I'm so very happy I came across your video. I was so much in pain walking, my nerve from top buttocks all the way to my below the knee. I did all the exercise as you directed. Thank God after a week or 2, I'm able to walk normal. Minimal pain as that occurs, I just do my exercise. It works. God bless you.
I want to cry. Finally someone has identified my problem. Sciatica aggravating the pudendal nerve. I will watch all your related videos and hope to solve my "pain in the butt" and in the groin area. Thank you so much for your videos.
I have watched soooo many videos on this subject, since I'm currently suffering badly and this, by far, is the most helpful. You've presented this information so usefully and have offered the best explanation both for the likely causes of it as well as the route out of it and I'm extremely grateful, thank you!
I second this!! He makes the most sense and he’s absolutely right about working on activating the glutes and core! I’ve had this yucky sciatica for 6 years or more, I just didn’t know what it was. It’s been getting worse. My glutes and core are extremely weak. I’m working those first!!
I was suffering from a terrible case of pes anserine bursitis for the past 7 years. Got treatment by several doctors and chiropractors and orthopaedics and even physical therapists, but unfortunately nobody was able to detect the problem, let alone treat it. I always thought I had an mcl tear or some ligament issue but a week ago, I found your videos on PAB and for the first time ever, I could relate and recognise the pain areas. I followed your exercises and a week later I'm a whole new person. I can walk. Sleep without pain. I'm overwhelmed and super grateful to you. Today I watched this video for my father who's suffering from sciatica and I hope it'll help him just as much as your videos helped me. You're truly a blessing. Thank you so much from the bottom of my heart 😊❤
Excellent video. 64 year old guy here, who has worked physically hard all my life, from professional drywall hanging, truck mechanics, home renovations, and other et cetera. My lower back muscles are likely over developed. I've had on-and-off problems with sciatica for decades. -Started in my 30 . . . ding! -Switches back and forth from leg to leg . . . ding! -Shots relieve it for a few months, then it comes back . . . ding! -Completely pain free just sitting around the house . . . ding! -Over-the-counter meds don't touch it . . . ding! -Chiropractic crunching had no effect or made it worse . . . ding! Thanks for the great information. I've begun working through your exercise playlist.
Hey Lugnut64052, we are so sorry to hear you've been dealing with this sciatica for so long! Glad to know you've been working on your exercises, please keep us posted on how you do!! Just in case you need it, we also have a 28-Day Back Health & Wellness Program. We'll leave the link right here below for you: "28-Day Back Health & Wellness Program" www.epmanualphysicaltherapy.com/back-health
I LOVE your videos! I have suffered from sciatica pain for 6 years. Tried meds, PTs, chiropractors, piriformus and SI joint injections, MRIs, etc etc. I have never been advised to strenghten the pelvic and glute muscles and will be trying that now. Keep making these videos as they are very informative and encouraging!
Hey Sammy, you are so welcome! We are glad that we were able to help you understand better. We would love to hear an update in the future about how these exercises work for you!
Been suffering for about 6 months with sciatica found your UA-cam two weeks ago and today I’ve actually got out of my house and walked a mile. Many thanks for how you explain things.
Oh crap, I have been doing a few of these exercices I have seen by other physiotherapists ….but you said they might worsen the sciatica ….ouch! I will check your other videos. Merci
