Those were not Itachi's eyes. Not only were they inverted, they were also a different shape. You CAN use the Izanagi more than twice. You just need to transplant new Sharingan. That's what Danzo did, he had a bunch of Sharingan in his arm so he could cast it a bunch of times. The reason there were so many Mangekyou is because the Uchiha learned how to get it, and a bunch of people started killing their closest friends to get stronger.
This backstory has so many plot holes. Like why are there so many Mangekyō Sharingan users when clearly the original backstory was only Madara and his brother were the only ones who unlocked it (during the warring era)? Did the writer think we forgot or.....
