Interoperability of healthcare data between various EHR/EMR systems....... could be another potential correct option for question two - "fragmentation could be mitigated to..".

Definitely, crypto is the future. I am thinking if anyone can start the open source software based on crytpo for medical records and insurances. There was a coin a while ago which started the project but i think it failed. Name was something like Medcoin.
But i think health sectors can be improved with this future tech.
The term for having networks speaking together is known as interoperability. One option is to force providers to interoperate with each other.

I believe EPIC and Cerner may be the top 2... Sometimes it depends on the size of the hospital or system and what they can afford...

Dr Ethan's explanations are really informative and I actually learned a lot for my diet! He is the most knowledgable med student in the UK and he talks about conditions and health symptoms!
