    I was listening to a NPR RADIO show with Colin broadcast from the Hartford CT studio and heard Briana from TWIV was on the phone with this talk show host. Colin made a nice recommendation to the audience that TWIV is a worthwhile show to listen to. Nice to hear.

      I like everything they do including that the music, which isn't charming, makes me jump. It's just so wonderful getting to know these boffins and getting the good stuff, even when I cannot understand it (and I have a medical degree) that somehow I like it that they are not slick in all departments.

      The outro has been consistently way too loud in these clinical updates - I hope Vincent notices this message. I can't justify bothering him over the email for this reason, yet I consider it every week after watching one of these :)

      Yeah...volume normalization is often overlooked in production. It would be useful to extend that to the intro and outro. It is common to set a standard volume level for all videos, and the most attentive pay attention to the volumes on similar videos.

    I would suggest bringing up the microphone levels by 40%. If you have VU meters, shoot for the "0dB" mark (or just under the red lines).

      Why? The +1000000dB for the outro music makes a good 0dB average 😵‍💫
      The intro doesn't bother me, I'm always right next to the equipment and turn it down temporarily. The sudden outro though. Holy hell that's loud!

      Boost the gain Daniel's lav and try different placement of the element. Yes, there's a bunch of bogies to dodge (bow tie, shifting coat & lapels) but move it higher up (as well as gain up that channel)

    I think Kari Stefansson's Icelandic data are even better than that lab data. They combined repeat qPCR, genome sequencing and contact tracing data (plus 6 antibody assays), this repeated multiple times for each individual until 60 days after symptoms gone. So they were able to identify all imported SARS-CoV-2 lineages as a baseline (ie from source countries), plus identified nearly 700 mutations that occurred in Iceland community, plus tracked these mutation transmission routes. Because of these mutations, contact tracing, transmission routes & timelines, they were able to estimate how long each person was infectious (better than identifying how long viral samples are viable).

    The theme song of this show is really loud and obnoxious. Is there any way to bring the volume down just a bit. I cannot imagine that it is that ear-splittingly loud on purpose, or that I am the only one who has commented on it. Yes, it is attention grabbing, but not in a good way. Otherwise, it is a great show and I have no other complaints. Thank you for your service to the public.

    I have been tested twice a week for work for the last year and a half and after aaaaall this time and aaaall the Covid that I have been exposed to, my Wednesday PCR came back positive. I have zero symptoms so far. I saw patients all last week not knowing that I had the presence of some Covid in my nose. They say I can go back to work Wednesday so fingers crossed. I am so grateful for the vaccine. Thanks for another update fellas.

    “PCR tests are overly sensitive”……duh. This has been known for well over a year. It’s nearly criminal to report a PCR test without the CT data.

      @@jmer9126 Why are you traveling during a pandemic? We traveled all the time before COVID-19 and in Business or First only, w/benefits of special short TSA lane & Lounge privileges so it doesn't get any better (Frequent Flyer miles). My wife worked for 2 international carriers until the mid-2000s. We have flown since late 2019 & won't for the foreseeable future as we don't want to become ill & suffer effects that would make traveling difficult (or die). A big change in lifestyle for now but not the time to roll the dice. Aircraft air circulation is misrepresented as to safety as studies were done under unrealistic conditions & do not include factors such as collective body heat (which rises, interfering with downward motion of cabin air circulation mechanism), interruption of air flow by FAs & pax walking through aisles, tray tables put down, beverage & meal service carts, seat recline, pax putting & retrieving luggage from overhead bins, poor mask wearing by majority of people & discounts that you must transit at least 2 airports, including retrieval of luggage at baggage carousel with exertion. If you're in coach you're inches from others. Even if middle seat is empty-they are only 18 inches wide. Yet many cases of Delta arrived in Business class. The restrooms in airports and on aircraft a nightmare. It doesn't matter that aircraft toilet uses vacuum system. People spend plenty of time in the them so virions accumulate. With testing poor, asymptomatic cases may not be picked up & one test represents only one point in time.

    Family friends of ours, 3 adult children in 20s and mom and dad, all 2x vaxxed. Entire family got Covid Dec 20. Just recovering now. Hearing lots of these stories.

    Guys, I just recommended your clinical updates to my hairdresser. She’s fully vaxxed, but has been reluctant to get her school-age sons vaccinated.
    I hope I convinced her, or at least got her to call her son’s’ pediatrician. At least she’s 100% on board with masks & social distancing.

    This episode is a game changer. I think this provides incontrovertible, evidence-based numbers that people can use as their entree into the world of the vaccinated. These are numbers that we simply didn't have before, and I think they can help set the course. Unvaccinated, young people... you are our future. Please pay attention to this podcast, and steer your ship away from the iceberg. By the same token, stop inviting other ships to join you on your course. SOS.

