Just a point of clarification. In 2020 the FDA added persistent sexual symptoms and depressed mood to the list of dutasteride sides in the package insert. The 2011 package insert did not include those items. They did this because they had already added them to the finasteride package insert and so they added the warning to dutasteride too since it is the same class of drug. In both cases the warnings were added without any scientific evidence, which the FDA admitted as mentioned in the video. But even despite this, we rarely hear anyone talking about dutasteride sides, and we still never hear anything about post-dutasteride syndrome!
Have you ever considered Dutasteride is not widely prescribed? Despite that, there are plenty of anecdotes claiming the same symptomatology. There is no need to differentiate with a different acronym as it’s the same exact syndrome from a stronger 5ARI.

It is less prescribed than finasteride, but still there are over a million prescriptions per year in the US, and in other countries like Good Korea where it is approved for treating hair loss, it is very commonly prescribed. So this can't explain the huge differences in reports of post-finasteride syndrome from finasteride, saw palmetto, lions mane mushroom versus from dutasteride, even if you want to use the term "post-finasteride syndrome" for all those situations.

I've suffered from depression since I was a child, like 7 years old. It got a lot worse in my 20s. In April 2024 I hopped on Finasteride (thanks to you and so glad I did) and for the first week or so, I felt a DEFINITIVELY increased depressive mood... I knew it was a side effect, but in my mind I thought "well I'm used to this anyways, how much worse could it be?". It was indeed worse but nothing I couldn't push through, and after a week it totally went away. No sexual side effects. The statistic is most of our drugs, even generics, cost about 3 times more than the rest of the world. $35 for 90 days in this country for such an effective drug is a crazy good price but that's with insurance. Sup choom, just been to a party where I've been with my fellow classmates and friends frome home. I advised a friend recently to take fin and another friend was there, which I also advised to take fin (and is already taking for a year). The friend who didn't take it yet asked the second what he thinks about it and he said it changed his life. A female friend stood nearby and asked what are we talking about and we explained and she said that she didn't believe any of us were balding. Hey dude, can you elaborate a bit? I've had good results on Fin, but then started getting limp dick and had barely any morning wood after 2 years of being on it. Was wondering if topical Dustasteride would be a better option.
i have heard a lot of post fin syndrome videos and a lot of them actually tell that they were and still are on antidepressants . i personally have bipolar disorder which causes hypersexuality. meaning i think about sex 24 hours a day, 7 days a week lol. but when i take fin and my bipolar meds i have no issue at all, i can focus on my studies, i dont have to focus on my hair and i know i will not go bald. when i was not using fin or dut i would spend the majority of my day looking at a mirror to see how is my hair. now i am much happier even with one of the worst mental illnesses.

I think it’s because most people start with finasteride initially and if they get pfs then they won’t ever try dutasteride and the only people that take dutasteride are people that tolerate finasteride
That may be true in the hair loss world, but with BPH many people start with dutasteride because it is more effective than finasteride in shrinking the prostate. The meta-analysis of BPH studies showed no difference in side effects, and the large French study on suicide that I quoted in the video showed no increased risk of suicide. So there really isn't any evidence anywhere that dutasteride is less well tolerated than finasteride.
I am on dutasteride 0.15 mg for 10 months now. Never had any side effects, while the drug was still being very effective for me. I want to thank you for informing me about this stuff, because i wouldnt have my curls back if it wasnt for your videos about this topic :)

Kevin you helped me get on Finasteride back in 2021, it’s 100% stopped my recession from getting worse. I was on minoxidil about 2 months after starting fin, fast forward to 2024 I’ve stoppped using minoxidil for about 3 months now. I never really noticed any gains and was wondering if that’s a bad idea ?
I think finasteride was probably doing most of the heavy lifting. Minoxidil probably didn't give you much benefit beyond just finasteride so stopping it probably didn't matter that much. It sounds like you can get by just fine with 5AR inhibitors. Not everyone needs to use a growth stimulant.
I’d love to up my dose to 2.5mg of dutasteride but i just can’t see myself swallowing 5 capsules every day and more importantly spending much more so until dutasteride becomes available in 2.5mg form in the UK I’ll have to stick to being on 0.5mg.
No exaggeration to say Kevin saved my sex life. 1 year ago the hair was totally on the way out (albeit, I did catch it early, but it was clear when the general trajectory was going) and I'm a small framed guy so definitely wasn't pulling off the bald and jacked look. But wait, if dutasterid is stronger yet, more people complain about finasterid. This tells me that most of these side effects are actually before finasterid or dutasterid usage.
Short answer: because this is not prescribed as a starter as often as finasteride. The ones having the bad sides that they are linking to their finasteride experience, and being loud with PFS associations, didn't try dutasteride because it's less known/affordable. So we have less fear mongering dutasteride past users. Proof that serious users having bad sides are not the ones behind PFS circus, in my opinion
For hair loss, most people do start with fin and move to dut if they aren't getting an adequate response, so there is a little bit of a filter there. However I think for BPH many patients start with dutasteride, which explains why there were so many more dutasteride subjects in the French study I quoted. So I agree there is some selection bias going on, but I don't think any of this plays a major role. If dutasteride caused a post-drug syndrome there'd be a lot of cases of it, which there aren't.

