Drinking Alcohol During Pregnancy

  • Drinking during pregnancy is roundly frowned upon, for good reason. It's a huge risk factor for lots of post-natal problems. But prevention can be counter intuitive. Strong prohibitions and punitive measures for pregnant women who drink mostly just have the effect of driving women away from prenatal care. We look at the literature.

I only see it being a vicious cycle of lying to people telling them that they are smarter than they actually are. @@AM-bj7yo no, that's completely wrong.
By calling it a drug and pushing it out of mainstream you're destroying healthy alcohol consumption culture.
US already has ridiculously high minimum alcohol age and all it does is push kids to do other drugs and treat alcohol as some technical liquid which they have to consume as much as possible.
You know who has very little problems with alcohol related deaths? France. Not because they ban alcohol, but because alcohol is mainstream and is woven into the culture, and people don't treat it as technical liquid to get shitfaced.

For the first ~11 weeks there is no direct mother to fetus blood-based nutrient transfer via the umbilical cord (Placenta, DOI: 10.1016/j.placenta.2015.01.002). The fetus survives of secretions of glucose and glycoprotiens. I'm not aware of any work into how alcohol interacts with those secretions, but it is likely not a significant direct source of alcohol to the fetus, as glandular secretions generally don't carry alcohol even when it's in the blood stream. However, alcohol may affect the glandular secretions in ways other than directly having alcohol in the secretions.

I can’t help but wonder how different things could be if instead of punishing pregnant women who test positive for alcohol or drugs, we tried helping them. Perhaps instead of putting them in jail we put them into a quality rehab or under a doctors care who specializes in substance abuse. My sister-in-law who was an alcoholic (and later abused drugs as well) actively avoided prenatal care & a hospital birth because she was afraid of being arrested. During pregnancy she’d quit pills, street drugs, & vastly lowered the amount of alcohol she drank or occasionally smoked marijuana. Don’t get me wrong, it’s horrible to abuse substances while pregnant & honestly, as glad as I am to have nephews a woman in her condition probably shouldn’t have had children (she had 4 total). But the whole time she was pregnant there was an attitude of disdain towards her sadly from many physicians & police. She kept her first two kids (now young adults), her third was adopted by his aunt & her fourth was adopted by a nice couple who couldn’t have children. Five years ago, my sister in law died of a heroin overdose when her “friends” left her alone in a car (while they partied) to choke to death until she lost consciousness & later died. She was 35. I can’t help but wonder how different things could have been for her, and her children if more people had offered help instead of punishment & threats. The “war on drugs” has been a massive failure & it needs to end. Addicts are people who need help, not punishment.

There were two recent terrible anti-drinking in pregnancy campaigns where I live. One was a glass shaped baby being filled up with wine with the caption "one drink for you is one drink for the baby" or something like that. Another was a photo of a crying child that said "its not only you you hurt when you drink." And I get what they are trying to do, but where I live, everyone knows alcohol in pregnancy is unsafe. The people who continue to drink are people with substance use disorders for whom a shame and fear based approach is a terrible way to go. We need more empathetic campaigns that talk about the risk but focus on where to go to get help, and what that help will look like.

The point is that it's a completely unnecessary risk.
Yeah, the child's health will likely be unaffected by the _occasional_ glass of wine or can of beer but like Aaron said: we don't know where the line is where it generally starts to get dangerous.
Why risk it? Is it really that hard to say no to alcohol for about a year, even if the health of your future child might be at stake? Because if it is, you have a problem with alcohol and/or self-discipline.
I agree that in cases that are not severe, it's excessive to threaten with CPS and such, but the avoidable danger that alcohol poses to the unborn child, as uncertain as it may be, should not be played down or even kept quiet about just to spare the woman from embarrassment and shame.