You have given me so much help dealing with my sciatica. It is like you really understand what it feels like and you know how to treat it. Thank you so much for being there and sharing. BTW the video on exercises on how to fire the glute and lower abs has been the MOST helpful!
Hey Eliana, we are so glad that we have been able to help you! And we agree that those two areas usually need the most work for someone who is experiencing sciatica pain!
I had left leg sciatica pain last year...went to physical therapy and after a couple months it went away.. however the left side of my rear is still sore. I do the exercises you show for left hip and left butt which really help. I want to do all my therapy at home 😅. Thanks a million 😅
An exceptional explanation!❤ thank you so much. No one has ever taken the time to explain all these possibilities in the seven years I’ve been in pain. Great work! I wish I lived in El Paso so I could get treatment with you and your team!
I slipped over many months ago and hurt my back. From there I fell over trying to get up from the toilet. I think that's when I did the most damage. The pain has gotten so bad that I cannot walk unassisted or even shower myself. I haven't been to a Dr because I am in a foreign country without insurance. Anyway I think the problem is definitely sciatica as I have had prolapsed discs in the past. I am hoping with rest the inflammation will be reduced. I am taking tramadol which doesn't seem to even put a dent in the pain. This is the best video I have watched in regards to sciatica. Thanks so much.
All the exercises/stretches you mentioned were all the ones my Pt had prescribed to me…I will now look at the playlist you’ve added to give them a try. Thank you for all the explanations!!
Hi Anne, The information we are sharing here is not common knowledge as these are innovative techniques addressing muscle imbalances. We are so sorry that you've been struggling with sciatica. The pain from sciatica is no joke and it can be torture. Thank you so much for the kind words and we hope you can find some relief soon! In case you are interested, we have a playlist for sciatica pain help. We'd love to hear how you are doing as you progress.🙌 Take care!
I am so glad that I found your channel! The way you explain things and the information you provide has been truly invaluable; And even though, it can't replace seeing a doctor in person and getting an actual diagnosis, it's helped to pinpoint and make sense of where my pain/sensations are coming from. There are so many, "physicians" out there that just won't impart knowledge because they want to make some easy money off someone who is none the wiser. Thank you sir! Wish more people were like you! watching from South Africa.
Hey Myron, you are so welcome! And you are absolutely right! We love that what we do is so useful, but unfortunately it cannot replace what we do in-person. Thank you for the kind words!
I really appreciate the good work you are doing, you are so down to earth. I have benefited greatly from your explanation on the various aspects of your recommendations on this subject. Please keep up the good work
This video is gold. The best ! I've been suffering so much for the past 4 months. The Dr. says i have herniation but they are not compressing. But i've been suffering from ciatic, from the butt till the foot. Its awful.
Great stuff, off to the Chiropractic this morning for a second session looking at my sciatic pain. These videos are a good insight into what my problem might be. I'm hoping it's a lumbar and abdominal region muscle imbalance.Thank you.
Nice explanation I been fighting sciatica pain due to L4 & L5 herniated disc but since I started taking vitamin D regularly my back pain is no longer bothering.
I’ve been dealing with back pain/sciatica since 2019. These past couple months i’ve been having bad nerve pain that goes down my left leg which causes weakness and trouble walking at times.

