Sean IS John Mallory and not his irish 'friends' who is only named Nolan. This film is the story of Sean "John" Mallory. The girl in white symbolizes the Revolution, whatever revolution that may be, both guys fall in love with the Revolution, but in the final scene Sean is disillusioned about it and as second thought about it all. The mexican character is Leone alter ego, he doesn't believe in politics or revolutions at all.
His name was never Sean. That's just something a lot of people that don't pay attention assume because of the obvious Irish stereotype. His name was always John and that's why Juan says something to the effect of "You're John and I'm Juan!" Juan is the Spanish equivalent of John. So John and John, which is why he was so visually excited
It's "Sean, Sean, Sean" referring to Coburn's characters' IRA friend who we see in the Ireland flashbacks. Coburn's John Mallory is riddled with guilt over the death of Sean and this guilt informs much of his motivations throughout the film. Sean is constantly referenced throughout.
Just to clear up a few points..."Sean, Sean, Sean", as I understand it, refers to John's past in Ireland, not any guilt over killing Mallory. Sean killing Mallory can be viewed two ways: they are both in love with the same girl, but also, based on Mallory's pleading look, a mercy killing that Mallory begs for so he won't betray other IRA members. Once John is in Mexico, he becomes increasingly disenchanted by revolutionary politics, not only in the final scene, but before then, because of the death of Juan's children, Dr. Villega's pointing out fellow revolutionaries, and a discussion with Juan that it's the poor who suffer in the process. When John throws his copy of a Bakunin book in the mud and tells Villega that he only believes in dynamite, it seems clear by then that he is rejecting violent revolution as a positive act. He sees no other personal role for himself however, and after being wounded in the battle against the Federales, he kills himself. This cue does not play over the opening credits with its horse and wagon but over a scene of John watching preparations for blowing up the entrance to the bank/makeshift prison in Mesa Verde (and it is truly one of Morricone's finest 'invenziones' with its nearly Baroque layering of whistling, vocals, guitar, strings, and percussion thumps).
