I had the Rezum procedure done over three years ago by Dr. Richard Bevan-Thomas at Arlington, Tx. It turn out to be one of the best decisions I ever made. I'm 80 years old, but I'm like I was when I was 25 years old. I never have to get up at night to go. I think the important part is to find a Doctor who is experienced at performing the procedure; And I chose to pay a little extra for Nitrous Oxide gas as an additional sedative, so that there was very little pain during the ten minute procedure. I considered the Urolift but I'm glad I chose the Rezum instead.

I just turned 70 and have endured BPH issues since my 50s. Decided it was time to take action. My urologist recommended Urolift. His description of the procedure had me running for the hills. Your video convinced me to go ahead and give it a try.
Your videos are informative, and the information is invaluable.
    Thank you.

I'm 70 and had Urolift about a year ago. I was able to stop taking Tamsulosin which was why I had it done. As far as urinating it's not like I'm 19 again. For me it's a little better now than what the pill did. I'm satisfied.

Great video Dr. Malik. Since I had my prostate surgery in February, I have discovered your channel and have developed a much greater appreciation for urology than I had before. I was having all the usual symptoms that you describe in the segment on storage lower urinary tract symptoms. That was me for sure. My partner in my Optometric practice, was about a year ahead of me with all the same symptoms and had the Rezum procedure. He had some minor complications but was overall very happy with the outcome so I decided to find out more. I had been seeing an older urologist for several years who no longer did surgery but he was very impressed with the Rezum procedure. I saw the doc he recommended assuming I was a candidate for the Rezum procedure. As it turned out, I was a better candidate for Aqua-Ablation as my prostate was scanned to be 95 whatever units you docs use for this measurement. My doc said the research shows that Rezum is not as effective as Aqua-Ablation after the 85 unit mark (is it cm squared?) so that's what I had. I have been very happy with the procedure and the outcome. Now I have been able to get off Finasteride and Silodosin, I am even happier as I am having better sex than before the procedure and it's still improving. I don't understand some of these comments about catheter pain. I had none although I had bladder spasms that meds controlled and totally quit shortly after the cath was removed, which I was instructed to do myself at home just before my first followup appointment with my doc. Easy Peasy. I am a big fan of the Aqua-Ablation and highly recommend it IF, and only IF, your doc does the procedure and recommends it. I am not a fan of patients coming in and telling me or their doc, what procedure they want. I encourage patients to bring up anything that have heard about it but the doc needs to make the final decision on which procedure is best for your symptoms. I have no clue if I will ever need it again but if I do, no biggie. I don't mind everyone and their dog seeing me naked, it's their business and they don't remember me any more than I remember all the retinas I see all day.

I had urolift 3 years ago after bladder cancer. Recovery from the urolift was easy for me. Sure a catheter is always unpleasant for a couple of days only. The urolift was a big success for me! After about a week of recovery I have been peeing like a young man again. Seems to be a permanent solution with no problems in 3 years.

A video just popped up on my "UA-cam" sidebar from one of my local news station's morning shows, from 2018, that showcased and interviewed two local urologists in this area who'd pioneered the Urolift method of treating an enlarged prostate in our area. The doctors had also followed-up with their patients for six years afterward and found that their patients, overall, were still in good shape. These two doctors have also been designated the "Urolift Center of Excellence" (That's how it was said on the video.).

It would be interesting to get your take on the new Procept "aquablation" procedure, which is advertised as an improvement on the traditional TURP. It uses a high pressure stream of water as a cutting instrument, and the debris is produced in small particles and therefore easily flushed out. The short video I saw showed what seemed like an excessive amount of tissue removal, but this may be an incorrect interpretation. You should do a piece on this procedure, as compared to Rezum and Urolift.

    • @drgibson410

That's what I had and can't say enough good things about it EXCEPT according to my doc and what I read which was probably what he read too, it works best on a larger prostate, above 85 units. Mine was 95 but I'm a big guy. Wish I could remember what the units of the numbers was. I am thinks it was square millimeters but may be cenimeters.

My unethical doctor never once thought he had done something wrong and wanted me to continue taking medication and even prescribed medicine for overactive bladder until I asked him to use the scope and find if the pathway is really open. After scoping he claimed there was tissue still hanging and blocking the pathway; he offered no remedy. If we had a responsible FDA They would reevaluate and resend the approval of Urolift.