thanks so much, you have saved me from myself! like some of the others commenting, i watched soooo many vids on this. nsaids seemed to take a long time to take affect, but did help. i was concerned at how often i was taking them. i've started to use ice packs, which works quite well for now. in a day or two i will see a chiro, maybe just one or two times. thanks again for the clarity!

Hi Novlet, we are so sorry that you are experiencing so much pain. With sciatica especially, we understand how painful and frustrating it is to deal with and to treat. We definitely don't recommend working through pain; soreness and mild discomfort are normal, but pain is not. If you did want to attempt to do any of the exercises Start with one or two exercises, beginning with the smallest range of motion and the lowest intensity. As you feel stronger, you can gradually increase both the range of motion and intensity. "Sciatica Pain Exercises and Stretches" ua-cam.com/play/PLJVIPiFvv1maZPQPPtirn4oxj8oCPuIMR.html Inversion Tables can also be very beneficial to sciatica symptoms. We have a video discussing how it can be used for back pain. We highly recommend researching Inversion Tables to see if it might be a good option for you. “Is an Inversion Table Helpful for Back Pain?” ua-cam.com/video/j61ImGQdFTM/v-deo.html We hope this helps and that your MRI results are able to provide some insight! We're hoping you are able to find some relief soon 🧡
I’m 77 years old with scoliosis. I have two Harrington rods in my back full length of my back in 2000. About five days ago, I woke up with pain that wouldn’t allow me to walk at all. It was the worst pain I have ever felt in my life. I take pain relievers just so I can move and do things I need to do. I’ve got an appointment with the doctor so I will ask about it first. But I need to mask it temporarily because I can’t take it. It goes all the way down to my leg to my feet ankles. The pain that’s in the lower part of my leg actually causes the front of my leg and the side of my leg down to the ankle hurt like hell. When I take something to help with the pain, it helps so I can do things that I have to do are very important. I have to feed my dog at a certain time because he’s diabetic and I need to give him insulin. I get up two hours prior and move around with some pain meds and it will fade well enough for me to take care of my little dog. After about two hours it is back full bore of pain again. I take some Tylenol, it fades it a little bit. Well you know what I’m talking about. Where I live in California you can’t just call up your doctors office and say can I come in tomorrow? It’s likely to be a week or two. Thank you for your time.
Thank you!!!!! Love you ! You helped me understanding much better what may be happening in my body though i still have some doubts😢after some ecografía/mmr they found out 3 years ago it had semi ruptured in the isquiios ( had already some lombar pain too) no much help from those doctors , i improved myself with some exercises but since few months ago i sterted with really bad lombar pain , i think my hips are compressed too , please advice me what to do😢😊 happy to havebfound you!!!!
Thank you so much for these videos! My sciatica is caused by SI weakness and is causing foot pain and cramping in toes. You mentioned getting feet stronger and I am doing the short foot exercise is addition to the recommended glute and abs. Any other foot videos? Can’t find them on the channel. Thank you!!
Oh no….I have been doing the figure 4 exercice and knee press, leg extension and more found on internet by other therapists…..and thought it was helping. Crap. I know the figure4 seems to stretch and help a bit but you say it might worsen the prob. I will check your other videos. Merci.❤
I have pain in lower back buttock and at night cant lie on my side as the pain on my left thigh and side of knee is intolerable. I cant sleep on my side. Is this Sciatica? Ps I find your videos really interesting and helpful. Thankyou😊
2:30pmMt Aug 29. Wow, you are always one step ahead of my "team". We will get me back up and about eventually. I am scheduled for knee replacement soon (really no way around it- my knees are shredded). I am barely walking and have "thrown out" my back and I know the sciatic is involved as a result, but the medical focus us on spine fusion. I will ask the PT to view your video. Thank you

@@epmanualphysicaltherapy thank you so much! I’ve been experiencing so much sciatica pain lately 😖 that my doctor wants me to have a steroids injection (never had one before). What are your thoughts on this please? Thanks 🙏🏻
I don't really have back pain so much the pain I have only happens when I lay down and it is a burning and hot sensation in my upper butt and all over both of my legs. Will strengthening the hips core and glutes help? The pain goes away once I stand up and walk. I do put a pillow under my knees but that does not seem to help the nerve pain so much. I have herniation in my L3, L5 and S1
Hey Jessica! So sorry to hear about your sciatica pain! :-( It is important to look for acute relief methods, but they don't always work the same for everyone, this is why it's important to begin fixing the root issue as soon as possible. His specialty is orthopedic manual physical therapy, which is the ideal specialty for helping someone avoid surgery, injections, and medications. Traditional physical therapists are best trained to help people right after surgery.
Hey Dan, sciatica is often caused by muscle imbalances. The abductor and adductor muscles can contribute to this muscle imbalance and is not recommended initially. Once the sciatica pain is gone, then these exercises can be resumed as long as they do not bring back the sciatica. Hope this helps!
Pam here….do you know anything about Ankylosing Spondylitis? Keith just diagnosed with this. Apparently some fusing happening in SI joints and low back. They want to do simponi aria infusions to stabilize???
Hey Pam, sorry to hear about Keith being diagnosed with Ankylosing Spondylitis :( This is a tough condition to deal with. Important background information is that AS is an autoimmune disease. This means that your body is attacking itself. The purpose of the simponi aria infusions is to help slow or stop the progression of the autoimmune response. We are not sure how well the medication actually works or how bad the side effects are. This would be a great conversation to have with your Medical Doctor. Hope this helps!
You mentioned in one of your videos that you have a video on how to walk and use your glutes. Can you tell me which one that is? I was unable to find it. Thank you.

