    I was bench pressing 70lbs dumb bells and 3 hours later I thought I tore my trapizius muscle. The next day I'm unable to look up, the pain is unbearable while standing upright but goes away laying down, the pain starts in the rhomhoid on the right side and goes into my shoulder socket and down my humorous. Very very painful. Thank you for the video.

      @@lornakist It went away, I theorize, as I fixed form issues with my deadlift and improved my pull-up/chin ups. I think as I balanced the strength of complimentary muscle groups it resolved itself.

      @@lornakist I also deloaded about 20% on my deadlift when I tried to fix the form issues, and I think that alone really helped in healing.

    I THINK this is what I've had bothering me off and on for the last 25 years. For the first 10 years of having it, when it flared up, I was practically bedridden for a week and in great pain the whole time. The only thing that helped at that time was heat / heating pad. I'd went to around 5 doctors, 5 chiropractors, tried various PT, lidocaine injections, and probably 5 different massage therapists. Nothing helped and it was never truly gone. Then, a massage therapist that worked in a gym noticed my pain and said she thought she could help. I doubted, of course, given the history, but in short, in just one 60 minute massage / stretch session, she solved what nobody before or after has been able to fix.
    It didn't flare back up again until about 4 years later. She fixed it right away again.
    Now I'm out of state, and I've tried probably 15 different massage therapists locally, and most didn't help it at all, while 1 or 2 had a little success (but never fixed it like the other one did). I sure wish the fix for this was better known. Everything I try myself doesn't work, and when I try to explain some of the things the good therapist did, the others are too scared to try since it's not what they're taught.
    Best I can remember, first she warmed up the region with a hot towel for 5-10 minutes, then we stretched for about 15-20 minutes (passive and active resistance stretching). Then, she massaged for a while to loosen everything up (probably another 20-30 minutes, and then she (the best anyone can anyway) tries to get a hold on the scapula and pull it / stretch it away from the chest. Done right, I usually feel some release light cracking/popping noise (that didn't hurt but felt good). I'm 95% sure this was done while I was laying on my back with her hands underneath, but I think both ways were tried.
    For some reason, most massage therapists don't want to pull the scapula away from the front toward the back.
    I'm not sure Dorsal Scapular Nerve Entrapment is what I have going on (and the good therapist didn't give it a name), but it's what makes the most sense to me.
    I write all this to encourage people not to give up. Don't bother with 'fru-fru' type massage places. Try to find one that either knows about this or perhaps works with weightlifters / body builders or at least tries to fix things vs just make you feel good.
    If anyone has a suggestion of a type of doctor / massage therapist / chiropractor to go to I'm all ears, as I'm trying to find a local solution and have exhausted the most-likely ones already.
    OH, and I should mention, even when it flares up now and feels crappy, it never makes me bedridden, since I know to immediately start stretching when it starts, and then it never gets 'horrible'. I remember at first, stretching was hard to do since it was painful to go to far, so I'd say just take it very slowly, but spend a lot of time doing it. Don't be afraid to spend 30 minutes stretching the neck, pulling it downards trying to loosen up the contracting ?rhomboid? and other local back/scapula/neck muscles.

      I also had tried acupuncture, cupping, and other things too. Bottom line, stretching and a knowledgable massage therapist was the fix for me, but finding the right one that understands this problem is very, very hard.

    Great video. Whether or not I have this, it was very helpful in understanding the phenomenon. I had a chiropractic adjustment that caused my neck and upper back seize up. I soon experienced tiredness/fatigue in my rhomboids and changed my posture to get away from this discomfort. My shoulder began to feel less stable over time. Radicular pain then started up in my rhomboids when I would retract my shoulder blades. An MRI of my cervical spine came back clear. So I am hoping this may be it.

      I don't think there is a best type of massage for this condition. If I were you I would schedule to see a Doctor of Chiropractic for evaluation, diagnosis and treatment. I would also see a massage therapist that specialized in sports injuries.