I had a Urolift 3 years ago after my second confirmed UTI and ultrasonic imaging revealed my bladder was not emptying completely. My Urologist recommended it over Rezum treatment. At the time, he lacked confidence in Rezum thereapy, thinking there was too little studied history on it at the time.
After the procedure, I had a few days of slight discomfort, which I would not describe as pain. Urination was quite uncomfortable for a few days caused, I believe, by irritation within the urethra. I did have signs of blood in my urine for a few days, but that was expected. The doctor's instructions warned against sexual activiy for 7 days, but after 5 days, I was "Ready To Go."
I haven't had any indication of need for another procedure (yet), although the Urologist told me that was possible.

I still wake up 1-3 times per night, which is better than I was doing 1-5 times per night...so some difference overall, but nothing to jump up and down for and the post operative pain isn't the best either...I believe my bladder is not elastic enough so it's always filling up quickly and there's no cure for that as far as I know. For those with great bladders, the result will probably be better. They'll give you drugs for the bladder, but I fear the side effects, so no thanks...I'm just wondering if testosterone enhancement would be beneficial?? Since we lose testosterone as we age and it's linked to the hormone that makes the prostate enlarge...

Hi Rena, I'm trying to see a urologist for T supplementation based on the results of a blood test, but there is _only one_ urologist in my (200k+ population) city after another closed, they're totally overwhelmed and I literally cannot get an appointment (when I called there were 34 on hold before me, waited an hour, then they're so underwater they can't f find my referral which was sent multiple times...) I'm wondering how this is possible; why is this vital medical niche going unfilled here? There's a lot of business to be had. Is there a shortage of urologists today? And why can testosterone seemingly only be prescribed by a urologist? Grateful if you have any answers!

You should be able to see an hormone doctor also, an endocrinologist?

Hi all. I'm 43. Recently found out I have mild prostate enlargement (41.7ml) but it is giving me urinary issues etc. I am not sure I want to go on pills maybe except Cialis cause the others scare me. Has anyone gotten the rezum and was it worth it? I'd love to get the itind but why is it only for men 50 and over of age ?

I had Rezum done 2 years ago. I had complications with it after they took the catheter out. Almost died and was in the ICU for 3 days. The most painful experience in my life. But all seems well now.

@@rustynail3630 what was the complication if you don't mind? I am thinking of having rezum myself, trying to decide.

What would the length of a typical catherization be following each procedure?

I don't know what is typical but I had an aqua-ablation on Thursday morning and had the catheter until Monday morning with no pain. They have to do a procedure with the cath called Constant Bladder Irrigation (CBI) to keep the clots from stopping up the cath after the procedure. That's why they keep you around in the hospital for a day afterwards. They are pouring saline in, rinsing out the prostate and draining it back out, all through the cath. It's great fun!! On Sunday after the procedure, mine stopped up anyway and quit draining which required a trip to the ED and let them flush it all out and get on with it. I could have done it from home but it's not what I'm trained in. Any good nurse can do it.

I just had the Urolift procedure 2 days ago. Don't believe the hype from the "testimonials" and experts about how you may have some burning and discomfort for a few days but it's no big deal. Day 1 I felt I like I was peeing battery acid-EVERY. SINGLE. TIME , for 24 hours. I still break into a cold sweat just thinking about it. Day 2-serious deep pelvic pain from the tip of my johnson to my anus every time I peed (thank God the burning has subsided); with frequency about tripled from my pre-surgery frequency problem. All the experts promise that by day 4 things should be getting better. I hope so-I'm exhausted from this thing. Maybe the Rezum is a better option, even with extended catherization. I don't look forward to having to repeat this ever again.

HI DOC, I HAD A FAIRLY NEW BUT VERY EFFECTIVE PROCEDURE CALLED PROSTHETIC ARTERY EMBOLIZATION.. ITS NOT DONE BY A UROLOGIST..ITS DONE BY A INTERVENTIONAL RADIOLOGIST. THEY BLOCK THE BLOOD FLOW TO THE PROSTATE THROUGH ARTERY IN LEG. ITS SLOWLY DECREASES IN SIZE AS IT DOES ALL THE SYMPTOMS OF BPH GO AWAY!! NO SIDE EFFECTS NO PAIN . AMAZING

These 2 procedures aren't available in Canada only thing is a Turp leaving the man unable to have children. Our country lags so far behind in modern medicine people here have to go to the States to get the treatment they deserve sad huh. Free healthcare means a 24 hour wait time at a ER for me 10 hours in critical condition waiting to see a doctor, no medical treatment option available the Government offer MAID service instead and so far 26,000 Canadians have participated in its short tenure.