    Appreciate the update. One correction: Paxlovid is contraindicated with losartan and simvastatin, but not atorvastatin, rosuvastatin, or other statins.

      It's recommended to decrease dosage or stop/switch if possible while taking Paxlovid per the clinicians' version.

    Doesn't make sense for you to be together. I know you are each triple vaccinated but a remote approach would be much safer, especially since Daniel has had direct exposure to Covid positive patients with high viral loads.

    What about blood transfusions between the vaccinated and unvaccinated. I am thinking of tainted blood as HIV was transmitted. Could those with the vaccine transmit the spike proteins which cause clots and myocarditis?

    In Switzerland every State (Kanton) has a different procedure testing K-12 and upper classes and the procedure depends on duration of days pupils are attending school.
    An example for one Swiss State in which testing at schools and wearing masks is mandatory for pupils, teachers and other staff:
    - pupils attending school for 5 days test once a week (saliva pooling test, one class= one pool), pool test result arrives evening same day at school, class teacher gets informed of the result by head of school and has to inform class of result, if the pool result is pos, every pupil has to test till the evening of the next day (electronically book an appointment for a depooling test), pupils attend school while waiting for personal test result (if the school principal or head medical adviser didn't mandate distant learning for the pool), if personal test result is positive, pupil has to isolate and close contact persons have to quarantine (5 days beginning day of showing symptoms resp. pos. result, ends, if day 4 and 5 no symptoms for people in isolation, 5 days for people in quarantine, beginning from last day of contact with positive tested, no quarantine for recovered and vaccinated

    This may surprise you guys, but most lay people have NO IDEA what you're talking about when you declare that, "rna copy number is NOT viral load!"
    Some of the details of these studies just go way over my head, and that's frustrating. (I am a college graduate.)
    It might be helpful to remember that most of your audience on UA-cam (for the clinical updates, in particular) is outside of your scientific world.
    I LOVE 💗 these clinical updates so much, and rely on them to counter the gross misinformation of main stream media on COVID-19.
    Thank you for your time and dedication to all of us. I know you spend countless hours on these presentations. You are greatly appreciated!!!

      Analogy: You have an active tiger. He is dangerous and can affect your health. Take a second tiger, put him in a meat grinder. It is an inactivated tiger. It is not dangerous. Sample each tiger. Both show up positive on a PCR test because both have Tiger genetic material.

    Clinical medicine has always been at the edge (fringe ?) of hard science and research, throughout human history. Opinions and reccomendations about Covid 19 are no different. I love this show but admittedly, I am not a member of the fan club. I have a healthy respect as well as scepticism of research Daniel is trying to translate in to clinical advice.

      Sometimes, obviously. But often it is locked in dogmatic 'remedies' - leeches and such, and is the most retrograde and hyper-conservative aspect of science. OK so I am not trying to pick an argument, but I am curious what your differences are with TWIV. I have my own critique - Vince is overly dismissive of concerns about G-o-F and does not explain his sanguine expectation that such research won't derail, and some of the moderators here are overly aggressive in suppressing criticism. But overall this is the most reliable and authoritative high-frequency vlog on covid, all the more so now that John Campbell is being called out on his theory on innoculation procedure. Any thoughts my friend?

      @@jdeverhard That's a lot to comment on. I find TWIV and John Campbell well reasoned at all times. However, Daniel trying to translate some preliminary research findings to useful clinical pearls tend to go a bit too far. It is not really his fault. He is trying to make things useful for clinicians.
      I have no opinion over GOF research because it is not something I have ever seen done in normal research labs.

      @@udayaliyanage3776 Daniel Griffin is also a research scientist with both an MD & a PhD and is a Professor of Microbiology at Columbia University School of Medicine. He is the head of Infectious Diseases for the NYC Metro areas largest hospital group. By June of 2020 he'd personally seen over 1700 COVID-19 patients.
      He is also a Parasitologist & on the Board of Parasites Without Borders. He's spent time treating patients in developing countries and is leaving soon for 2 weeks in Ghana to treat patients there. He's not some local GP who treats the typical problems of suburban Americans. Try getting up to speed about those on whom you're being critical. He'd run circles around you on any discussion of COVID-19.

      @@udayaliyanage3776 Shows your ignorance. GOF has been done for decades. It does not require some special lab as many thousands of biology-related experiments use GOF. Most have nothing to do with diseases-as an example, the flavor of many wines are enhanced by tweaking the characteristics of ingredients via GOF. It merely means changing an aspect of a biological entity & used regularly in many fields. How embarrassing for you that you don't know this simple concept that's been used for decades in non-medical as well as medical fields.