I came across a channel called 'Moral Medicine' that makes videos about PFS with many supportive comments, some even claiming they took it for a few years until PFS developed. Are you aware of this channel and have you responded to any of their videos?
Check out the thumbnail of this video, obviously I have heard of them. None of their videos are supported by any scientific evidence, just a bunch of people making wild, unsupported claims about finasteride and other drugs, so there isn't really much to say. A claim that is presented without evidence can be dismissed without evidence.

Cheaper? In norway is costs $80 for 3 month supply dutasteride compared to only $40 for a year supply of finasterdie. Not sure how what its like in other countries though
I've been taking 0.5mg dutasteride / week and a topical minox + fin solution the other days for about 30 weeks now. Pretty happy with the results so far. Seriously getting hair greed from watching this, but will stick to my routine.
That one dutasteride study would have shown that 0.5mg gets the same results as 2.5mg if the study had gone on longer. Honestly 0.5mg dutasteride longterm just takes longer to get the same results as 2.5mg so there's no real reason to even take 2.5 dose sadly.
I got less side effects on Dutasteride than Finasteride and know many others who have. Although I switched back to Finasteride because Dutasteride made my testosterone levels over 900ngdl and I didn't want them that high. Suffering from success.
Unfortunately Dutasteride completely reduced my ejaculation to almost nothing and completely transparent and completely disappeared my libido, so I am stopping to see if it all turns to more normal just being on finasteride alone. I was taking Dutasteride twice a week in addition to finasteride 5 days a week. Back when I was taking just finasteride I didn’t have it that severe so it must be the Dutasteride.
It is absolutely 100% confirmed dutasteride stops acne. I was on finasteride for many years and it wasn’t until I switched to dutasteride that it stopped within days. Sometimes when I went back to the finasteride when I ran out of dutasteride, the oily skin and acne came back. It went away once I went back to dutasteride. Note I also I take TRT 200 to 250mg per week.
I definitely noticed an increase in oily skin when hopping on fin, and a major drop off about 2 weeks after incorporating dut. Also DHT itch seems to have eased up significantly as well.

Hi Kevin, love your videos! Got a question about topical finasteride: will it work applied only on the temples? So far I have zero thinning or hair loss on the crown, so seems like a waste/hassle to apply it there. My plan is to simply monitor the crown every 6 months or so, and start applying there if I see any thinning. Does that seem reasonable to you?
Yeah, for sure. It will work locally where it is applied. There may be some systemic absorption but that will give you other follicles some additional protection, so not necessarily a bad thing.
The reason I "only" take 0.5 mg of Dutasteride instead of 2.5 mg a day is that it's quite expensive in Western Europe to chug down 5 capsules a day, even the generic ones. For me, it would amount to almost 800 Euro ($850) a year. If someone knows a cheap and safe way, please let me know.
Admittedly I didn't watch every story on the Moral Medicine channel :) So my challenge was met. However, my point remains that reports of "PDS" seem to be awfully rare in comparison with PFS, especially considering how much more potent dutasteride is than finasteride.

