be sure that is a PMX file, not a PMM file. PMX file is only for models, PMM is the whole motion, pose, etc. If I'm wrong, pls someone correct me, still trying to remember how to use it ;)

this worked on my Mac but not on windows, I had to actually put the mmd_tool zip in the blender add on folder manualy and that worked. It weird because last year this method worked but when I upgraded my computer this method did not :( work in 2021

I'll check and let you know if there's a way to make the plug in run in 2.91 right now but there's no shame in having multiple blender versions on your PC. I have 9 different versions:
