I actually just had to do this myself. In Access on the right, you will see the Donor Statements Report. Right click it and choose "Design View." From here you can Insert Image or Label from the Report Design Menu.
Hi Jeff, thank-you for a high quality video that addresses this problem of donor statements that nonprofits need to deal with. I really appreciate your investment of effort and attempt at a solution. It is much more than Intuit seems to want to do! The solution, however, is not one that I will be using, but it is clever and you seem like a really wonderful man, so I wanted to encourage you and give you a thumbs-up.

Agreed! I'm now using an app called "Donor Receipts" that connects to QuickBooks Online and, so far, it seems OK. The format looks so much better than the QuickBooks Online form even after I customized that QuickBooks form as much as possible. Donor Receipts even has a place for me to put my signature (which is required). Anyway, grace to you, @@JeffPospisil