      @A Q welp ok … it’s been close to 2 years and I thank God it’s still extremely high. I tested last about 12/13/2021( about)..and again I thank God …maybe just like getting the Measles the antibodies will be for life … I pray🙏🏽🙏🏽🙏🏽🙏🏽.. Question: I know the synthetic antibodies looses it’s efficacy… are they he scientist saying that the natural ones are “ suppose” to loose it’s efficacy? Also, what do you think about being over vaccinated for Covid? I am hearing some scientists expressing concern based on their studies that it will damage the natural immune system.. what say you?

    My household and my mom's- all fully vaccinated, except for my youngest. Visited on New Year's Day. Took off mask only for picture taking. Five days later, mom's fully-vaccinated help gets a runny nose- positive. Two days later, my youngest (who never went anywhere else, ever) gets the flu and within two days infects my family including two boosted individuals. So no, I don't believe droplet exposure between fully vaccinated individuals is worth the risk if any of them live with anyone who haven't/ cannot be vaccinated.

      Yes, similar experience with some of my relatives. 4 households, all adults vaccinated. One vaccinated adult transmitted SARS-CoV-2 to all the others, including two toddlers. No one wearing masks, big family gathering.

      I definitely believe that being fully vaccinated significantly reduces risks of severe illness, but the simple truth is VE against symptomatic infection has significantly reduced from early Israeli results (which was Pfizer 90% VE against all infections, including asymptomatic).

    Since Delta was twice as risky as Alpha and Omicron is half as risky as Delta , so we are back to where we were with Alpha? This is like a grade school word problem.

    In Iceland they combine qPCR CT values plus antibody assays to estimate whether someone is infectious (border screening, where they don't have histories).

    Transmission and human behavior
    Many people have been very confused that Japan seems to have largely escaped the firestorm of Omicron. There is tremendous confusion about why there is no obvious reason for their near miraculous escape. But no one seems i interested in the fact that there is near universal voluntary masking in Japan…. And, presumably other population driven voluntary mitigation measures.

    A coworker of mine is 22 years old and healthy. She is not vaccinated. She has missed two weeks of work bc of covid. She has felt miserable. She may not end up in the hospital but she is very sick

    44:20 vaccinated shed less virus - Vincent Racanciello. Ph D cite this time stamp when arguing the point. If anyone has the reference page in 'today's paper' please put in comments. This is a key point in the debate with vaccine sceptics and needs citable sourcing.

      I live in small-town MN where there is a serious disconnect with reality; the majority of people feel like this virus is a joke or a hoax. I have 5 kids who are kindergarten - 12th grade. We've been distance learning since this began and as they've been fully vaccinated, they've returned to school (a no mitigation school, but nonetheless the closest place within 25 miles). Anyhow, my 3 youngest (K-6) are set to return to in person learning in a week. Should I keep my kindergartener home distance learning (which has been incredibly adventageous, at least from a learning standpoint)? My county has very low vaccination rates- far below the national rate. No masks. And the local school boards have chosen to bury their heads in the sand. Our rural clinics and hospitals are "packed".       @ty Ty Thank goodness no, my kids are within normal BMI range, it's hard to get them to eat their veggies but we keep up on supplements (which might be just a waste of $) and have 2 indoor trampolines that work good for exercise in the winter months. I was diagnosed with COPD while in my 30's (not long ago). I'm pretty sure growing up and working on a farm (hog) didn't help any. So I guess I'm higher risk... Could stand to drop a 25 pounds as well, but as far as I know, my kids have no comorbitities other than maybe not eating as much fruits & and vegetables as they ought to (my fault, I'll work on that, you make a great point).

      @@jillstruthers If your kid is doing okay remotely a little longer won’t be too bad and once your county is less stressed the school will still be there.

    Has anyone looked to see if Omicron persists longer on surfaces? Hand hygiene messaging seriously dropped off when the ventilation message came out, but we weren’t talking about drippy noses and sneezing then. Snot doesn’t evaporate as quickly as spit.

    When it comes to ambiguous terms, none are more so than "natural immunity." I suggested early on to use the term "acquired immunity." Regardless, people believe want they want to in this "black and white" country we live in. Wait, I'm not referring to black and white people, see there you have it, ambiguity rears its ugly head. Imprecision in language can be as harmful as imprecision in science, where science is self-correcting given sufficient time; language not so much.

      Can't you "acquire" immunity from a vaccine? Anyway's I think "natural" is to contrast with "artificial". One is from nature, the other is man-made.

      Why can't we just say "immunity from infection" and "immunity from vaccination"? That is unambiguous, as far as I can tell.

      They don't. But there's plenty of t-cell epitopes in the S protein.

      @@EvonZundel As Paul Offit quoted on recent children hospitalisation data, 1/3 of hospitalised Covid cases had no underlying conditions.
      Recently in Australia a 24 yro fit man, double dosed, with no significant underlying conditions, died from COVID.
      So, would he have died if given 3 doses? Possibly, but the 3rd dose might have been enough to keep him alive, and why die wondering.