Sup kevin, always having a prime time watching your videos, i want to ask you an opinion I have stopped minoxidil for about 2 months now and had started adding 0.5 mg of dutasteride once a week while the other days i was doing standard 1 mg fin I did it for 6 weeks but now have been diagnosed with the beginning of a slight gyno (I had been feeling tingling sensations and enhanced sensitivity in my nipple since the 3rd dose of dut but i brushed it off thinking i was imagining it, guess i shouldn’t have) Now, my pre-gyno ain’t that bad but i certainly don’t want it to get worse, at the same time i don’t want to lose my hair I had been using fin for 1 year before adding dut and it never gave me any kind of side effects so now my question is, could i get back to only finasteride without fearing the progression of my gyno? Hi Kevin, a point of clarification that I think may be quite important: on one study, you inferred that people would grow more hair on dutasteride than finasteride, as hair counts were higher in the dutasteride group. Isn't it in fact the case, strictly speaking, that you have a higher PROBABILITY of increasing your hair count with dutasteride than finasteride? If I've interpreted your other videos correctly, it seems that reaching that DHT suppression "cutoff" will cause the favourable hair follicle cycle change we want. Your conclusion seems to imply that more DHT suppression intra-personally (via dut) will increase hair count even after this cutoff has been overcome (via fin). The point I'm trying to make is that higher average hair counts on dutasteride could come about from the DHT suppression pushing more people over the cutoff (so the treatment group is made up of some non-responders and some responders, but crucially with more responders than in the finasteride group), or by each person getting more of a bounce in hair count than they would on finasteride, or a combination of both. The only way we could break this down would be to treat each individual with finasteride, and dutasteride (and preferably with a placebo too), but aside from some sort of cloning, this seems unlikely. Does this all make sense? It's been a while since I studied any statistical inference/study design so I'm likely rusty. Love your videos
Statistically all you can say is that the average hair counts increased with increasing percentage of DHT suppression. However the Korean study that looked at finasteride non-responders showed they had a better response to dutasteride. So that kind of was like running the different experiments in the same people. Whether you consider that the drug pushed more and more over some hypothetical threshold or whether it's just the more DHT suppression the more growth, it's kind of a moot point. What's important is, they had better hair growth on dutasteride.

Neuropathy is a common cause of ED in young men and doesn't implicate finasteride as a cause, see www.ncbi.nlm.nih.gov/pmc/articles/PMC10521948/ And the fact that finasteride helps prevent prostate cancer, and has been shown in in vitro tissue cultures in a high dose to inhibit glioblastoma cancer cells from growing is something that most people would consider a benefit. Especially since glioblastoma is a terrible form of brain cancer that in fact killed John McCain and Joe Biden's son Beau.

I'm currently in the process of this now too. Already noticing less oily skin and DHT itch after about 2 weeks. How'd you taper off? I took dut twice for the first week, 3 times in the second week (w/ fin daily). Planning to eventually take dut 4-5 times per week, or even daily; haven't decided yet.

I believe Dr. Kyle Gillett's theory is that there is more even inhibition of dht with dutasteride, and thus less side effects.

Been on it for 4.5 years every month. No sides, I get anxiety but I'm a heavy drinker and if I quit booze for a few days it goes away.
I've suffered from depression since I was a child, like 7 years old. It got a lot worse in my 20s. In April 2024 I hopped on Finasteride (thanks to you and so glad I did) and for the first week or so, I felt a DEFINITIVELY increased depressive mood... I knew it was a side effect, but in my mind I thought "well I'm used to this anyways, how much worse could it be?". It was indeed worse but nothing I couldn't push through, and after a week it totally went away. No sexual side effects. A little bit of pain in the testicles in the first week or so but that went away too.

I'm on dutasteride, when i was on finasteride i was still losing ground because my hair loss was more on the aggresive side, dutasteride and minoxidil together however i am now seeing consistent improvements every 3 months when i go to my clinic. Thankfully my dermatologist is based so he pretty much threw me on dutasteride once he could see my situation

Sup choom, just been to a party where I've been with my fellow classmates and friends frome home. I advised a friend recently to take fin and another friend was there, which I also advised to take fin (and is already taking for a year). The friend who didn't take it yet asked the second what he thinks about it and he said it changed his life. A female friend stood nearby and asked what are we talking about and we explained and she said that she didn't believe any of us were balding. Felt good hearing that choom 💪 been on fin 1.5 years and though it maintained my hair I'm gonna hop on Dut (or at least try to get a perscription) and maybe regrow some. i have heard a lot of post fin syndrome videos and a lot of them actually tell that they were and still are on antidepressants . i personally have bipolar disorder which causes hypersexuality. meaning i think about sex 24 hours a day, 7 days a week lol. but when i take fin and my bipolar meds i have no issue at all, i can focus on my studies, i dont have to focus on my hair and i know i will not go bald. when i was not using fin or dut i would spend the majority of my day looking at a mirror to see how is my hair. now i am much happier even with one of the worst mental illnesses.

