Sensitive skin is one of the most common presentations to a medical dermatologist. Recent studies show that up to 68% of the population reports skin sensitivity.
The topic of sensitive skin is extraordinarily complex as skin sensitivities fluctuate with seasons, flare factors, age as well as the underlying disease state.
Diagnosis of sensitive skin
Medical dermatologist will always seek the cause of sensitive skin. There are numerous inflammatory skin conditions that can give rise to skin sensitivities such as acne, rosacea, atopic dermatitis, seb derm, irritant contact, allergic contact, photo allergic reactions, psoriasis, and many, many other conditions. In some cases the cause is multifactorial. Finding the diagnosis can often take multiple visits to the dermatologist. Test such as patch testing, photopatch testing, RAST testing, and ROAT or repeat open application tests can help medical dermatologists arrive at a diagnosis. As a procedural dermatologist I work in a team to help medical derms. Physical treatments for skin sensitivity include lasers for rosacea, as well as many other procedures outlined below.
SKIN CARE TIPS
SUNSCREENS
Mineral sunscreen over chemical sunscreen eg. Avobenzone, cinnimates, oxybenzone. My choice? Invisible Zinc. In acute flare ups, avoid all creams for 2-4 days until barrier function is established.
MOISTURISER
Twice a day. Choose a moisturiser with no active ingredients. Dr Sam’s moisturiser is great. Also consider Obaji, both Hydrate and Luxe. HAs from The Ordinary can be a good humectant.
THINGS TO AVOID or CAUTION with
1. SLS, preservatives Parabens, formaldehyde, and fragrances.
2. Irritants such as Citrius, teatree oil, sandlewood, bergamot, Photosensitizers
3. 3. Avoid high strength AHAs and acidic formulations. This includes AHA such as glycolic lactic acid, best use salicylic acid in moderation. Use Vitamin C with caution.
4. Avoid physical exfoliation such as microbead products. Use baby wipes to remove make-up. “Simplify to amplify.”
5. Vitamin A, retinol and retinoids can be used with extreme caution. Dilute the product in a moisturiser base, use 2-3 nights per week and titrate accordingly.
PHYSICAL or PROCEDURAL METHODS for SENSITIVE SKIN
A medical dermatologist may refer you to a procedural dermatologist. Once a diagnosis has been established by my colleagues, procedures as outlined below can help control inflammation and provide safe, non-contact skin rejuvenation. I do not recommend chemical peels if you have sensitive skin, instead laser and light technology can help.
Lasers - dermal toning, laser genesis, pico lasers, Clear and Briliant, Fraxel, IPL or BBL
Dermal toning uses both pico and nanosecond pulseduration with longer wavelengths such as 1064nm and 755 nm Alex. As the laser penetrates into the deeper dermal layers the epidermis is largely spared. 5-10 sessions required for optimal results. With picosecond lasers similar results can be seen in less than half the time. The flipside is that the incidence of PIH is more common in more powerful short pulse duration lasers.
Laser genesis is particularly useful in this setting. Much like dermal toning, laser genesis provides a gradual increase in collagen over 3 months. As with dermal toning, the long wave 1064 nm is the wavelength of choice as it penetrates into the deeper dermal layers reducing inflammation as well as blood vessels. I use the newest laser from Candela called the Prima for laser genesis.
Fractional lasers such as Fraxel Dual 1927/1550 can be highly effective in providing dermal remodelling for any skin type, and any skin colour. Density of fractional laser beams, the fluence of power and number of passes can be controlled and a tailored solution given to patients with sensitive skin.
IPL or BBL stands for Intense pulse light or broad band light is a flexible physical modality- unlike lasers this treatment uses high fluence light with specific filters. IPL can treat many conditions, however using a specific filter IPL can reduce redness, inflammation and surface veins. Sensitive skin conditions such as acne, rosacea and flushing/blushing can be decreased after 2-4 sessions of IPL.
PDL or pulse dye laser can significantly improve rosacea. The 595nm wavelength is ideal for blood vessels with the target being haemoglobin. In most cases inflammatory skin conditions can be improved within 2-3 sessions, spaced 2-3 weeks apart. Pulse dye laser can also improve dermal collagen.

Can only some parts of my face be sensitive??
Cause whenever I apply any " kind of " strong active, only a quarter of my smile line on one side gets very irritated when applied and dry, red painful for the following week, while the rest of my face stays there same.
2. I'm Indian originally and currently not employed. So I stay indoors literally all the time, but next to a window. Do I still need to wear sunscreen everyday??
3. Can you do a video on dry vs dehydrated vs thin skin??
I don't know what it is but I have extremely one of the above long of skin from my knees down and nowhere else.
4. When you or generally people talk about hyperpigmentation, it is usually about scars from past acne. And occurs as spots.
What I want to know is, what about huge patches of darkened skin?? Like inner thighs or groin or underarms?? It's not because of sun damage, cause it's much worse. What can be done about this??

