Everything that you see in the movie is not happening inside Luke's head. Most of the series of events post the car accident are all real. Except for those ghost sightings. Luke was subjected to 'white room torture' in Dubai because he was initially the prime suspect for the death of his wife. That screwed up his state of mind. He returns to Kerala to seek revenge only to find out that Dileep was killed in a car accident. He finds it hard to accept that reality (just like how he hasn't come in terms with his wife's death), which is why he has these hallucinations. Deep within he knows that you can't kill a person twice. So then he shifts focus to destroying Dileep's family and every bit of reputation they had.

None of these events happen inside his head. He went through white room torture in Dubai where he resided with his wife before she was murdered by Dileep.so now he's trying to take revenge on Dileeps family. The only way he can find peace with himself.
In fact, the movie itself is a "Rorschach"! You can interpret it as you wish, like in a Rorschach test.

Basically luke was trying all this while to provoke dileep ...he triedvarious ways of provoking him...buying his home dear to him nd tried breaking it...nothing happened...brought a prostitue into his house nd she said dileep hated her...nothing happened...atlast he got hold of his weakness which was his wife...nd that triggered him...
There's a possibility that all of this was in his head too.. that theory is floating around as well..
The more you watch the movie, the more you feel that way..
