In advanced Stage of breast cancer the tumour probably reach’s to the muscles behind the mammary gland ، so when we remove the tumour in mastectomy operation the nerve which supply serratus anterior muscle will injuried, so the winging of the scapula one of the negative effect of this condition , and it appears clearly when she pushed the wall because of paralysis of the serratus anterior muscle and it can’t do it’s action (protraction of the scapula) so it will stand out of the body.

Answer 1: Because of the late-stage of cancer the long thoracic nerve had been damaged, so the muscle (serrratus anterior) will not contradict. then you will noticed backward projection of her scapula. Answer 2: Because of the humorous fracture, there was a sever damage in the axillary nerve that supply the deltoid muscle. Due to that the middle fibres of deltoid will not contact (do abductions). The answer to the first question is that there is winging of the scapula due to paralysis of serratus anterior as a result of injury of the long thoracic nerve which supplies it. The answer to the second question is that fracture of surgical neck will cause an injury to the axillary nerve which supplies the deltoid muscle so the patient cannot abduct his arm to 90 degrees.
q1- winging of the scapula occurs because the medial border of the scapula loses contact with the chest wall. q2- the fracture of the humerus severed the axillary nerve so he is unable to raise his arm as the axillary nerve is the nerve supply for the deltoid and it is responsible for abduction of arm from 18 degrees to 90 degrees

the answer of interactive question :first Q - she has winging in the scapula the medial border is prominent due to injury in the long thorathic nerve .this causes paralysis of serratus anterior and the force of pushing move to scapula . second Q-due to accident ,the axillary nerve injured .so, this will cause paralysis of deltoid .this will lead to paralysis of teres minor

he was unable to do abduction up to 90 degree , because it happened a cutting in the nerve supplies the muscle that acts this action [middle part of the deltoid muscle] , this nerve is [ axillary nerve c.5,6 which is a branch of the posterior cord of the brachial plexus] which its run around the surgical neck of the humerus that broke .