I have been suffering from pain on my right buttock for over a year. It’s constant pain. Some days it’s worse than others. I have moderate levoconvex scoliosis. I start physical therapy next week. I have to do physical therapy before my insurance will cover an MRI. I wish there was more information on si joint issues with scoliosis.
Been at this for almost a year now. Did conservative treatment with chiropractor, three steroid injections since December 2022. Insurance Company denied a MRI. They want at least six weeks of documented physical therapy. Sciatica down both legs, left foot is numb most of the time. I am never not in pain.
Doctor . I live in Chicago I sit for living over 5 years sometimes up 12 hours ... I don’t longer work sitting that long but still. Today has been a year since the pain started. They did 3 mri. Hip, lower back and sacroiliac joint. Hip and SJ shows no abnormalities, but lower back shoes disc dehydration and annular tear L5 S1 . My pain started like in my groin. Lower abdomen like pinched and needles after a dull ache pain while sitting working. Also like pain around my genitals left side, then like causing pooping and clicking in different part of my hip and down the leg. When I lift my leg up. Laying face down a bad pain flares up where leg and glute meets. I tried piriformis injections steroids and didn’t work. Nothing helps the pain doctor thinks the L5 S1 finding is not the cause of the pain .. what I should do ? I wush I could find someone with some relief
Hey Kathy, some people find relief with this method. It is not 100% effective, but it may be useful in combination with stretches and exercises! I've been dealing with this for quite a while. Question: Is there a definitive/obvious way to differentiate between sciatic nerve issues from stenosis or spine problems vs. piriformis syndrome?
Hey Charlie, great question! When standing, if you bend your back backwards and it makes it worse, then it's more than likely from stenosis. Hope this helps!
Could this pain come grom a cervical spine issue? When I tilt my head back and reach up above my head I get that intense pain from around my glutes to my hamstring area.
@@topgun12fly Wow, that's quite an interesting question to me. I have cervical central canal stenosis. I been wondering if things I'm experiencing are related to that.
Can you make a video as to why people has pain months after getting arthroscopic surgery . I'm in my 40s and I still getting pain on my knee after almost 3 months of surgery of articular cartilage tear.

Hey K! Yes, it can be overwhelming with so many videos on that playlist...but sciatica is a complicated condition. Video #1 on the playlist is the best place to start. Then work your way down the list. You might find you need to jump around on the list as you begin to learn more and need to find something specific. Here's the link to the playlist just in case you need it: "Sciatica Pain Help” ua-cam.com/play/PLJVIPiFvv1maZPQPPtirn4oxj8oCPuIMR.html
NO, My MD, told me my symptoms were not consistent with the MRI, I could not walk across the street. The best chiropractor, confirmed by PT, my hips were not level, and my upper back twistef. With the first adjustment, my cognitive, vision and the urgency to urine immediately improved. All the exercises I was told to do, you say, never do.
Hey there, sciatica is very commonly caused by a rotation or torsion in your pelvis causing your hips to not be level. The adjustments, when done correctly, can greatly improve your symptoms! The exercises we say to avoid are exercises that would aggravate sciatica symptoms if somebody is already dealing with constant pain. Some of these exercises would be beneficial after an adjustment, however, we try to guide people who are unable to receive these services. Hope this clears things up!
Doctor, According to my doctor. I am fit to work. I have mild disc buldge. But I am constantly in pain. Both sides of legs go numb but it's on and off. I am walking, doing my house hold work, doing household work. Can you suggest me what should I do. Physiotherapy like traction and IFT works. But I am feeling pain for two months now. Please suggest.

Hey Petie, this is a tough question to answer. Stem cells are a rather new treatment method within the medical field and do not have enough research to make a definitive decision on whether this is a good treatment method. Sorry we could not provide a more targeted answer.

