So why didn’t you include the liver and kidneys since at least 80% of t4 is converted to t3 there and the other 20% is converted in the kidneys? Thyroid makes t4 but the liver and kidneys have to convert free t4 to active t3 in order for it to work on the pituitary and hypothalamus gland? Just wondering why this step was missed because if you have fatty liver it could effect the conversion of t4 to t3.

So I’m a little confused, my Kaplan MCAT text book agrees that T3 and T4 are tyrosine with 3 and 4 iodines attached, BUT they disagree that they are lipid soluble. In fact, the book says that they are water soluble. Who do I listen to?!?
So upon further research it seems that T3 and T4 are both classified as peptide hormones but they behave like steroid hormones because they are lipophilic, so they actually do cross the lipid bilayer of cells and bind to an intracellular receptor.

Total T4 includes the free T4 (floating in the plasma) and bound T4 (bound to the taxi 🚕, known as TBG). If the number of Taxis (TBG) decrease, the number of taxi passengers (bound T4) will decrease, and therefore the total T4 is reduced. However, a reduction in the number of taxis will have no effect on the persons that walk to their work (free T4), and since the feedback loop (TSH) is based on the response to free T4 (which is normal), the TSH will be normal, because the free T4 is normal! Hope this helps!
