How Long Does it Take to Feel Better With a Postural Restoration Program?

  • When using Postural Restoration to help people with chronic pain, the most frequently asked question I receive is "how long does it take to feel better?"
    I usually reply that it'll take a month or two but that under most circumstances you should feel better before that.
    However, a more complete answer, albeit a rather abstract response, can be this: "how threatened does your brain feel?"
    Pain is a way for your brain to change your behavior.
    It tells you that something is wrong. Idiopathic pain, pain of unknown origin, is usually an indication that your body can't move properly from side-to-side.
    A body that can't move from side-to-side is a threat, because it means that a body is unstable, and the pelvis is unstable and that means you are under threat of falling.
    To decrease your brain's sense of threat, you must stabilize your pelvis. That's why pelvic stabilization is so central to Postural Restoration.

    I kinda knew some of this through my own trial and error as I have many injuries and had to walk with a stick for quite a long time, however my posture and balance is getting much better and I'm becoming less reliable on my walking aid and getting stronger, I've just subscribed as I've found some interesting stuff that you've said which confirmed my beliefs about certain traumas and various injuries!, so I will be checking out more of your videos thanks!🙂✌️

    Hi Neal.
    I thank you from the bottom of my heart for this information. Thanks to the almighty, he brought me to your page and I could identify this is the problem with me. If you can help us what habits we should follow to reverse it. Like standing, sitting on a chair because we mostly sit on a chair for more than 9 hours. If we do the exercises of AIC pattern as many times in a day, will it harm the body or structure or will it resolve faster as the brain will learn the pattern.

    Hey left aic sufferers. I recently neutralized my pelsvis eventhough my body wasnt ready for it. What i mean by that is my right glute isnt strong enough to support my weight being neutral again. I almost tore my MCL just from walking simply bcs of right glute weakness.
    So yeah you can easily become neutral again by getting stronger left hip adductor muscles (this is key to neutralizing pelvis) . But right abductor muscles are equally as important to stabilize your body walking.
    Key take away is.
    1. Dont rush yourself , it might take a few weeks. Rushing it may lead to unwanted knee injuries.
    2. Exercise your right abductor and left adductor EQUALLY.

      Have you tried terminal knee extensions? Or just sitting against wall with straight legs? I found my knee would not hyperextended, and yeah did similar, but have been doing sled pulls to help strengthen my quads it helps my knee

      Do you know any good exercises that target the adductor? Using a thigh trainer works both sides. Single leg pistol squats are ok but works the hamstring more.

    This sounds like I have to completely re-learn how to walk. I understand how this would help, but it sounds so difficult. I don't know if I can learn this from a video. Sounds like I need to learn in person. Do all Physical therapists know how to do this? Also, I'm not sure what all the abbreviations are, like right AIC and TMCC?? And what is a PEC? I feel like I need a glossary and dictionary of these terms. Is there a manual with diagrams of all of these, and a description of how I'm supposed to move my pelvis, legs, etc.? I have had lower back pain for 8 months now, and would really like to learn how to minimize it. I just ordered the book

      There are many Postural Restoration therapists in North Carolina. Everyone needs help from a professional. No one can be expected to do this on their own. This channel is for people who are studying PRI, or to make people aware of it.

    I’m starting with a PRI practitioner later this month! Iv been to so many other ones and we are all at a loss of where my pain in my back, hip and butt cheeks is actually coming from! Listening to this last video I’m wondering if it might be coming from my teeth! I’m just getting ready to go into invisilgn because iv noticed my teeth have moved and my bite is different than it was! Have had TMJ issues for over 30 years.

    Hey Neal! Will there be improvements of rotation of spine torso area? Im having crazy spasms on my R mid trap and L mid trap tightness . Or do i need something else other than just L AIC exercises?
    Edit: oh yea i tried your "Skill 2" YT video. Leaning/shifting weight into L stance im getting very worked right glute during pronation of R feet is it normal? And should i anterior tilt my left hemi pelvis while keeping a slight bend leg and where should the distribution of weight be? The heels? hamstrings should fire like the 9090 hip shift?
    Im really interested on that particular exercise since it is standing i could do it anywhere. Hope there will be an in-depth tutorial

    So are those with astigmatisms or other eye problems not "completely" correctable are doomed to bad posture problems? I've been told that it is prohibitively expensive to correct my left eye to 100% and will have to settle for 90% and just live with the difference without a sum of money that I cannot provide.