Thank you, Dr. Lim. We need you here in the USA. I’m supposed to be referred to a specialist for pulsed dye laser, but my paperwork must have gotten misplaced. Still waiting. I’m an acne rosacea patient, but my skin is calm now because of my own dermie, you and Dr. Dray. Also, Dr. Sam’s gel cleanser and moisturizer are really helping me, along with my prescriptions and mineral sunscreen. Not bad for almost 60 years old. 👍🏻

Humble thanks Dr. Davin for discussing simplified skin care approaches point on point. My skin was diagnosed as sensitive & triggered by chemical, environmental and food allergies. I only use mineral sunscreens but it’s so drying.
Iam so glad you're back! I am using only Ph 5.5 skincare and it has helped soooooooooooooooo much. The cetafil has a Ph of 7+ and SLS/SLES is a big no no for my atopic chlorine induced beetroot crusty face ( daily swimming for spinal surgery) The very best products i have found are Ren evercalm serum, Keep cool and soothe bamboo serum & toner and the Illiyoon probiotics essence drop and Beta Glucan from Iunik. If anyone has any other products they swear by, let a girl know! xoxo

I find the lightest La Roche Posay sunscreen barely tolerable (sensitive and white rubs off on darker clothes, even in Aussie winter 😳)

Thank you for being to the point and very helpful. I Haven't done patch test but dermatologist had me tested for allergens by going to get my blood drawn. Am curious to see results, what's up with m😉y sensitive face as she is not sure if I have rosacea. But I know I only use cetaphil and cerave face washes and creams I use sunscreen and a big hat. But my skin cheecks and parts of my forehead are red and tan like but doesn't think it's melasma! I think it's sun spots sun damage ); I don't know. @@datgreekgal pretty simple!! i have a normal to oily skin...i wake up cleanse my face w/ mild cleanser->the ordinary hyaluronic acid as moisturiser->zince sunscreen if im stepping out->the ordinary niacinimide/alpha arbutin if im just staying at home(for my pores and comodone)->mild cleanser before bed and thats it for my daily routine..
the ordinary peeling solution twice a week->derma roller 1.5 mm once a week( i have really bad scarring from acne but derma roller has been a miracle..great improvement for acne scars,texture and hyperpigmentation lets say about 80% improvement in a span of 3 months) overall i feel like derma roller is what made a huge transitioning from my really bad skin to a much clearer porecelain glass skin

@@datgreekgal youre right derma rolling can be very dangerous unless you follow the proper steps(like cleaning the derma roller w/ alchohol,lifting the roller with each roll,avoiding excessive/improper use etc)if you have only superficial skin problems like pores,hyperpigmentation,texture irregularities theres really no need for longer needles like 1mm....0.5 should do the job as it also enhance serum absorbtion...talkin bout two birds w/ 1 stone ;)...in the end nothing could go wrong if you know what youre doing

You are right, every responsible dermatologist should care when the patient has sensitive skin. I have a sensitive seborrhoea sicca and told the doctor a few times before my fractionl co2 treatment. The aftercare he gave me made my skin break out very badly. Afterwards the cream i got against the post-inflammatory hyperpigmentation gave me a rush so my face is now completly covered with post-inflammatory hyperpigmentation.
Hey dr Lim, I use purpose cleanser I hope that's ok. I use a powder sunscreen 30 because it doesn't seem to irritate my skin. My problem is my neck, it super sensitive and I can't find products that my neck likes. I lost weight and not liking the way my neck is really looking!! Is there a way to firm the neck with out doing surgery and what creams are easy on the neck area. What a great video!!

The Jane Iredale tinted moisturizer I had actually contained lavender oil. Bottom line, don’t assume any product from anyone won’t react with your skin and check the ingredients label first. The company that carried the Jane Iredale product was very good about allowing a return.
I've been looking for La Roche-Posay's Hydraphase range in Australia, but I don't think it's sold here. If LRP do sell Hydraphase in Aus, can you please advise where I can purchase it?

Is tea tree oil good for sensitive, dry and acne prone skin? And I have been using 200mg dose of zinc sulphate. My age is 20. How often should I take it? Is this dose too high for me?

Using BHAs at a high ph (5.5 or higher) they work wonderfully for my rosacea skin ( Benton Aloe BHA Toner). LHAs are super, too.

Hello Dr. Lim, now I finally figured out why I always experience redness after long showering sessions, thank you! I am recently having Discovery Pico lasers for treatment of my acne scars and my face would often flared up with a week of extreme redness right after the lasers. Do you say that the high energy emssions from Discovery Pico is not quite suitable for sentive skins? Cheers.

