Help please? I have asked for a second opinion multiple times, my MediCal group (PIH Downey, California) just sends me back to the same Orthopedic Surgeon who tells me the same thing; “You need a four-level fusion!” When I reply; “Yes, I agree that my neck is indeed broken! However, the recovery plan of one night in the hospital, and six months at HOME…might be a problem!? I am homeless, unemployable, and living in my 2001 Ford Expedition? His answer is basically, “that’s not my problem, I’m just the mechanic.” I’m paraphrasing, of course, but sadly he did actually refer to himself as a mechanic-not very comforting.
What do poor people do? I’m new to this new reality…

I wish I had this in depth knowledge when I went to the Doctors…
I was off work through Covid and to get some exercise I went out daily on my bicycle. I fell off and landed on my right shoulder, I didn’t go to the doctor because at the time it didn’t really hurt that much, just nursed myself at home. Obviously, due to the strain on our useless NHS system, I stayed away from doctors and hospitals.
Not thinking I’d done anything serious I took it easy until I was asked to come back to work. I’m a motor mechanic and whilst doing some strenuous welding work I got a crick in the neck on the right rear of the neck.. it actually felt like it locked up, with a crunch of bone on bone? I couldn’t help but think it was as a result of the fall from the bicycle a month or so earlier.
As soon as this happened I struggled with strength loss in the right arm. Also, I was getting pain under the shoulder blade and tingling in the right hand.
I went to the doctors but just as well not have bothered! I was not happy! I also went to a other specialists who did help but not entirely. The weakness in my arm improved over 6 months but I couldn’t put my head back to the right without it being very painful, thus also causing pain in my thoracic under the shoulder blade. Eventually, after a great battle with doctors I managed to get an M.R.I scan. I’ve been given a results letter but not seen the scan results personally! In the letter they stated degenerative wear due to my age as the conclusion. I was so angry, they completely dismissed my previous fall and injury.. no further treatment was given apart from maybe a pain relief course, I couldn’t take that offer due to the fact that I had chronic tennis elbow in the right arm and had to eventually get an injection to relieve it. Now I have it in the left arm! This meant an injection in the left elbow, that hasn’t worked, still in pain. My neck has improved but is still painful whilst doing my work, not had injections for pain relief in my neck yet, due to possibly needing another in my left elbow again. One of the things I detest about our so called “wonderful “ NHS is that no one listens! I’ve been a motor mechanic for 36 years, owned a business repairing vehicles for 19 years. The one thing that is so, so important about the diagnosis of any fault is listening to the customer! They may not know the terminology but sometimes the basic description provides the answer! Over here in the UK I believe the problems we have is in the breakdown of the chain of so called specialists. They don’t listen, they all think they know better, and they’re all too quick to make assumptions about how someone else is feeling. The problems I’ve been dealing with have spanned over 3-4 years, all whilst working a tough job. I’m sure that if the so called specialists that operate in these fields had a more understanding, sympathetic approach, and listened more to their patients, the right action for repair would be taken. Listening to your video helped me understand my conditions, and to be honest, with my information from my accident and your video information I have been able to get a clearer understanding of what potentially may have occurred all those years ago.. I’m 54, and although I’m no spring chicken, I don’t believe all of my injuries were due to degenerative conditions! I’m trying to self heal with stretching and exercise to improve my arms and neck… it seems to be working.. just wish the tennis elbow in my left arm would go completely!
Try a chiro, get onto high dose magnesium chelate/glycinate for muscle and b1,b6,b12 to feed nerves etc...might reduce the nerve muscle sensitivity.stress exacerbates all these disorders as well.