I think it’s because most people start with finasteride initially and if they get pfs then they won’t ever try dutasteride and the only people that take dutasteride are people that tolerate finasteride
That may be true in the hair loss world, but with BPH many people start with dutasteride because it is more effective than finasteride in shrinking the prostate. The meta-analysis of BPH studies showed no difference in side effects, and the large French study on suicide that I quoted in the video showed no increased risk of suicide. So there really isn't any evidence anywhere that dutasteride is less well tolerated than finasteride.

I started Finasterid over one month ago. Till now I don't have a single sideeffect, besides beeing happy about my decision and increased libido...
Some relatives even said to me, that my hair somehow appears a lot darker and thicker (They don't know I take Fin).
My plan is to go with Fin for about 2 years and then maybe implement Dut once a week or so.

Kevin you helped me get on Finasteride back in 2021, it’s 100% stopped my recession from getting worse. I was on minoxidil about 2 months after starting fin, fast forward to 2024 I’ve stoppped using minoxidil for about 3 months now. I never really noticed any gains and was wondering if that’s a bad idea ?
I think finasteride was probably doing most of the heavy lifting. Minoxidil probably didn't give you much benefit beyond just finasteride so stopping it probably didn't matter that much.
It sounds like you can get by just fine with 5AR inhibitors. Not everyone needs to use a growth stimulant.
I’d love to up my dose to 2.5mg of dutasteride but i just can’t see myself swallowing 5 capsules every day and more importantly spending much more so until dutasteride becomes available in 2.5mg form in the UK I’ll have to stick to being on 0.5mg.
No exaggeration to say Kevin saved my sex life. 1 year ago the hair was totally on the way out (albeit, I did catch it early, but it was clear when the general trajectory was going) and I'm a small framed guy so definitely wasn't pulling off the bald and jacked look.
Now - hair is fine, even managed to do some small-ish cycles of gear so getting a decent-ish body too after so many years of spinning my wheels as a natty.
But wait, if dutasterid is stronger yet, more people complain about finasterid. This tells me that most of these side effects are actually before finasterid or dutasterid usage.
I think you're saying the sides are a nocebo effect, and I agree.
Short answer: because this is not prescribed as a starter as often as finasteride. The ones having the bad sides that they are linking to their finasteride experience, and being loud with PFS associations, didn't try dutasteride because it's less known/affordable. So we have less fear mongering dutasteride past users. Proof that serious users having bad sides are not the ones behind PFS circus, in my opinion
For hair loss, most people do start with fin and move to dut if they aren't getting an adequate response, so there is a little bit of a filter there. However I think for BPH many patients start with dutasteride, which explains why there were so many more dutasteride subjects in the French study I quoted. So I agree there is some selection bias going on, but I don't think any of this plays a major role. If dutasteride caused a post-drug syndrome there'd be a lot of cases of it, which there aren't.

Are you aware of this channel and have you responded to any of their videos?

None of their videos are supported by any scientific evidence, just a bunch of people making wild, unsupported claims about finasteride and other drugs, so there isn't really much to say. A claim that is presented without evidence can be dismissed without evidence.

I've been taking 0.5mg dutasteride / week and a topical minox + fin solution the other days for about 30 weeks now. Pretty happy with the results so far. Seriously getting hair greed from watching this, but will stick to my routine.
I should post a warning on this type of video to watch out for hair greed.
That one dutasteride study would have shown that 0.5mg gets the same results as 2.5mg if the study had gone on longer. Honestly 0.5mg dutasteride longterm just takes longer to get the same results as 2.5mg so there's no real reason to even take 2.5 dose sadly.

Unfortunately Dutasteride completely reduced my ejaculation to almost nothing and completely transparent and completely disappeared my libido, so I am stopping to see if it all turns to more normal just being on finasteride alone. I was taking Dutasteride twice a week in addition to finasteride 5 days a week. Back when I was taking just finasteride I didn’t have it that severe so it must be the Dutasteride.

It is absolutely 100% confirmed dutasteride stops acne. I was on finasteride for many years and it wasn’t until I switched to dutasteride that it stopped within days. Sometimes when I went back to the finasteride when I ran out of dutasteride, the oily skin and acne came back. It went away once I went back to dutasteride. Note I also I take TRT 200 to 250mg per week.
It can. Doesn't work for everyone, but it can make a big difference for some people.
I definitely noticed an increase in oily skin when hopping on fin, and a major drop off about 2 weeks after incorporating dut. Also DHT itch seems to have eased up significantly as well.