    I’m so confused I keep thinking im a left aic but apparently your right side obliques are supposed to be stronger but mine are weaker or normal and my left side is just overactive it stronger so I’m so lost now, I think I’m oriented to the rught

      @@ashleytaylor994 I’ve made good progress. I’m doing ANF therapy, massage and working with a PRI provider and I’ve been able to greatly improve. Going at it alone is tough though. I’d suggest you find someone to see in person if you’re going through this too. Hope that helps you!

      @@seanc.5310 where did you start? I dont see any pri providers in my area? Nearest would be a flight away. I want to see someone in person but not sure if it’s 100% necessarily

      @@postskeetclarity i live in Boston so i was lucky and had a number of options. if you’re not in a major city it may be more difficult to find providers. I worked with a provider for the assessment and then about 10 sessions after. I still work on it daily on my own, i feel way better and understand what’s going on now.
      My advice is; Educate yourself from Neal Hallinan, Conor Harris and Zac Cupples on UA-cam. These guys all get it and have fantastic content that will help you.
      Some providers do offer remote sessions but it’s going to be more limited than in person.

    Neil, i’ve taken sessions in person with a PRI graduate and he helped me a lot. I cant afford in person right now. He advised me to only tune into you because you know the science of PRI I have overused rt side, iliac crest high on rt side along with hip, knife like pain at thoracolumbar junction. Nothing touches it. I need help.

    I can’t believe all my right side from head to toe is completely out of balance even myself has noticed the difference between both sides I wasn’t feeling like that until I was very sick and was bedridden for a year I developed peripheral neuropathy but I wasn’t in pain no one told anything about physical therapy just medication a lot I didn’t take all the milligrams they prescribed but I took. I understand that everything is connected to my childhood but this situation is more recently. After having Covid and developing long COVID symptoms that when my whole body and mind changed. Little by little my pain has been increasing until like now that some times is so strong it blocked me out. I started working with a good physical therapist and seeing a chiropractor but I had the bad luck to fall like 3 times and I noticed when my right glute shift under and my right foot shift out my knee is locked and that’s my from my hips down on my upper side is everything you said I know I don’t breathe with my diaphragm my breath gets stocked in the middle of my chest my neck is very tense my shoulders I can’t barely put it on not just the pain I have more issues putting together since Covid I developed a chronic dehydration my body it’s not absorbing not matter how much I drink so right now very dehydrated also my age is a factor the same I don’t teeth because i lost and I haven’t been able to by new one. This theory sounds very interesting. I’ve also been listening about fascia and how they can shift and put together in place all is displaced I don’t much about because their information is more restricted than yours and I am not forgetting my mental health living in constant fear and anxiety because I don’t have any one having my back I am alone fighting for my life for the last 3 years and getting information from here UA-cam somethings makes sense to me others not but I think yours got me more round it because and overall of everything. My case is not easy not just age because 3 years ago I didn’t have almost nothing and now I have a bunch of things so I deteriorated very fast with a bad health decision of others no caring what will happened to me if things didn’t work out

    I've done so many things wrong in the past. I've looked at old lifting videos and old photos of my body and I've had a tilt my whole life. But the problem is I done powerlifting on an elite level for my bodyweight. I also had a job digging and fixing gas mains, so my tilt is so so bad now. My ql was actually so tight kn my right my rib was touching my hip and the other side was just completely open

    Thank you Neal for all the information on PRI and helping us understand to a better degree of the techniques. My husband has a prosthetic L eye and can only see with his R eye. His body is hurting on the R side. What is your recommendation as far as where to start since his vision is the driver. We have purchased the recommended PRI Brooks shoe and he stated this helped about 40% more to feeling better while walking verse his old shoe. My question is: Is there any recommendation for this condition with the PRI vision information? I look forward to your response! Mahalo, Julie

      Hello, Julie. Great question. If you watch my videos about vision (which you probably have), I mention how my brain effectively "ignored" my left eye for most of my life (unbeknownst to me). Not being able to see with the left eye puts a right dominant visual system on top of a right dominant body and right dominant awareness of the ground.
      My guess is that the only thing you can really do is do standard PRI techniques because they all emphasize stabilizing the left side of the body while de-compressing the right side of the body.
      Did you see a PRI therapist?
      Does your husband feel more right sided molar contact? Or any issues with his bite?