My MRI showed at both C5-C6 and C6-C7 moderate to severe canal stenosis and cord flattening, bilateral foraminal stenosis, and posterior bone spurs. My symptoms have been slowly worsening with numbness on my fingertips, neck pain and tingling between my shoulder blades, unsteady hands, balance issues, and positive Hoffman. Bending my neck back greatly aggravates the numbness on my fingers and neck pain almost immediately.
Does the immediate worsened numbness on my fingers from neck extension indicate compression of the nerve roots or Meyelopathy compression of my cord?
How severe do symptoms need to get before surgery is recommended?
I have c5c6 herniated from falling off the roof at wrk and c6 nerve is pinched ive been complaining telling them i also get shoot burning throbbing in both my legs feet knees and esp my right knee i cant walk on it both my shoulders,shoulder blades arms and esp my hands my wrist my fingers burning pins and needles shooting throbing headaches Cant Concentrate Drop objects cant open jars can write my doctor said that I shouldn't have problems with my legs but from what ive learned u can my doctor sats that my lumbar mri was fine but im still having these problems and he walks out slamming the door on me waiting on 2 nd opinion
Similar, I have c6 damage and was unable to move much for a few weeks. Leaned over and collapsed in pain a couple of times, had saddle numbness, so I suggested another MRI further down the spine to check for more issues but Dr said it's not caused by the spine issue but inactivity. The collapse only happened twice and with time, I was able to move more and it hasn't happened again so far so perhaps he was right. I still have numbness and pain in my right neck, arm, back etc but that's related to c6.
Is the Anterior ACDF and the Posterior Lamanectomy one or two operations at different times?

Hi.i am not a doctor but wondered if i could Ask you a question regarding the shaking hands..You gave such a good example with eating with a fork and the food shakes of. Never found any info about it.my doctors say it is secondary. Had an accident and are now changing the pillows between c5-c7..the roots are squeezed. I am a goldsmith so my hands were 100%steady before accident...Also have muscle aficilations or Una ble to control the small muscles in the face. Any tips on where to find more info on this..seems like the spinat cord Also is affected in my accident ?
Radiculopathy symptoms from Thoracic Outlet Syndrome seems likely from an accident severe enough to cause cervical radiculopathy and/or myelopathy.
With TOS the Scalene muscles in the anterior neck pinch branches of the brachial plexus as it passes between them.
There are several other syndromes named after the muscles involved in pinching nerves between the spine and fingers.
Dr. Nabil Ebraheim has great videos for each one here on UA-cam.
I would suggest starting at the hand, determining which nerve can cause the problem, or problems, & follow it through the entrapment sites back to the spine.
You could have several problems producing the same symptoms.

I have had 2 cervical disc herniations and I have nerve damage from the neck down, I don't understand why doctors refuse to believe that spinal cord damage can affect everything below the point of compression. My upper and lower extremities along with numbness on the left side of my body from chest to toe, with extreme skin sensitivity, and bladder and bowel issues. And no I do not have cord damage in the lubar/thoracic area's. All of my issues stem from disc herniations at the C5-7 levels.
Albany Mountain Homestead you’re absolutely right. Any compression injury of the spinal cord in the cervical spine, even from a simple disc herniation, can result in numbness, tingling, muscle weakness, skin hypersensitivity, etc. all below that level of compression. If the disc hits the spinal nerve root, sure, it causes symptoms stemming from only that level on that affected side. But if the disc hits the spinal cord, the symptoms can be anywhere and potentially everywhere below that level. Any medical provider who tells you otherwise has forgotten their neuroanatomy and needs to refresh their memory. Sorry you’re dealing with those annoying and problematic symptoms after your disc herniations. Best wishes to you!

Such as the extreme skin sensitivity along with other related symptoms, I learned could be small fiber neuropathy, yet I cannot get any doctor to acknowledge or confirm it. What am I to do?

My God you hit a nerve on different diagnosis you explained and have not been explained to me exactly what's happening to me. I have alot of everything you're explaining. I wish you were my doctor. I don't know what to do been to different doctors and still suffering and deteriorating. They all get stuck on the one major pain I'm having for my visit. I try to explain it's universal and many things and pains everywhere.
I had a ACDF on C5 - C6 - C7 followed by a laminectomy a year later to decompress a bit more. lower back pain is ever present and is driving me insane; in its own way it's worse than the myelopathic nerve pain. Having a nerve block in a couple of weeks to see if that helps. I hope the junkie who hit me in the face with an axe is rotting in jail somewhere.

For many, yes. That the hope with the surgery- to decompress the nerve so that it has a chance to return to normal function, which generally means the goals is decrease pain, numbness/tingling, and motor deficits. The earlier this is accomplished, the better chance there is for a good outcome for the patient.

Thanks for watching! Second opinions are a great idea in almost all situations. MRI of the C-spine reveals a lot of information if degenerative discs are to blame for neuro symptoms. It’s also possible to have two things at once.
You should consider plasma cell injections into the disk before any surgical procedure. Some people do stem cell as well. There are a couple of clinical studies out in January. There is a clinic in Michigan. Athletes do this frequently.
Thank you for very helpful explanation. I was very confused after undergoing ACDF C4-7 and still having problems. This mess came to hunt me after accident. Still C6 and C7 radiculopathy.
Many said fusion is not the best solution unless spine misalignment or instability
I have cervical spondylitis mylothapy I had the operation in 2009
I would have enjoyed it even more if not for the fact that I have both problems.

