The small ketchup, mayo and so on packs only cost extra if you buy them seperately. If you get a whole menu like the Mc Menu you usually get a free one of choice for each "bigger" item like fries, nuggs and so on. So usually 1-2 packs per menu for free.
I believe it the price on mayo and ketchup depends on your location because in northern North Rhine Westphalia you normally don't pay extra for mayo and ketchup. Atleast if you order at McDonald's. Smaller restaurant like an "Imbiss" often charge extra for that.
I haven't eaten McDonald's in the last 10 years, but they used to get free mayonaise and ketchup. You could say how many you wanted and get the appropriate amount. At least here in North Rhine Westfalia. Since the McDonalds restaurant operators are only franchisees, I think that there are differences from operator to operator. Some give it away for free, as a kind of service to the customer, while others want to be paid.
Meanwhile some grocery stores have also some vegetarian/vegan options "toGo". Ketchup/mustard etc. wasn't always extra to pay. They wanted to make money but I think as well to avoid garbage. So you have to pay more often for extra garbage portions and less if you can use a big ketchup container. Free portions often end up in the garbage without being used anyway 😞
The fast food places here seem a lot cleaner than what I've seen in the States. Less one-use items the better. I kinda want to go to a McDonalds in Paris as I saw they have reusable everything....

I hate to tell you but Parmesan is not vegetarian - but being "vegeatrian" is alway hard to define exactly. Lots of people have different feelings about it. I met vegetarians in Germany that all defined themselves as such and some eat parmesan, some eat gummybears and some even eat fish - so hey do what you feel is right :)
OK and reading the other comments it seems that sauces are indeed sometimes free. I always lived in Bavaria and in the last 25 years I allways had to pay extra for the sauces - including ketchup.

For some time now you are able to see the ingredients of the individual products on the McDonald's homepage. I haven't eaten McDonalds since I looked in there. McDonalds couldn't add any more flavor enhancers and preservatives, the "food" is more chemical than biological and 100% unhealthy..

Apparently the fries in Australia are vegan (b/c of a court case by Hindus - as maccas didn't disclose used to be cooked in tallow / beef fat)

i always had to pay for ketchup/mayo if i wanted some for the fries (berlin/bremen/switzerland). for nuggets you get those without additional cost.