Hi, Dr. Lim! 👋 I just wanna asked about my skin because I’ve been dealing with red spots & dark spots from my previous breakouts and I went to my dermatologist and she recommended me to use my low ph cleanser, their clindamycin toner & azelaic acid and she did a Photodynamic Therapy (PDT) on my first session, IPL Intense Pulsed Light on my 2nd session and on my third was the PDT skin treatment again and I still stick to the skincare routine that she recommended. I see a big improvements especially on my cheeks redness & some dark marks fade away but I don’t know why still have a small bumps that didn’t hurt & doesn’t look like a pimple on side of my cheeks those things bothered me 😭What do you think Dr. Lim about my skin problems? What should I do and to some red spots? or because my skin is dehydrated? Thank you so much Dr. Lim! 😬💕
Thank you so much for making this video. I went to dermatologist because I wanted to get rid of little bumps undernight my skin that you can see only in specific lighting (I have oily skin and those are oil bumps not really acne). I never had acne. She didn't make any tests or anything on my skin. She just looked at it and gave me all new products to use on which I asked is this going to bring the bumps out to surface or they will just disappear. She said they should just go away but all those products flaired my skin up and gave me horrible rash on my chin and neck, not to mention I got cystic acne all over on my chin area. I stopped using everything and it took me months (if not a year) to get it back to normal and now it's sensitive because all those products were to harsh for my skin. In all this proccess I realised that I get acne from salicylic acid. Any recommondations on a product that would do what a salicylic acid does like low % vitamin c or bha maybe?

Please do a video on what to use to reduce redness! I have clear skin, it's not sensitive, I protect it and rarely see direct sunlight since I burn to easily so I avoid it even wearing high spf, rarely ever have a breakout, but my facial skin and my top chest is so pink and red while my body is extremely pale, I'm talking I'm the palest person I've met in person pale.

Dr Lim, would you also classify sensitive skin as skin that shows no noticeable signs of irritation, but just does not feel right; such as mild tightness, and even slight burning when even the most irritant free products are applied to the skin? It seems like no matter what products I use, my skin just does not feel right. Anyhow, as always, thanks for a another very informative video!

I have sensitive skin and I feel I'm having an allergic reaction to lidocaine after filler. Can you get fillers if you are allergic to lidocaine?
Good tips,I am very sensitive, but I actually find a chemical sunscreen for the face that's hyperalegenic and fragrancefree much better,I use soltan sensitive face spf50.I found physical suncream very heavy and caused breakouts, it also is harder to clean off,which if your subbing face to long can aggravate flareups,I think I've mild rosacea on cheeks to,so less rubbing the better!I think always aim for spf especially for the face the boxy ones to heavy and can have harsher ingredients.

nizoral shampoo as a face mask! (make sure u rly hydrate ur skin after tho - it helps if moisturize it while it's wet so the moisture locks in) also the sa lotion by cerave

Hello! Here let me help you 🙂
Am routine:
Cleanse
Moisturize (on wet skin)
Sunscreen (wait a bit for your face to dry, then apply).
PM routine:
Oil cleanse
Water based cleanser
Moisturize (on wet skin)
Actives (like lactic acid, retinols, azelaic acid etc). Some actives like niacinamide, Hyaluronic acid can go before your moisturizer on wet skin.
You really don't need to use vitamin c as it oxidises quickly and does a very little for your skin with a jacked up price.

Hi Dr. Lim ! I've been researching retinol and from what I've read, it can take a few weeks to convert to retinoic acid. So it doesn't convert each time it's applied ? I'm so confused about this. Can you please explain this to me? Thank you so much.

Personally, I find most physical sunscreens way, way too heavy for my skin. It sits on top, contributing to even more heat on my rosacea skin, it traps the heat and prevents my skin from cooling down by way of sweating. Nano zinc is better, but albeit so rare in sunscreens.

I would like to know what you think about pills and drinks in regards to skincare. What do you think about SuperFoods powders/drinks, beauty collagen pills, and hair skin and nails pills in regards to effectiveness on the skin and whether taking any of these could improve the look and texture of your skin and hair. Would you recommend any of these? Or do u consider them as a gimmick?
Preservatives like parabens keep our skincare products mould free, unlike Herbivore’s all natural approach. The moisturisers you listed have preservatives, I’ve haven’t heard a dermatologist mention the low risk of parabens before. Surprised.
Hi dear dr davin. i have hypersensitivity skin wirh mild Rosacea and Mild DS. i used oint include 50gr eucerin , 20 gr Hydrocortisone and 5 gr zink oxide for 1 mounth. it treated me completely but when i stopped oint , flared up.. i have to tapper sloely or i depend to that? thanks handsome doctor