Tell me more I had surgery 10 months ago
I am John Goh, Chinese Singaporean, aged 81, married with grand children. I have been suffering from spinal stenosis and had been advised by my Orthopedic surgeon to go for spinal surgery, but I have not decided to do so due to my age and also many cases I followed on UA-cam, who had undergone spinal surgeries of one kind or another were not encouraging. Some had gone worst. So I am interested to join your support group to learn more - like other alternative treatment without surgery. Thank you.
Good ,my mother has been diagnosed with cervical spondylitis myelopathy and was advised for surgery while she is 75 years ,diabetic ,hypertension n heart ,and severe COPD that keeps us whether she be put on surgical intervention....and what are chances for here recovery from myelopathy.

I was diagnosed with myelopathy (bulging disc) after having constant neck pain and occasional paresthesias of thumb and index finger (both sides) for 8 weeks now. Doctors recommend surgery. Is there a chance of self-healing after such time of non-improvement? Thanks.

@@mylovleybabe I could hardly do anything. Maybe some very gentle mobility exercise like looking up and down, left and right and some isometric resistance training (holding your head straight and pushing gently from all sides: front, sides, back). Sleeping was the worst part because of the pressure. I bought a very slim pillow (1 inch) and placed head and shoulders on it, neck absolutely straight! 2 of 3 surgeons advised against surgery in my case. I had none. Now, after almost 3 years, my neck is much better. I have no more numbness in my fingers and I can sleep without pain. However, any small false movement or sitting on the couch the wrong way and I am stiff for weeks again. There is no one solution for everyone. It's important that you get an expert opinion! Even better: get 2 or 3! All the best!

I simply can’t accept any surgery. Those types of surgery are a mess from what I’ve heard. You mean that in your situation it healed by it’s own?
@@mylovleybabe Well, the bulge was finally absorbed. The damage is done, nevertheless. The degeneration increases over time. There will never be a "like before" again. In some cases a surgery makes sense: if the discs or vertebrae are completely destroyed. Your neck needs to be fixated then. The neighboring discs/vertebrae will be abused even more then, though. Wait a few weeks and see if it gets better by itself before you make a final decision!

I did an x -ray Ct scan (whiteout contrast) I wasn’t able to do RMN cause I’m claustrophobic as well as some other conditions. Can you take a look at the results and maybe you can draw a conclusion if there is a possibility of spinal cord compression. They don’t mention anything about no narrowing of the cord. Here are the result...

I have front and back myelopathy surgery a month ago do not do it im 38 and i been useless to the point depression has took over my life do not do it
I just want to say if you can do therapy or stem cells and avoid surgery do it 1 life
I see plenty of normal patients with 3+ reflexes. No clonus, though. I think the idea all normal patients have 2+ reflexes is a myth.
I would agree with you that not every "normal patient" will have 2+ reflexes. Like you've seen many with 3+, I've also seen many patients with what I would call 1+ without any other signs or symptoms of lower motor neuron disease. As you know, many physiologic processes can alter reflexes, such as electrolytes and hormones. That being said, I think it's important to acknowledge that we would want to see a collection of these signs and symptoms, rather than a single physical exam sign, before saying a patient has a certain syndrome. By this I mean that just because a patient has 3+ reflexes at the patella does not mean I'm going to order a whole list of MRIs looking for the cause of UMN disease. I would need to see other signs and symptoms that go along with those 3+ reflexes before I get excited - need to see the "syndrome." If they had 3+ reflexes, clonus, and a questionable or positive Plantar response? Now they've got my attention. Like I tell my students- the clues have to fit together, and if there's only one clue and no other sign of the disease, it's less likely that's what you're looking at. Thanks for the comment!

How can you see a syrinx when you have an ADR in c6 c7 ???
ADRs to cause a decent amount of artifact on MR imaging, so it makes it hard to see things like a syrinx at that level. Unfortunately, the other types of cervical spine imaging would not reveal a syrinx, including myelogram or CT. A syrinx is really only appreciated on a clear MRI.
