. Passing gas and poop causes severe pain very very hard to get out. The nerves are now damaged in my rectum took over two years to find out what was wrong now I have to get Diverted ileostomy bag, so I can stop using my rectum. Eventually, Using my rectum, the pain, the throbbing, the aching the nerve damage it is in the same with this happening to the inside of my rectum outside of my anus. My other phone fixed is going up my leg. I have vagina issues. It’s a real big mess. I live in Canada. If I had the money, I Would fly to California because I feel a little bit anxious because I think there are certain things that should I have been done that haven’t been. And I feel that you would do all the necessary testing I’m on Lyrica and diazepam. I’ve had vaginal Botox. Botox twice in my rectum in two different areas. I have rectal nerve spasms. I’ve lost the quality of life. I have zero capacity. I will need younger two kids. I have severe cramping all day and night pain all day and night in my rectum my God, my life is ruined. I’ve lost a lot of weight I just wish I had the money to come to USA to California.

    I’ve also had numerous biofeedback and pelvic floor therapy and nothing has worked. I’ve been told my anal canal will not open up all the way like external sphincter muscle is not working well sorry external sphincter muscle. I’m text speaking there’s not a lot of functionality test. You can get done down here in my province. But I literally have done everything that you said in this video and nothing is working for me. I can’t even pass gas normally which I haven’t done in over two years I use heat like crazy. I have to put a cream on my rectum to relax my sphincter muscles just to try and have a poop in the morning. Yoga moves just to relieve gas. I’m living a nightmare. I’ve never suffered in such much pain in my life. I have really bad cramping all day every day. When I eat it gets bad and then a few hours later it gets even worse. And I do have Constant groin, pain,pubic pain, tailbone pain, I have vagina pain, I never get relief. I’m in pain for 18 hours a day. Literally nonstop. I have a party Scotty sorry potty squatty. I’m text speaking. I gotta stop doing that. Literally I’ve tried everything and nothing is working. I think the disfunction is too bad. But because I don’t have a disease, I’m not allowed to get my rectum removed, but I’m never gonna have any quality of life. I still have to use it and I well with ostomy because of mucus. Even pushing a little bit causes pain. I started pre-menopause between 42 and 44. I just didn’t know but I knew signs were going on back then, but they were so irregular. I didn’t pick up on it that I stopped having. May 20 20 then by June 2020 that’s when I started getting crazy rectum symptoms and for a year and eight months was horrible and then I started to get those lightning bolt rectal spasms. I still get them but not as bad it’s just very painful and that’s what put me down.

    Try not to chest breath. I thought I was diaphragmatic breathing for a long time but PFPT said I was still breathing too much with my chest muscles and not enough in belly still.

    My Dr. proscribed Nifedipine ointment and it seems to help. But it's not something I can buy without a prescription.
    Is there a similar over the counter muscle relaxer for the anus that we can buy without a prescription?

    Dr. Chung literally what I feel now is hard for me to go for toilet and anus was so painfull and no toilet for 3days from now. Dr. Due to I eat to many strong foods like flour and biscuit....

    Relaxation is so counter to every bathroom visit. Three weeks after rhoid surgery everything seems pretty good till I have to poop. It just can't clear the anus, regardless of how soft.

    I am Amazed every CRS I have spoken too seems to know very little about this. And yet now I’m pelvic therapy from my own research she has diagnoses pelvic floor dysfunction with hypertonic pelvic floor. Suspecting cause a recurrent fissure that no one and no procedure has been able to see. She’s managed to ‘unlock’ the muscle enough to see some granulation tissue which is pink so hoping that’s good. What bugs me is the mri showed no signs of a fissure at all so surgeon dismissed me but the physio is now finding the evidence. Sigmoid camera found nothing (duh) but no one even mentioned an anoscope. I mean in your opinion doc if the mri is clear, could an anoscope still be worth it?

      An MRI is not good enough to detect fissures, unfortunately.

      @@yourfriendlyproctologist thank you for the reply. Would an anal ultrasound be any good?
      Is it possible for a fissure to heal at the surface but still be a wound under that skin?
      Or is an anoscope always the best? I’ve seen two CRS surgeons both are claiming they can’t see evidence of a fissure. Which is honestly insane.

    I have the same problem, after i poop I have to rest 30 minutes on my stomach and then I walk about 1-2 hours.

    WOW I could listen to him all day long, just his voice and his friendliness is very relaxing, its relaxing my spihncter already. But I have a question doctor can non activity like bedridden (no health reasons) mostly remote working, laying on bed, not leaving the house, working everyday, (covid period) cause pelvic floor issues? My problem is, I finally figured out its the anus muscle, I felt it very hard and whenever I push, it seems like I can't make the anus open at will. When I really have to poop, like if I ate a lot, then somehow I finally poop, but if I only ate a little then I have to struggle a lot to poop out the little food I ate, and found out its cuz I am unable to open the anus at will, like the winking thing the pornstars do lmfao... The farting had become louder after this problem started and it makes sense because the pressure built up and then when I had to fart the non cooperating anus ring is so tight the gas escapes with a very loud blasting noise. My question is, I never had any sort of pain so far, its just the constant battling against constipation (when the food was consumed less) but if I ate a lot and if I drink psyllium husk then I get to poop once a day and even then it feels like it doesnt get emptied because after the intial push and the majorty comes out the anus doesn't want to cooperate anymore. Your video is sort of confusing in a way in my cas ebecause I never had any actual pain, I had itchiniess like once in 3 months but no pain. I hope I can fix this after having watch this part 2 video. I'll post back updates later.

    I have done piles and fissures lasar surgery before 6 weeks. Still its very painful while bm. Can't live without painkiller. Doctor told me it will take time. If i don't get heal do i have to go for further operating regarding anal stenosis?

      No, I didn't feel pain, but it's still itching and burning.

      Sorry to hear your recovery is so painful. You will need to see your specialist to determine whether you have developed anal stenosis first.