Hey there, sorry to hear that you are having this sciatica-like pain :( this can happen for a number of reasons, which makes it difficult to determine exactly what is going on. We have a great playlist of videos that may be able to help if you are interested in learning more. Here is a link: "Sciatica Pain Exercises & Stretches" ua-cam.com/play/PLJVIPiFvv1maZPQPPtirn4oxj8oCPuIMR.html We would love to hear from you in the future if something new develops!
Hey Eraina, the research on laser treatment is inconclusive. What we support here is addressing the root cause of the problem, rather than just trying to get rid of the pain. With sciatic pain, the root cause is very often a muscle imbalance that is causing a misalignment of your hip complex. We have a great playlist of videos that can help to regain proper mechanics in the hip region to help abolish the symptoms associated with sciatica. Here is a link in case you are interested in learning more: "Sciatica Pain Exercises and Stretches" ua-cam.com/play/PLJVIPiFvv1maZPQPPtirn4oxj8oCPuIMR.html
I thought I’d wear 8” stiletto platforms for 26 years n then transition into trucking after the clubs starting closing down. The weight of the safety shoes + climbing in n out of the truck to learn how to back the trailer totally destroyed me. I’m off work now unable to walk for more than a few minutes. I have an MRI booked for Jan🤞
I've had 3 c sections in a period of 5 years. After the birth of my last born son in 2020 my tubes were tied. Since then i have this terrible pain everyday.
Hey @msxatasi4825! We are so sorry to hear you've been in pain for this long 🙁 Glad you are here so we can help! Changed car 2 was ago...unfortunately the driver's seat is a sports seat aka a bucket seat.....wow...7 days in and sciatica has hit.... what is the best position for sleep please.

Hey Rashmi 👋! It is not unlikely that you could be experiencing pain in the back of your calf due to a sciatica issue but we can't 100% say as we don't fully know the extent of you diagnosis. Sciatica pain in regards to the calf shouldn't cause any swelling or redness so we'd suggest to make sure you aren't experiencing any of the two. We also recommend giving the stretches/exercises in the video a try and if your pain seems to improve then that could possibly answer your question. Also we'd like to leave you with some additional resources, you can access it here: "Sciatica Pain Exercises & Stretches" ua-cam.com/play/PLJVIPiFvv1maZPQPPtirn4oxj8oCPuIMR.html

@epmanualphysicaltherapy thank you! This is what a surgeon suggested. I have hEDS and would like to avoid surgery. "Her MRI shows a grade 1 spondylolisthesis at L3/4 with moderate stenosis. We discussed a right L3/4 anterolateral lumbar interbody fusion/posterolateral arthrodesis/Instrumentation with a right L3/4 laminectomy."
When I fell a few weeks ago and went up to a&e I told them I had been to my gp a few weeks b4 I fell and I had sciatica in my back the doctors in a&e told me it wouldn't have b in my back but in my leg and hip
Hey Sue! We are terribly sorry about the falling accident you had a few weeks ago. The thing about Sciatica is that it's a complex issue, the pain can radiate from the back all the way down to the leg and sometimes the calf, in more progressed cases. This is better explained in this video for you: "Sciatica's SECRET ROOT PROBLEM!" ua-cam.com/video/abBIdtf05-8/v-deo.htmlfeature=shared You can also check out all the information we have on sciatica and all the things we suggest to do and not do here: "Sciatica Pain Exercises and Stretches" ua-cam.com/play/PLJVIPiFvv1maZPQPPtirn4oxj8oCPuIMR.html

I LOVE your videos! I have suffered from sciatica pain for 6 years. Tried meds, PTs, chiropractors, piriformus and SI joint injections, MRIs, etc etc. I have never been advised to strenghten the pelvic and glute muscles
and will be trying that now.
Great to hear Tony! keep up the good work!!!!!!!!!!!!
I need exercises to help strengthen muscle development.
How????
Oh crap, I have been doing a few of these exercices I have seen by other physiotherapists ….but you said they might worsen the sciatica ….ouch! I will check your other videos. Merci
My pinky toe@epmanualphysicaltherapy my pinky toe is numb what could I do for relive left leg
Hey there, you are so welcome! Thank you for the kind words! We appreciate the support and would love to hear an update in the future!
Me too!
7 years for me too😢
Spondylolisthesis for me
I slipped over many months ago and hurt my back.