Hi Kevin, love your videos! Got a question about topical finasteride: will it work applied only on the temples? So far I have zero thinning or hair loss on the crown, so seems like a waste/hassle to apply it there. My plan is to simply monitor the crown every 6 months or so, and start applying there if I see any thinning. Does that seem reasonable to you?
Yeah, for sure. It will work locally where it is applied. There may be some systemic absorption but that will give you other follicles some additional protection, so not necessarily a bad thing.

The reason I "only" take 0.5 mg of Dutasteride instead of 2.5 mg a day is that it's quite expensive in Western Europe to chug down 5 capsules a day, even the generic ones. For me, it would amount to almost 800 Euro ($850) a year. If someone knows a cheap and safe way, please let me know.

Admittedly I didn't watch every story on the Moral Medicine channel :) So my challenge was met. However, my point remains that reports of "PDS" seem to be awfully rare in comparison with PFS, especially considering how much more potent dutasteride is than finasteride.
My dermatologist switched me from Finasteride to dutasteride thinking it would be more effective for treating my crown. Definitely noted an uptick in sexual drive and function.

I have stopped minoxidil for about 2 months now and had started adding 0.5 mg of dutasteride once a week while the other days i was doing standard 1 mg fin
I did it for 6 weeks but now have been diagnosed with the beginning of a slight gyno
(I had been feeling tingling sensations and enhanced sensitivity in my nipple since the 3rd dose of dut but i brushed it off thinking i was imagining it, guess i shouldn’t have)
Now, my pre-gyno ain’t that bad but i certainly don’t want it to get worse, at the same time i don’t want to lose my hair
I had been using fin for 1 year before adding dut and it never gave me any kind of side effects so now my question is, could i get back to only finasteride without fearing the progression of my gyno?

Hi Kevin, a point of clarification that I think may be quite important: on one study, you inferred that people would grow more hair on dutasteride than finasteride, as hair counts were higher in the dutasteride group. Isn't it in fact the case, strictly speaking, that you have a higher PROBABILITY of increasing your hair count with dutasteride than finasteride? If I've interpreted your other videos correctly, it seems that reaching that DHT suppression "cutoff" will cause the favourable hair follicle cycle change we want. Your conclusion seems to imply that more DHT suppression intra-personally (via dut) will increase hair count even after this cutoff has been overcome (via fin).
The point I'm trying to make is that higher average hair counts on dutasteride could come about from the DHT suppression pushing more people over the cutoff (so the treatment group is made up of some non-responders and some responders, but crucially with more responders than in the finasteride group), or by each person getting more of a bounce in hair count than they would on finasteride, or a combination of both. The only way we could break this down would be to treat each individual with finasteride, and dutasteride (and preferably with a placebo too), but aside from some sort of cloning, this seems unlikely.
Does this all make sense? It's been a while since I studied any statistical inference/study design so I'm likely rusty. Love your videos
Statistically all you can say is that the average hair counts increased with increasing percentage of DHT suppression.
However the Korean study that looked at finasteride non-responders showed they had a better response to dutasteride. So that kind of was like running the different experiments in the same people. Whether you consider that the drug pushed more and more over some hypothetical threshold or whether it's just the more DHT suppression the more growth, it's kind of a moot point. What's important is, they had better hair growth on dutasteride.

0.5 MG daily is your best option. I wouldn't do it 3x/week unless you get side effects.
I challenge this guy to explain why PSSD and PFS sufferers have small fiber neuropathy and to explain how SSRIs, Accutane and finasteride stopping cell proliferation isn't concerning. SSRIs Accutane and finasteride all show efficacy in the treatment of cancer.
Neuropathy is a common cause of ED in young men and doesn't implicate finasteride as a cause, see www.ncbi.nlm.nih.gov/pmc/articles/PMC10521948/
And the fact that finasteride helps prevent prostate cancer, and has been shown in in vitro tissue cultures in a high dose to inhibit glioblastoma cancer cells from growing is something that most people would consider a benefit. Especially since glioblastoma is a terrible form of brain cancer that in fact killed John McCain and Joe Biden's son Beau.

I'm currently in the process of this now too. Already noticing less oily skin and DHT itch after about 2 weeks. How'd you taper off? I took dut twice for the first week, 3 times in the second week (w/ fin daily). Planning to eventually take dut 4-5 times per week, or even daily; haven't decided yet.
