I am Sprinter suffering from patellar tendinopathy for over 7 months now. So e.g. 3x a week HRST and 3x Sprinting/Jumping exercises etc.? And does the pain score mean in general or during the exercise? Thank you!

I've been following your advice with HSR for the last two weeks and my morning pain and stiffness has gradually been decreasing FINALLY! My only difficulties with HSR right now are figuring out the 15/12RMs - and wondering if I should be increasing my weight even within the same week if the previous 12RM is now more like a 15RM?

As a gym rat my experience tells me it is VERY extreme to do more than 20 sets/muscle group/week within 3 reps to failure each set and the performance literature would support the idea that the sweet spot is somewhere between 10-20 sets/week on average. It would be near insanity to do more than 20 sets of 5RM back squats/week. However leading clinicians such as yourself (if I am understanding this video correctly), peter mailliares and others assert that with tendon treatment weekly volumes should be anywhere from 20 - 40 sets/week. Maybe this is because tendon rehab exercises are single joint where much of the performance literature is compound lifts? Can you shed some light on this discrepancy? Thanks for all your work!
Hi Nathan, I see your point but there is good evidence supporting this approach and clinically it appears to be very effective. We have to make a judgement with each patient, some will opt for a lower volume approach, others do very well with rehab like this.
I have been dealing with post tib tendon pain for a year, despite committing to a regimen of exercises that take an hour of my daily routine. It’s quite frustrating that I still didn’t succeed in getting back to running
Hi Carla, yes, you can but care needs to be taken to ensure the rehab is at the right level. In particular loading into dorsiflexed positions can be very provocative if used too early. I'd favour a slow progression and work with a podiatrist for their expert input where possible.

is there a way to say at which point in the treatment of lets say gluteal tendinpathy one can start to run again? trial and error?