Also from SA🇿🇦with many thanks!
I really appreciate the good work you are doing, you are so down to earth. I have benefited greatly from your explanation on the various aspects of your recommendations on this subject. Please keep up the good work
Hey Daniel, thank you for the kind words! We really appreciate the support and love to provide this service to the world!
Hey Hassan, that is awesome that you found something that was able to help you ease your pain!
Really? How does vitamin d help with sciatica or back pain? Just curious
I’ve been dealing with back pain/sciatica since 2019. These past couple months i’ve been having bad nerve pain that goes down my left leg which causes weakness and trouble walking at times.

Thank you so much. I am doing your exercises but I am in so much pain so I will be doing an MRI
Hi Novlet, we are so sorry that you are experiencing so much pain. With sciatica especially, we understand how painful and frustrating it is to deal with and to treat. We definitely don't recommend
working through pain; soreness and mild discomfort are normal, but pain is not.
If you did want to attempt to do any of the exercises Start with one or two exercises, beginning with the smallest range of motion and the lowest intensity. As you feel stronger, you can gradually increase both the range of motion and intensity.

Start with one or two exercises, beginning with the smallest range of motion and the lowest intensity. As you feel stronger, you can gradually increase both the range of motion and intensity. It’s best to do these exercises every day throughout the day. Aim to tire out the muscle without losing form or control. Consistency is key. Always avoid working through pain; soreness and mild discomfort are normal, but pain is not.

Strengthening your glutes and core will put you on the path to long term relief! We are glad you are here :-) We have a playlist with several videos consisting of exercises and tips to help you remedy your sciatic nerve pain! Check it out:

I have not been able to find a fit with a physical therapy. Know any recommendation where I am. Shreveport, La
Hey Tammie, sorry to say we do not know any PTs in your area :( In the US, you can find a physical therapist trained similarly to Dr. David at this website: aaompt.org/Main/Public_Resources/Find-A-Fellow/Main/Member_Resources/Find-A-Fellow.aspx?hkey=108c9a56-f793-45e4-b3ed-cefe5d871a94
Dr. David is a fellow of the American Academy of Manual Physical Therapists. His specialty is orthopedic manual physical therapy, which is the ideal specialty for helping someone avoid surgery, injections, and medications.
Traditional physical therapists are best trained to help people right after surgery.
Love the channel. Are hip abductor and adductor resistance band exercises reasonable to try out with sciatica and/or hip arthritis ?
Hey Dan, sciatica is often caused by muscle imbalances. The abductor and adductor muscles can contribute to this muscle imbalance and is not recommended initially. Once the sciatica pain is gone, then these exercises can be resumed as long as they do not bring back the sciatica. Hope this helps!
@@epmanualphysicaltherapy - thanks very much - does the same idea apply to hip osteoarthritis in general ?

Thank You for the detailed information..
You seem really good.. You wouldn't practice in NYC area? Would you. I need someone really good.❤😢
What do you think of dry needling with stimulation to treat the piriformis syndrome?
Hey Kathy, some people find relief with this method. It is not 100% effective, but it may be useful in combination with stretches and exercises! Here is a playlist of exercises for sciatica, which can be due to piriformis syndrome:

Is the emergency mat works to end sciatica?

Thank you ❤

What do you think about stem cells for sciatica plroblems?

What do you think about laser treatment
Hey Eraina, the research on laser treatment is inconclusive. What we support here is addressing the root cause of the problem, rather than just trying to get rid of the pain. With sciatic pain, the root cause is very often a muscle imbalance that is causing a misalignment of your hip complex. We have a great playlist of videos that can help to regain proper mechanics in the hip region to help abolish the symptoms associated with sciatica. Does walking help sciatica pain?it's been over two months I still can't sit
My pains seems to be worse. What can I do
Can it be side of the calf... Rather than back of it?
Hey Rashmi 👋! It is not unlikely that you could be experiencing pain in the back of your calf due to a sciatica issue but we can't 100% say as we don't fully know the extent of you diagnosis. Sciatica pain in regards to the calf shouldn't cause any swelling or redness so we'd suggest to make sure you aren't experiencing any of the two. We also recommend giving the stretches/exercises in the video a try and if your pain seems to improve then that could possibly answer your question.