    My pelvic tilts again and again, due to pelvic tilt, my right leg is just a little longer than my left, my ribs in the upper back (on the right side) also go here and there, my whole right side is apparently stronger than my left side, also i have hyper extension in my lower back, please tell what exercise I must do, am only 28

    I have been doing some of your techniques and these are honestly life changing! My right QL pain has already subsided in a matter of days. Sometimes after doing the techniques I am experiencing some muscle aches and a small headache. Is this simply because my body is going back into proper alignment similar to when you receive a chiropractic adjustment? I am due to see a PRI accredited Chiro next week but just thought I would ask!

    I am dealing with all of this. But since I started wearing bifocals I haven’t been able to get a pair of glasses that actually help me see better. So I am constantly having to take them off and put them back on. And as far as shoes go I don’t walk well if I have much sole under my feet. I walk very well barefoot. Mostly. I have fallen so many times since 2016. I fell and tore my left rotator cuff in four places. And I was two years or more before I could get it repaired. I know my pelvis is not right because when I walk I end up veering off course. And I have to pay close attention to where and how I put my foot down. I am so interested is what you are say. I feel like I can relate to it.

    Can you do strengthening exercises at the same time as PRI? I had tendonitis in my entire left leg.. but I was told not to do any other exercises for the first 7-10 days of this PRI which goes in conjunction with what my massage therapist said because you get to many systems involved at once and the brain doesn't know what to do with all the information

      I would lay off traditional strengthening for a few weeks. PRI does strengthen, but it's "neuro-muscular" strength based on a new position/range of motion of the bones and muscles, timing of muscular contraction, respiration, and a host of other variables that are mostly opposite of the effect of weight lifting. If you weight lift too early, you may offset the goals of PRI.

      @@NealHallinan Thanks for the response Neal. I currently have posterior tibial tendonitis, something going on behind my left knee and gluteal tendonitis. So i'm hesitant on not implementing at least isolated band exercises for the tendon in the ankle just to keep it going as i'm about 9 months deep on that condition now.. I also have not been able to stand on my left leg in I dont even know for how long 8+ years. But i will take your advice as I'm confident that i'm a PRI client as i check off just about every single box of the mentioned patterns and after receiving my first session, I can literally feel my pelvis autopilot back left once I now fully expand my diaphram while breathing. Its really interesting to actually feel your bodys subconcious take over a pattern. Regardless.. My question that still remains; is my left side really that much weaker then the right, or is it just not firing as it should. Therefor in theory, will the neuromuscular pattern stick and allow the brain to change the compensation if the supporting musculature cannot support it.

    I have a weak eye/amblyoplia which I was born with that I can barely use. I’m wondering if bc my right eye which doesn’t really connect or communicate with my left brain is why all my compensations have occurred on the left side? and forced me into right tmcc pattern that was made significantly worse after whiplash?

      Have you seen a neural optometrist? I would start there. I can tell you from experience of having binocular vision dysfunction that you will get HEAVILY patterned from a screwy visual system.

    Been watching a lot of your videos and worked on the exercises as i think i might have these pelvic problems... I have a question: If i sit on the floor and rock my pelvis forward i always feel my right sit bone folding forward BEFORE the left one... Does this fit the picture? Having some trouble to envision the pelvis in space 😅

    Your work is a breath of fresh air. However, does this mean that patients who have eye problems that permanently impairs their vision, can never be our out pain and discomfort around the pelvis, or can PRI change this ?

      Hello Chand. It probably depends on the condition and how your brain can compensate for it. Sometimes you just have to learn to manage the symptoms as best possible.

    Anybody else have a protrusion at the xiphoid process? ( left side) Would this be normal for the flared/rotated left rib cage?

      Perhaps because the sternum is resting in a patterned position. In other words, the sternum is angled because of the patterned position of the ribcage, and maybe that causes the xiphoid to appear protruded.

    i got two morton neuroma - in my right foot and propably related to my lateral pelvic tilt.
    so far the exercises helped alot in my tight calves- hamstrings
    i have peripheral neuropathy so my sens of the floor i guess is bad - my feet are sore most of the time
    just praying the neuromas will disappear in time

