Jos Verstappen's pit stop fire happened because Benetton removed the fuel filter as it made pit stops slower. Another one of this Benetton's ilegal parts. So, in order to win time they nearly lost a driver
They contacted intertechnique the makers of the fuel rig and asked if it it was possible for them to remove the filter, intertechnique said yes. So if you wish to blame someone, blame them (this is all documented btw)
This is a myth, Yes they removed the filter, but that was never illegal, multiple other teams had contacted both the Fia and the maker of the fuel hose to ask if it was OK to remove the filter and both said it was OK, and the filter had nothing to do with the spilling of fuel