    I got taken for a tackle in a football game years ago. I was basically pile driven right onto my left shoulder causing a compound fracture to my collar bone. Years later the collar bone is fine but my dorsal scapular on the opposite side is in terrible pain. I was advised chiropratic massage for relief but this made it worse to the point that the pain was excruciating and i was unable to move my neck. I also gained a lot of pain down the back of my arm. I will not allow anyone to touch this area anymore and and i have learned to live with it.

    Hi Dr. Ozello. I live in Georgia. I have been searching for relief of scapula pain which after a couple years now the pain continues to the front of my shoulder, down my forearm and I have tinging in my fingers... all on my right side. Over the last 4 years, I have seen 4 chiropractors, had 3 sets of trigger point injections, months of massage therapy and none of them have eased my pain at all. I've been reading a lot about Dorsal Scapula Nerve Entrapment and seems I have all of the symptoms, but need to find a doctor that knows/understands how to diagnose and treat me if I do have this issue. How can I find a doctor in Georgia that has experience with this?

    This happend to me because I was ego lifting and not warming up and stretching properly. I lost all strength in my left chest, tricep and shoulders. I literally couldn't lift 10% of my strength on my left side. It happend just before lockdown so I wasn't able to see a professional. It's not as bad now but still abit weak

    I was finally diagnosed with this after two years of seeing so many different types of doctors- 2 spine docs, PT, chiro, etc. Everyone kept telling me my different back muscles were really weak (just an average 50 year old women who hikes a lot but does not lift weights). For two years I did all sort of stretches, every PT for rhomboids etc and it would often feel worse but def. not better! My question is, if I try and strengthen the involved back muscles, won't this aggravate the nerve? I feel like everything I have done before the diagnosis was not helping at all!! thank you!

      Excellent question. The nerve entrapment needs to be removed. If I were you I would see a Doctor of Chiropractic and a massage therapist on a consistent basis to lessen the nerve impingement. I would ask them to focus on the middle and lower regions of the cervical spine (AKA neck) and the scalene muscles.

      Thank you! I realize the nerve starts in the neck region but my pain is ONLY in the mid back area- where the nerve sort of comes to an end (under scapula etc). Knowing that, still focus on the neck region only?@@drdozellodc

      @@lauraleegreen7516 The focus of the treatment should be the elimination of the source, which is the nerve entrapment. I don't know where your nerve is impinged, but in most cases I have treated, the impingement is in the cervical spine at the scalene muscles.

    Great video Dr, really appreciate the content. Just a quick question - all my life I've had this pain come and go. It's not too bad or unbearable, a slight burning/tingling in my right shoulder blade. However it seems to be causing a shoulder/latissimus dorsi imbalance, with uneven trap control and shoulder flexion. It seems to go after months of heavy lifting with compound movements and special focus on form when weight lifting. Is this something to worry about? It only hurts when I bow my head forward with an upright posture. Thanks again Doc

      Great question. If I were you I would see a medical professional, most preferable, a Doctor of Chiropractic for evaluation and treatment. Strength imbalances and burning/tingling symptoms can continue to progress in intensity so it is best to get it checked and treated.

    Hi doc.
    I’m italian.
    From 4 years, I had big pain to the scapular and occipital nerves.
    I made many magnetic resonances, but nothing help me.
    this situation does not allow me a life. When I get up, not bad, then after 10/15 minutes, a mental confusion arises, as if I were drunk, in addition to severe pain under the clavicle and shoulder blade .

    Can this cause cold feelings in the back and in between shoulder blades randomly and sporadically for different but not constant length of times. If that makes sense. Thanks for any insight.

      Very good question. I don't know for sure but I doubt it. The reason is that the Dorsal Scapular Nerve does not supply sensory innervation to that area. The thoracic nerve roots supply sensory innervation to that area. If I were you, I would schedule to see a Doctor of Chiropractic for evaluation, imaging, diagnosis and treatment.

    What tests could I have done to determine if I have this condition? I started thinking I had cervical radiculopathy bc I have intermittent shorting pain that went from my shoulder blade to rotator cuff/tricep area. When I didn’t get those shooting pains, I would just have various aches in my shoulder blade dependent on activity. I did PT to target my neck. By the time I got into an MRI, no compression was seen. I have not experienced the shooting pains in months, but I still have a pain in my shoulder blade and the muscles in front of my shoulder are tight. I also have mild lateral winging of my shoulder. Is there a way to get a definitive diagnosis? Does it take a long time to improve this condition?

      If I were you I would see a Doctor of Chiropractic for evaluation, diagnosis and treatment. I would ask for an MRI of cervical spine (neck) and thoracic spine (midback). (You wrote you had an MRI, but did not say in what area). I would start performing stretching exercises for the chest and neck, I would start performing strengthening exercises for the posterior neck, midback and scapular retractor muscles (Rhomboids, middle traps and lower traps).

    For three years I have thought I had a bad muscle tear or tendon tear. I've had 3 MRIs trying to find what's going on, but now I'm approching this as a nerve injury to my DSN? I'm at the point that when I pull my shoulder blades back it triggers intense weakness with some pain down my upper back and across the shoulder blade. It started after lifting something over head in a twisting matter, after doing that I had 10/10 pain down my upper back starting near the c-7 and went down a few inches. Had the sharp pain for about 3 hours then it was dull after, but 3 years later finally might try a DSN nerve block just to see ?

      @@drdozellodc I did do PT twice and chiropractor, but never really let the chiro get into the cervical area in great detail, I think I'll let him just go for it ! I used to be nervous about adjustments but after I let him adjust the first three ribs in the back, I'm more comfortable

    Thanks! There's very little info about this condition online so I appreciate the time gone into this. Quick question if you don't mind; I have a complete numb spot on the rhomboid/scapula, and slight numbness on the levator scapule on the other side, with chronic posterolateral shoulder pain, but I only have very very slight scapula winging and I can still retract the scapula (albeit not without neck pain) and because of this, my doctor tells me it can't be the dorsal scapula nerve, but he also has no idea why I have numb spots. Should I take his word for it, or ask for a scan anyway? Thanks!

    Sitting at the computer (arms extended for long periods of time) causes my left side upperback muscles to cramp on the dorsal nerve. I know the aching pain is coming from the dorsal nerve because when I stretch it , it definatly hits "that spot". I dont know what to do though, either stop using the computer/driving a car or stop lifting (I got much hyperthrophy in the surrounding muscles)...I have tried everything non-surgical but nothing helps. Is there any kind of surgery for this problem??

      If I were you, I would see a Doctor of Chiropractic for evaluation, diagnosis and treatment.
      I would concentrate on building strength and endurance in the scapular retractor muscles (Rhomboid major, rhomboid minor, middle trapezius and lower trapezius). I would stretch the anterior shoulder muscles, chest, lats and ribcage muscles.

I would focus my rehab on stretching the chest muscles (pec major and pec minor) and strengthening the muscles that retract the scapulae (rhomboid minor, rhomboid major, middle traps and lower traps.)

    I've been experiencing severe pain in my shoulders and upper arms as well as neck ever since I went to an acupuncturist for carpal tunnel syndrome. During the treatment I felt one of the needles she placed by my shoulder blade touched a nerve because I went to slightly move my arm and I felt an electric current and awful shock going down my arm. The Accupuncturist just left me there for 20min so there was nobody around to remove needle. I was breathing through it and after a while it dissipated. I left and never went back there but now I'm having weakness on my left arm and sometimes right arm as well and feel like my neck and shoulders are always in spasm. What kind of doctor do I need to see?

      If I were you I would start by seeing a Doctor of Chiropractic for evaluation, diagnosis and treatment. I would inquire about imaging (X-ray & MRI) special testing (Nerve Conduction Velocity Test).

