Hello Doctor, I am now 50 yrs of age. Five years back I had gone through Surgery at C5-6-7 due to cervical Myelopathy. My Symptoms initiated in Starting of January 2018. Due to compression at C-5-6-7 I felt pain in neck & radiating pain in shoulders. Then after @ 15 days my GAIT got affected. I felt imbalance while walking and poor grip in both the legs and feet. In between I was trying with treatments like neurotherepy, physiotherapy. However after one month means in Mid of February 2018 Tingling started in the neck region which were going till feet if I stretched neck to backward…. By End of February 2018 I started numbness in RIGHT Hand only & then in a week time I lost power / grip of my fingers. Additionally the legs also started jerk while walking and more imbalances. Hence in SOS situation I went for Surgery as advised by doctor in Mid of March 2018 to avoid further loss.
Now after surgery my Heavy pain in Neck, Shoulders, tingling and the jerk in walking all are completely removed. The grip in my right hand /fingers slowly recovered and in next 6-7 month means by October 2018 @ 85-90% Power gained in Hand/Fingers. Rarely some cramp develops if there is any abnormal movement in hand but I am happy with the improvement.
HOWEVER legs still not recover as that of my hand. My both legs are not weak since I can do trade mill, lift the weights by legs. ONLY I still feel stress/stiffness /spasticity in my both legs, feel numbness in legs, no firm grip. Feels stress in dorsal side of neck, thoracic region, lumber region, legs. …I am doing my regular activities but not as that earlier natural. Can't seat/walk longer causes lumber pain, numbness in legs. Can’t lean forward more causes stress/pain in lumber. Now is there any hope to get rid of this looking at meyolopathy ? It's now 5 years of surgery. I tried advanced physiotherapy for lumber, legs, Ayurvedic treatment/ medicines, doing daily yoga, etc however no improvement. Whether this could be recovered by any means? Looking at surgery at cervical is it advised to go for pain management/ spine injections etc ? Please advise suitably. Thanks!

      Hi
Good to know that you made a good recovery
Myelopathy patients usually never go back to 100 percent
Some sensory symptoms like numbness or heaviness or burning and some spasticity or tightness remains in back and legs
That’s probably best u could have achieved considering your symptoms
Now I believe no intervention is required or can help in such symptoms
Keep doing your regular exercises or Physio to keep your muscles relaxed
      Thanks

@@DrSahilBatra Sir, thanks for the reply. Last week i did MRI and as per films and reports there is still persistence compression at C3/4, C7/T1 and L5-S1. These compressions are small. Pre and post surgery MRI FILMS are exactly similar. However the compression which removed by ACDF at PIVD C6/7 still shows compression because that is where the cord got damaged. These compressions might be creating these symptoms of stiffness, numbers/weakness in legs? In that case whether these injections will help to remove/ decrease the pressure on nerves and have some relief apart from Myelopathy symptoms? Further whether these types of day care procedures are covered in insurance? Please advise. Thanks!

Sir Steroid me preservative hota hai ,jispe saaf likha hota hai ki that is not for epidural or spinal use.
FDA ne bhi is procedure ko ban karna shuru kar diya hai because agar Steroid Thecal sac me chala gya to arachnoid memberane inflamme ho jayegi and patient poori life ke liye gya.
Mai khud ek Doctor hu is liye apse hath jod ke request hai ki pehle thoda research karein aur fir injections ki taraf sochein.

    • @DrSahilBatra
      @DrSahilBatra  Рік тому +2

Well I am sure you must be qualified enough to speak on this topic as you seem to know a lot more about the procedure than many qualified neurosurgeons, spinal surgeons, orthopaedic surgeons, pain Physicians and anaesthesiologists who are regularly performing these procedures with minimal side effects and very high efficiency due to which a lot of patients get a pain free life.
FDA doesn’t include many common procedures which are still commonly performed by physicians and surgeons worldwide because they feel they can give relief to their set of patients.
FDA is a body constituted by physicians and litigation lawyers who have to save their asses if anything untoward (even though very rare) happens. If for FDA , no new invention or procedure would ever be discovered or practised.
The actual issue with the complications is if u end up injecting your drug into spinal tissue or nerve tissue and that is why the evolution of epidural steroids happened from caudal epidural to interlaminar epidural to Transforaminal epidural or selective nerve root blocks which I am talking about here and the risk of complications with that is minimal as we are very away from the spinal cord or thecal sac yet giving the minimal and most effective dose to the target nerve.
Also we use steroids without particulate matter so that the chances of arachnoiditis is minimal.
In the end, I have been practising this procedure on a day care basis since 2014 without any serious side effects and hence I vouch for this procedure and it’s wonderful outcomes considering it’s very low risk to benefit ratio.
I hope u read well and do not misguide people on topics u have half knowledge of!