    • @mortalcoral2196

It’s not about sparing people from shame, it’s about being pragmatic. Like it or not some pregnant women will drink. With punitive laws, those women will not seek the counciling or medical help that could spare their children from a life of suffering with chronic deformities and illnesses. I agree it definitely shouldn’t be kept quiet, but that’s not what anyone suggested.

      You misunderstood.
"I agree that in cases that are not severe, it's excessive to threaten with CPS and such"
My point is that we shouldn't swing from "drinking any alcohol during pregnancy is child abuse and should be punished as such" all the way over to "Drinking during preganncy might be harmful, but we shouldn't stress that because then some women might not seek help". Yes, we should absolutely stress that. A lot of people are just that naive and/or or thick that they either don't know that or let their friends talk them out of it.
I agree witht the direction, it's just that in my opinion, his target is beyond of what's actually helpful and what'll create a whole lot of problems on their own.

I wasn't arguing "I turned out okay, so drinking while pregnant is fine." I said that neither of us have birth defects. The relationship between drinking and birth defects is mediated by our DNA and other environmental factors and we're lucky that our we had the right combination of protective factors.
My mom's doctor at the time was caught up with the scientific literature which implied drinking red wine can help the fetus. I think the mechanism was that a chemical found in red wine increased blood flow to the placenta, but my memory is a little hazy on that. But the point is that a doctor who is trained in medicine, and who is caught up on the scientific literature, is more often than not better able to make recommendations to their patients than legislators. There are exceptions, of course, but usually legislatures know less about a women's body than a doctor does.

A 2013 mete analysis of 1593 articles on the subject it's findings " The results from this analysis showed a small, beneficial association between mild‐to‐moderate prenatal alcohol exposure and child cognition. We suspect that this association was due to residual confounding as the association was no longer significant when considering only studies that controlled for SES." there was also a "moderate prenatal alcohol consumption at levels less than daily drinking might affect child behavior." but these differences are TINY both on the good and bad and put together, it seems reasonable to conclude that the effect are almost none.
But, the issue becomes more complex, as, even though the scientific data shows that moderate prenatal alcohol consumption has no real effect on outcomes, the recommendation that drinking a small amount while pregnant is fine may lead to an increase in binge drinking, which the scientific data shows is very harmful. An interesting idea that MD should not be entirely truthful with their patients to improve overall outcomes.

You wouldn’t give an infant an alcoholic beverage so why do you think it’s okay to drink while pregnant? Also many pregnant women are going to point to your video as an excuse to justify their behavior.

What we need is free birth control that you can't forget to take. The earth has enough people that we should be able to limit reproduction to people who make a conscious decision to have children. But that would be bad for the economy and the american political system because people who don't think before doing things that can have massive effects on their lives make the best consumers and voters.

Have you noticed that many of punitive policies are often directed at people that have historically not had political power and are under-represented in Government?

Women are not the only humans to be a gestational parent. Inclusive language is important.

That's not the point of the legislation. The point is that a woman who drinks ir smokes during pregnancy IS committing child abuse and they have forgone any right to raise their children. If it wasn't illegal, then these women could keep their kids, and those children will be trapped in an abusive household. Better to keep it illegal for the continued safety of those children

That only works if the system finds out, which it wont. The only realy effect its having its allowin more complications to occur due to bad prenatal care because of fear.

Because creating more instability in a kid's life - as opposed to getting their parent(s) the requisite medical treatment and therefore keep a stable family life - is much much less abusive *eyeroll*

Also, when it's expanding the poor outcomes in said children, your whole "but it protects the kids" argument goes to shit.

11.5% of women report drinking during pregnancy...But hey, you can just replace a child's parents, right? There's no potential harm there, and plus who would even care about a woman, soon to be a mother, with substance abuse?

My doctor said that stress was worse for the baby than on occasion having a glass of wine with dinner. It's not illegal where I live and I had maybe 2 drinks during each of my pregnancies. My children are healthy, loved and well cared for. They are much better off with me than who knows who in the system.