      This is also what I'm searching for, unsuccessfully. Any updates? Lots of nerve flossing techniques out there for other nerves in the region but haven't found a specific DSN technique.

      @@DBRMT Place a ball inbetween your shoulderblade and the spine, depending on which side the nerve is impinged. Then bow your head to the opposite side in a repeated fashion.

    I have a burning pain in what I think is the right infraspinatus area or border of scapula but I’m not 100 sure, I have pain in that area when I look to the right and up, but prior to this specifically I have continues right arm weakness and pain down the arm to my fingers that comes and goes, I can “pop” my right shoulder joints multiple times a day by lifting my arms over my head. I’ve grown use to the arm issues but this burning 🔥 se sensation “somewhere” in my right upper back/shoulder blade area is going on for 2 weeks. My question is, should I start at PCP, or go straight to see an orthopedic dr? Or Chiropractor?

    Hi sir, I got severe pain in shoulder due to working on system for long hours. I also have bladder and bowel dysfunction due to this. Mri showed C4-5,c5-6 problem slightly impinging on thecal sac. Early cervical spondylitis. I have been to many doctors but every doctor kept saying i have urinary infection or stone but the problem is i either have urinary retention or frequent urination. Please help sir.

    I’ve had radiating nagging pain under my shoulder blade down into my firearm for a month now. I’ve had 3 chiropractic treatments with active muscle release, 1 massage with cupping and myofascial release, 3 muscle activation treatments, I’ve tried every nsaid and muscle relaxer. Nothing is relieving this pain. I am a busy hairdresser and I’m sure my repetitive movements have brought this on. How long does this typically take to go away? I’m losing my mind.

      Recovery from nerve entrapment conditions can take a long time. If I were you I would continue with the treatment you are receiving and I would perform at home rehab exercises to reduce the pressure on the nerve.

    Can this cause weakness in the arm when doing a pushing motion? I have weakness while pushing but none while pulling, and pain in that rhomboid area with no relief.

      The Dorsal Scapular Nerve innervates the rhomboid major, rhomboid minor and levator scapula muscles. The rhomboids function to retract the scapulae, therefore providing stability to the scapula and shoulder complex. This can lead to weakness and pain in the rhomboids.

    Hello, i been feeling pins and needle near my shoulder blade. When i used a lacrosse ball, to massage the scapula, it didnt help. When i slouch because of my bad posture, and my neck is down as you said, i feel the pins and needles there. I also had stiffness near cervical and thoracic spine, when i look up, i feel it very stiff. Do you think i have this condition? ive had it for a month also.

      Excellent question, which I can not answer. If I were you I would see a Doctor of Chiropractic for evaluation and diagnosis. Getting the correct diagnosis will allow you to focus your recovery strategies in the proper direction.

    Can this be the cause of not feeling those rhomboids firing up. Pain in the area that radiates to the trap and neck, eventually down arm? I just got hydrodissection thinking it was TOS. But yesterday I felt multiple spasms in the scapula area and could literal not be able to release or relax the muscle. Those muscles back there stopped firing correctly when this started four months ago. My upper back and armpit swelled up with pain in my shoulder. Eventually the pain progressed to neck and arm and I thought tos. I forgot that scapula was the culprit when this all started.

      The Dorsal Scapular Nerve innervates the rhomboid major, rhomboid minor and levator scapula. Entrapment of the dorsal scapular nerve can lead to weakness of these muscles which can contribute to improper scapular positioning, improper scapular movement and shoulder dysfunction.

      @Dr Donald A Ozello DC So I just got done with my doctor. He stated after checking me that I have Scapular Dyskensia due to my rhomboids not firing. He suggests PRP(Prolotherapy) which is out of my budget. What would you recommend is the best course of action for Scapular dorsal nerve entrapment with dyskinesis due to: an excersize injury, bad posture?
      (Chiro, massage therapy, PT, Nerve Gliding, acupuncture, ext) I'm not sure where to start. I'd like prolo but it's very expensive. Ty!

