  • In this video, Dr. Tansavatdi explains the difference between the two common techniques for facelift surgery -- SMAS Facelift vs. Deep Plane Facelift.

    I am seeing results from a lot of women who’ve had various facelifts recently and who’s facelift results are not lasting. Especially with Smas facelifts which seem to look great for the first 4 months and then the face sags again back to baseline. By month 6-8 they look just how they did before they had any surgery. I’ve seen pictures from about 10 ladies in the last 2 months alone who are now all needing revisions. I have also seen 3 ladies who’ve had deep plane facelifts whose jowls have returned after 8- 14 months. I suspect this is a very common occurrence but not something people are made aware of because no one really wants to advertise that their facelift results have failed after such a short time.
    It seems that most women need to hold up their results with fillers or fat grafting etc also.
    Why do we get told that facelifts last up to 10 years when they really only seem to last months or at best a couple of years?
    And why do we only ever see before and after photos where the after photos are on average 4 months post procedure? This is when people look their best.
    We need to be shown photos at 8 and 12 months plus down the line so we can see evidence of longevity.
    Please can you explain what factors decide that a facelift will last longer on someone? Is it due mostly to lack of good bone structure? In which case do you advise implants if necessary? I’d like to know why so many women are experiencing such temporary results? And all of them are different ages. From 38 - 55. Different weights and different nationalities. Some are skin in the face, others are slightly rounded in the face. Some with good bone structures. Others not so much.
    Also please can you explain how the smas layer is stitched up? With plication it’s folded over and stitched but what happens to the sutures if they dissolve? Does the smas layer come loose again and unfold under the skin causing sag? And if permanent sutures are used, do they last forever whilst stretching or pull down over time? I know some people are allergic to permanent sutures so can only use dissolvable sutures.
    During a smas lift where the skin is separated from the smas layer, does the skin and the smas ever bond or knit back together to being one layer again or do the two remain separated? If so then this would surely weaken the structures of the face?
    These are the types of things that are not being explained by surgeons and I would very much appreciate it if you would please explain these sorts of things to us?

      What’s your background that you’re seeing a lot of women with facelifts? Where are you finding these photos and how do you know the timeline of these women’s procedures? I honestly never heard such claims other than reading your post.

    Wow, you are simply amazing. So intelligent. Thank you for sharing this with us! I’ve been trying to figure out what type of facelift is right for me. I am only 30 but have deep nasolabial folds and under eye darkness. I’ve been getting filler for years now and it has gotten to the point of looking unnatural and puffy. I am wanting my cheeks lifted, nasolabial folds gone, and under eyes rejuvenated. I live near Seattle so I will be going to Seattle for surgery. I’ve been doing lots of research, trying to find the right surgeon for myself but that is extremely difficult. I don’t want to invest in myself and then end up having to do another surgery in a couple of years.
    Do you have any advice? Thanks so much for your video, it was extremely informative!!!

    How are those released ligaments reattached to the bones? Do they find their way back and heal naturally?

      Yes, your body creates new attachments from fibrous adhesions that develop after surgery

    I just had a high SMAS nine months ago and its true I am actually getting fast progression of marionette lines. It did nothing for my inner face. Can mid face lift also address the marionette area? I heard there is detaching of ligaments involved in that.

    I’m only 37 my middle face and jowls are sagging pretty bad.. I’m thinking of having SMAS lifted.. because I’m worried after 5-10 years it will be saggy again…. So is it possible to do deep plane lift after doing a SMAS lift?? Thank you

    How long is the recovery from these surgeries? Also, wich one tends to generate less bruising?
    And can you explain the post surgery indications (aftercare)?

    Will the nerve injury be avoided better in a smas or a deep plane lift? I have minimal sagging cause of jaw bone shaving and I'm only 23, so i want to avoid the harm for he nerves at all costs, will the smas be better for me??

    Doctor, would you recommend facial fat grafting prior to a deep plane face lift for a better result?

      Hi Sophia, facial fat grafting can be performed at the same time as a deep plane facelift. It can also be performed before or after. The advantage of doing it at the same time is one recovery. Depending on the areas addressed for fat grafting, it may or may not be better to do fat grafting prior to a deep plane facelift.

    Question.. during a facelift, using a laser for deep wrinkles between the eyes. But it burned a hole a little larger than a jelly bean. How does that even happen? This happened to my friend and she has no clue how it could have even happened. Dr said equipment failure but we don't believe it. She has a huge hole between her eyes now

      Yes, it can address some of the hollowing under the eyes if there has been some sagging in the midface (or upper cheeks).

      Will a Deep Plane facelift work much better (lasts longer) than SMAS or other conventional facelifts?

      @@PhillipLearnTeach The length of time a deep plane facelift or SMAS facelift surgery lasts depends on your rate of aging and may not necessarily depend on the technique itself. However, the strength of a deep plane facelift is in its more natural elevation of all the tissues that descend with the aging process and less dependence on the quality of the skin.

      @@orchidj4618 yes it was a tear ear drum from blood dripping in or a nerve that healed. My ent said it was one or other person that did my hearing test said torn ear drum
