I had two questions on the CCRN where it asked about a patient who was admitted for dka because he wasn't taking his insulin because he had moments of hypoglycemia when he did. Would you contact the diabetic educator (because maybe he isn't administering his insulin properly) Or would you contact the endocrinologist. I can't remember the specifics about the other question but the two answers I was between was the diabetic educator or the endocrinologist. when would you contact one over the other.
That's a great question! With the information that you mentioned, I would probably lean towards contacting the endocrinologist. With the patient stating he has moments of hypoglycemia, it leads me to believe that perhaps there needs to be adjustment in the insulin dosing. I would be interested to see the correct answer and rationale, though! In real life, it would certainly be beneficial to consult both to give the patient the best possible outcomes going forward. I hope that helps!
Hi! On pg 241 of Barrons in the endo section, it says that lab findings for adrenal crisis include LOW cortisol and a LOW ACTH level. But I saw you have high ACTH. Just wanted to clarify this? Thanks!
Hi! I did see this in the Barron's book. After doing a little bit more research, it looks like ACTH levels with low cortisol levels indicate a adrenal insufficiency whereas low ACTH levels with low cortisol suggest aadrenal insufficiency. So i guess it could be both depending on if this is a primary or secondary adrenal insufficiency. Good catch!

I have a question. In the state of hypovolemia the urine specific is high. Why is it low in DI 🤔
In DI, your vessels are hypovolemic because you are urinating a lot and the urine is DILUTE! But usually in hypovolemia, the USG would be high because you are not making a lot of urine, thus, it is concentrated. Hope that helps!
