Think about how you would feel if your were hungry and the only food you ate were human brains. That 1. Definitely changes how you relate to all around you as they aren't just other people, they are your sole food source and 2. I imagine dreaming of brains like this as a mindflayer is the equivalent of a human daydreaming about a piece of cake, buffalo wings, a burger, or some other food.
It would definitely change your outlook on people, for sure. Omeluum and the Emperor show that it IS possible for illithids to maintain enough regard for other humanoids to treat them fairly and even with respect, but for most illithids, it is something they never even consider, the same way most people do not give a second thought to the animals they consume for meat. What's more, most illithids are far, FAR more intelligent than regular humanoids, and to them the thought of extending equal rights and status to "mere fodder" would be like us wanting cows, goats, pigs and chickens to be able to vote. Your average 18 Int illithid will be able to ponder concepts and conceive of schemes in 5 different languages (all at the same time) that your average 10 Int peasant could scarcely imagine. And more than that, yes, the need for illithids to consume live brains is a deeply fundamental need. They can, and do, sustain their physical bodies through "regular" (as regular as you might imagine mind flayer food to be like) food on a daily basis, but they also extract psychic nourishment from brains, and this can only be gained through their devouring of intelligent, sapient brains. At a bare minimum, an illithid needs to consume one brain every 2 weeks, and this is the ABSOLUTE minimum to keep from starving. Given the opportunity, most illithids will prefer to feast far more often than that, although even in the largest illithid colonies, most mind flayers will only wind up eating a brain once every week or two, given the difficulty in procuring enough mature brains. Assuming that Illithid Tav decides to remain "Good", they could probably sustain themselves in a manner similar to Astarion, by going out "hunting" every week or so and restricting themselves to Evil humans/humanoids. Goblin brains will suffice, for example, although it would be like sustaining yourself on bland gruel every day in terms of how appetizing it is.
