In the 1990s, Pawel Jastreboff created the neurophysiological model of tinnitus and based on it Tinnitus Retraining Therapy to help patients habituate to tinnitus. However, a recent study found TRT is no more effective than standard of care and the European guideline for tinnitus gave TRT no recommendation but a strong recommendation for CBT (Cognitive Behavioral Therapy). Jastreboff discusses the mechanisms behind TRT and looks back on over thirty years of working in the tinnitus field.

So hard i swore to myself id come back onto those "forums" once my T would be gone. 80% of the sound is gone, now very tolérable, 2 weeks (16 days to be exact) after they started. Been ordeal free for 5 days. My ENT gave me Piracetam, corticoids and vasodilatators. Don't know which did the trick (if any of those or combination) but sthg worked. I would say worth a try depending on what your country allows. Also listened to Joe Dispenza's meditations, helped a lot. This is a re-birth. Folks do not lose hope. @williamperri3437 5 місяців тому

@@TheDallasxt Woo! What’s your story man? For real, not being confrontational but I’m trying to figure out where your heads at. First of all, TRT is NOT the same as “listening to sounds” because I followed a very specific protocol over 18 months. And, it was “white noise” which worked for me. Also, the first month the DB level was practically in audible, and every 1 to 3 months there was a very small increase in sound. Even at maximum volume the ear devices were always lower than the ringing in my ears! And, when you say a very low level of tinnitus and hyper is pertaining to me, are you saying that the ringing in my ears wasn’t that loud in my Hyperacusis wasn’t that severe? I may be wrong, but I feel that’s what you’re suggesting because it’s been suggested by many other people over the years when I said I made improvement with TRT. and I can tell you if that’s what you are suggesting let me just give you a quick idea. The ringing in my ears was so loud by the time I was 20 or 21 that I could hear it over a crowd of 7 to 10 people talking or walking down a crowded street. hyperacusis severity? I’m pretty sure there’s not a specific scale for measuring the severity of Hyperacusis but from what I was experiencing and From 25 years of dealing with other people if there is a level 10 I was a 9.8! I’m talking about so sensitive to sound that I could not tolerate my dogs tail wagging and hitting the couch! Of course chewing potato chips and nuts was a problem 90% of the time, then there’s door knobs opening and closing, the wind blowing, taking a shower, flushing the toilet, most peoples voices, my voice, oh and clearly the dreaded clinging and clanging of silverware and plates. Oh, I almost forgot about just turning my head on the pillow. Yeah buddy after over a decade of gunshot level snare drums, Marshall stacks, gigging playing being on tour, and never wearing earplugs and then the dreaded summer of 1997 I’d say I had basically a level of tinnitus and Hyperacusis that could not be any more severe. I can hear my ears ringing over almost anything. Running, crying freaking out, stretching running Moore smoking weed deep breathing meditation still insanity in my ears, still thinking how am I not going to kill myself how can I even consider surviving this if this stays this way for the rest of my life. Naturally I thought who would not kill their self this is torture! Nothing was working at Best I’d get 10 to 25% improvement for two or three days and then a severe relapse but I had about 18 months of total torture from sound with none of the above really working until I tried TRT through Gail Brenner at the University of Pennsylvania. Hearing technology associates.
Now, I hope that was clear cause I’m doing voice text at 7 AM in the morning, and let me add you are coming off next level beyond negative, and I say that with sympathy because if I never got better since 1997 and I had to deal with this relentless torture for almost 30 years I’d be bitter as hell too. Maybe you’ll feel better that I’ve been having relapses for the past 25 years and I was never completely healed from TRT maybe that’ll make you feel better. But! If I didn’t try to go back to being a musician, and I wasn’t riding a motorcycle as well within a year and a half of a bit of recovery i’m pretty sure by two years of TRT I would’ve been fully functional aside from going into loud sporting events with people whistling etc. I would’ve been 85 to 90% recovered but not being a professional musician anymore probably some people say I would’ve but it seems that’s not the case. I do know one thing TRT has helped countless people and it is the best thing I’ve ever done for this condition. No I’m not capable of saying TRT is a cure for Hyperacusis but I can definitely say it can dramatically help Hyperacusis and change your life under the right circumstances. Maybe not everybody, but many. As for tinnitus? TRT is the number 1 best thing. It was a miracle worker. Brought my tinnitus down 75 to 90%, in fact, I remember having to listen for the ringing in my ears it was so distant and low and then when I finally heard it it was just like a dull white noise that was very tolerable. My brain habituated the ringing in my ears ! But, I went back to trying to play drums, I went back to trying to get a major label record deal, I went back to jamming with my friends, and playing with people that produced flaming lips albums, and worked with NIN, and started bands with members of Maroon 5. You gotta stop being so damn negative and open your mind up you are not an authority on anything other than yourself brother!

I saw Pawel Jastreboff at a grocery store in Warsaw yesterday. I told him how he was holding back the entire audiology field, but I didn’t want to be a douche and bother him and ask him for evidence TRT works or anything.
He said, “Oh, like you’re doing now?”
I was taken aback, and all I could say was “Huh?” but he kept cutting me off and going “huh? huh? huh?” and closing his hand shut in front of my face. I walked away and continued with my shopping, and I heard him chuckle as I walked off. When I came to pay for my stuff up front I saw him trying to walk out the doors with like fifteen Milky Ways in his hands without paying.
The girl at the counter was very nice about it and professional, and was like “Sir, you need to pay for those first.” At first he kept pretending to be tired and not hear her, but eventually turned back around and brought them to the counter.
When she took one of the bars and started scanning it multiple times, he stopped her and told her to scan them each individually “to prevent any electrical infetterence,” and then turned around and winked at me. I don’t even think that’s a word. After she scanned each bar and put them in a bag and started to say the price, he kept interrupting her by yawning really loudly.

urine-therapy for tinnitus is pouring & pooling some of your own (sterile) urine which heals wounds in your ear for a short while, regularly to help restore ear hair follicles properly based on your particular biology.
Good sounds to tune into are open windows, some background music or show or especially the outside breeze, birds chirping or wind gusts or oceanside tide etc. (potentially"pink noise" or organic pink noise / ocean tide played through open-ear headphones to also hear ambient acoustic sounds inclusively) regularly or often, thus combining physical and mental neural pathways plasticity restoration recovery. Plus, protect yourself and reduce or eliminate stress, strain and abuse on the body and mind to allow steadfast recuperation