    I cant thank you enough for your channel, its a gift that keeps on giving. I had immediate results when i first learned about laic
    bc but things somewhat plateaud for me. All this talk about extension made me experiment a little. Ive had neck problems for a while always feeling like i have to crack it (by whipping my head violently, makes me look crazy) always stretching out my Nuchial ligament and cracking things around my occiput. What youve been saying clicked last night and i decided to put my neck in a flexed position and keep it there, immediately my whole upper body changed, whole back cramping feeling muscles i never felt before (can actually see my lats now instead of rhomboid city) experienced a whole new percption of my pecs and abs (a lot more harder, more tone) i looked like a different person in 30 seconds. I honestly believe fixing your posture is 10x more effective for aesthetics then building muscle. And it makes performance that much better. I can see how a lot of people are doing things wrong by trying to counteract "forward head" and "rounded shoulders". But i guess we all gotta start somewhere. Oh and my ass is looking a lot better thanks to you. Im going to experiment with spinal flexion again today and see what happens. Im thinking if I work on getting out of extension i can make new gains in symmetry also. Thanks again man your channel transformed my whole body.

      Jarett, it's not just spinal flexion, it's "entire body" flexion. Many people are living in "entire body extension" and thus have overactive hip flexors, back extensors, and neck flexors. The all Fours Breathing exercise on my site puts the body into systemic flexion. Breathing in that position is designed to "turn off" all those muscles that keep us in systemic extension.

      So the whole spine is extended then the neck comes forward then the head extends squishing the atlas? Maybe flexing the neck isnt the right way to say what i was doing, maybe just flexing at the base, Like a mackenzie chin tuck to me feels like im using scm but not going forward. Im going to refresh myself on your exercise videos right now. Its taking me a while to understand but i can tell just being conscious of what youre saying is helping me. Did some shadowboxing in the park today and I could tell I was sensing the ground more. Felt stronger. Thanks Neal.

      @@Noone-ew2wk Yes, exactly. The lumbar/thoracic spine is in extension (you're basically falling backwards) and the neck/head has to come forward to maintain a horizontal line of sight and balance. As the head comes forward, it has to posteriorly rotate (extend) and that's where the OA joint compression (head on atlas) occurs and all sorts of funky symptoms can occur. Brain stem can get compressed, vagus nerve compression etc..... In order to free up the neck, you need to get the pelvis and ribcage back into neutrality via PRI exercises, and then stabilize the unstable areas. Otherwise you'll just fall back into extension again.

      Hello, John. Honestly, I don't have much experience with either. I think some stretching for calves, quads, and chest is good, although it won't change the patterns that I'm talking about in my videos.

      It won't work because your pelvis and ribcage are not positioned in "neutral". You'll just be strengthening dysfunctional movement and posture.

      I really don't. I've never experienced it. However, PRI looks at things from a different perspective, that it's ultimately a "pattern" issue and the only way to break a "pattern" (or habit) is to re-learn a new one, and that means only your brain can do it. It's motor skill learning. Only your brain can do that.

      My Chiropractor has helped more than the doctors, PT appts and then some. He actually wants to help me help myself outta the problem and the doctors and PT seem to want to me to be reliant on them an only bandaid the issue. Maybe I have shitty docs and Pt and a great chiro idk!!

    Thank you for these videos! I’ve had issues with lateral and rotational pelvic tilt for years. Currently, my standing xrays make my right hip look higher and my lower lumbar spine looks curved to my left slightly. My main complaint is chronic left back tightness...my Chiro says my left QL and left Psoas is tight/locked up. Dry needling and stretching aren’t helping, so I’m wanting to try your exercises. Do I sound like a typical left AIC patient based on these findings?

      @@pokemonrockzful sorry to hear that! Mine has improved, but I think it was from a really tight psoas muscle compensating for a hip labral tear. I had labral tear surgery in 2019; afterwards I’ve focused on PT to strengthen my glutes and inner thigh adductor muscles to help prevent pelvis rotation. I also started using a small heel lift in my left shoe to help level my pelvis…I had some PT’s mention one leg might be slightly shorter than the other. I also use custom arch supports. Hope you’re able to get some relief soon!

      I think you should fix your posture/pattern first otherwise you would be strengthening your muscles in a wrong pattern/posture, at least that’s what happens to me every time I try to do strengthening exercises, PRI techniques put me 2 steps forwards but regular exercises from UA-cam to fix me set me back 3 steps making my chronic lower back and groin pain worse. When I focus only on PRI feel improvement

      If you need glasses, and don't have any, that can absolutely prevent anything from working. I can't guarantee that will be the case for every person, but it happens often.

    Your smart, granted. Your " horrible" at getting to the point of exercises to correct the problem. Maybe you should " talk" while showing the exercise, to save your viewers a high level of anxiety and frustration waiting for the execution, just like you didn't show in this podcast.