    Hello, I had a long thoracic nerve decompression and brachial plexus decompression. I have a winging shoulder blade still. Emg and Mri do not reflect a dsn nerve entrapment. If my spinal accessory nerve doesn’t present and issue can it still be present that a dsn entrapment is there even if not shown? In your history that is...thank dr

      Hi Brittany. Great question. Pressure on the DSN may still be present even if the MRI & EMG does not show it. If I were you I would try Chiropractic care & massage therapy to the neck, midback & shoulder complex. I would also try to strengthen the scapular retractor muscles (rhomboid major, rhomboid minor, middle traps & lower traps).

      Dr Donald A Ozello DC thank you for the quick reply. I just have so much atrophy now and I still feel like I am unable to get the rhomboid to respond. It feels so limited or stretched in way. I am sure the recovery will be long. I did massage and chiropractic care for a long time. I did that as well as other forms of physical therapy for almost two years until my shoulder started to droop and my bone began to wing. I am just fearful that I will not get full
      Resolution with this. Thank you for the communication with me sir.

      Hi Brittany. If I were use I would start with isometric scapular retractions to strengthen the muscles. Start with 1 second holds & build to 6 seconds

      Hi Mohammed. Thanks for watching the video & for asking a question. The Dorsal Scapular Nerve innervates the following muscles - Rhomboid Major, Rhomboid Minor & Levator Scapula. These muscles move the scapula and do not directly perform internal rotation of the shoulder. An impingement of the Dorsal Scapular Nerve can indirectly affect shoulder internal rotation because of the abnormal motion of the scapula.

      like rhomboid and traps injury and breathing issue , also about limited internal rotation why i feel pain in triceps when i put my arm behind back and about scapula movement how to correct scapula position if there rhomboid and traps injury..... 😊

      Hi Mohammed. Tightness in your triceps muscle may be the reason your experience pain in triceps when you put your arm behind your back. It is difficult to correct scapula position with an injury to rhomboid & traps. Your best approach may be to first focus on stretching the chest & anterior shoulder muscle. This may help to reduce muscle tension & improve motion of shoulder complex.

    Doctor I have a straightened dorsal spine due to spasm will this work?BTW I have a spasm in the neck too.

      Hi Austin. Using heat helps to increase blood flow to the area. It may help lessen the symptoms but most likely it will not remove the source of the symptoms. I recommend you see a medical professional as soon as you are able to.

      @@drdozellodc thank you! I've had this tight cramp pain between my shoulder blade and spine for 6 days now. And down my right arm towards my elbow on the bottom side that feels over stretched and crampy. I'm what you call a crack addict haha, I love to crack my neck and back. I know it's not good but it became habit. But I'm not sure how this has come about. I dont want to self diagnose but, all these videos point to pinched dorsal scapular nerve being pinched. However when I'm moving around and working I barely feel it at all. It's more when I sit or stand sti is when the pain sets in. Thank you again and I wi be making a doc appointment

    I think i might have DSN entrapment, my doctor is also suspicious but we're not sure how to diagnose it..
    Should i try and go try a DSN nerve block?
    (i know your advices are not medical advices, i'm just asking for your opinion on the matter!)

      Recovery from nerve entrapment conditions may take a long time. I do not know of any one who received a DSN nerve block so I can't give a first hand account.

      @@drdozellodc Good afternoon, i have tried many stretching exercises to the levator scapulae and the scalene muscle with no success. I will try to massage it perhaps.
      The link you provided was very clear and confirmed what i've read.. My symptoms are exactly the same.
      A good question for you : How would you differentiate DNS entrapment pain from brachial plexius issues (TOS for example!)... They seem to cause very similar issues, since the nerves pressed passing through the BP (including the DNS?) can all cause very similar symptoms?
      ps. My symptoms are bilateral, both shoulders are hyperlax... Although I have scapular winging on the right..
